| 2 | 针刺太冲穴得气及疼痛激发相对抗的脑功能网络效应fMRI研究显示文摘目的:采用磁共振脑功能成像技术,比较针刺太冲穴得气及针刺疼痛时所激发的脑功能网络效应异同。方法:47例初次接受针刺的志愿者参加手针右侧太冲穴fMRI实验。针刺时脑fMRI扫描10min,共2次捻针,各2min,间隔3min。其中17例接受体表触觉刺激作为针刺对照,分为3组:①针刺得气组;②针刺得气合并尖锐性疼痛组(简称针刺疼痛组);③触觉对照组。采用Fisher’s精确检验和t检验分析感觉频率及强度;用AFNI处理fMRI数据,得到激活、负激活脑区;用种子相关分析法,以前扣带回膝部为种子点,分析其与全脑的功能网络联系图谱,并比较得气与疼痛组之间的差异。结果:针刺2组的酸痛、酸感、压感、麻感、钝痛等得气感觉的频率和强度均明显强于触觉刺激组;针刺2组间除尖锐性疼痛及酸痛外,各种感觉差异无统计学意义。针刺得气及疼痛2组均激活了体感及丘脑岛叶皮层,重要的是,针刺得气时在大脑边缘叶-旁边缘叶-新皮层系统(LPNN)产生了较强而广泛的负激活区,加强了负激活区之间的脑功能网络联系。此网络包括杏仁核、海马、旁海马、颞极、前额叶腹内侧回、前扣带回膝及下部、前楔叶内侧回和后扣带回(BA31,23_腹侧核,29,30),另外,小脑蚓部、导水管周围灰质及脑干网状结构也见负激活现象。然而,在针刺疼痛时,这些脑区负激活程度减低且区域缩小,大部分脑区fMRI信号逆转为激活信号,被疼痛激活的脑区之间功能网络明显增强。这些脑区参与了疼痛、镇痛(如感觉、情感、认知、自主性、内源性镇痛)功能调制环路。结合既往研究表明,负激活与脑血氧消耗及血流量下降呈正相关。本结果支持针刺通过调制LPNN和疼痛中枢网络的活动,产生其镇痛、抗焦虑和其他调节效应的假说。结论:针刺得气及针刺得气伴随疼痛产生了相对抗的脑功能网络效应。针刺得气对LPNN及疼痛脑中枢网络(Pain matrix)的负激活效应,可能与针刺镇痛脑中枢机制相关。LPNN和静息态默认网络(Default mode)之间的相似性,提示针刺需通过这一内在脑功能网络系统发挥其广泛的调制作用。 | 方继良 Hui KS Kathleen Liu Jing Nixon Erika 周科华 王小玲 朱兵 | 2012 | 中国中西医结合影像学杂志2012,10,1: | 52 |
| 3 | Beneficial effects of naringenin in liver diseases: Molecular mechanisms显示文摘Liver diseases are caused by different etiological agents, mainly alcohol consumption, viruses, drug intoxication or malnutrition. Frequently, liver diseases are initiated by oxidative stress and inflammation that lead to the excessive production of extracellular matrix(ECM), followed by a progression to fibrosis, cirrhosis and hepatocellular carcinoma(HCC). It has been reported that some natural products display hepatoprotective properties. Naringenin is a flavonoid with antioxidant, antifibrogenic, anti-inflammatory and anticancer properties that is capable of preventing liver damage caused by different agents. The main protective effects of naringenin in liver diseases are the inhibition of oxidative stress, transforming growth factor(TGF-β) pathway and the prevention of the transdifferentiation of hepatic stellate cells(HSC), leading to decreased collagen synthesis. Other effects include the inhibition of the mitogen activated protein kinase(MAPK), toll-like receptor(TLR) and TGF-β non-canonical pathways, the inhibition of which further results in a strong reduction in ECM synthesis and deposition. In addition, naringenin has shown beneficial effects on nonalcoholic fatty liver disease(NAFLD) through the regulation of lipid metabolism, modulating the synthesis and oxidation of lipids and cholesterol. Moreover, naringenin protects from HCC, since it inhibits growth factors such as TGF-β and vascular endothelial growth factor(VEGF), inducing apoptosis and regulating MAPK pathways. Naringenin is safe and acts by targeting multiple proteins. However, it possesses low bioavailability and high intestinal metabolism. In this regard, formulations, such as nanoparticles or liposomes, have been developed to improve naringenin bioavailability. | 4 | Management of negative pressure wound therapy in the treatment of diabetic foot ulcers显示文摘Diabetic foot(DF) is a common complication of diabetes and the first cause of hospital admission in diabetic patients. In recent years several guidelines have been proposed to reinforce the the management of DF with a notable increase in diabetes knowledge and an overall reduction of amputations. Significant improvements have been reached in the treatment of diabetic foot ulcers(DFUs) and nowadays clinicians have several advanced medications to apply for the best local therapy. Among these, negative pressure wound therapy(NPWT) is a useful adjunct in the management of chronic and complex wounds to promote healing and wound bed preparation for surgical procedures such as skin grafts and flap surgery. NPWT has shown remarkable results although its mechanisms of action are not completely understood. In this paper, we offer a complete overview of this medication and its implication in the clinical setting. We have examined literature related to NPWT concerning human, animal and in vitro studies, and we have summarized why, when and how we can use NPWT to treat DFUs. | 6 | Multidrug-resistant bacterial infections after liver transplantation: An ever-growing challenge显示文摘Bacterial infections are a leading cause of morbidity and mortality among solid organ transplant recipients.Over the last two decades,various multidrug-resistant(MDR)pathogens have emerged as relevant causes of infection in this population.Although this fact reflects the spread of MDR pathogens in health care facilities worldwide,several factors relating to the care of transplant donor candidates and recipients render these patients particularly prone to the acquisition of MDR bacteria and increase the likelihood of MDR infectious outbreaks in transplant units.The awareness of this high vulnerability of transplant recipients to infection leads to the more frequent use of broad-spectrum empiric antibiotic therapy,which further contributes to the selection of drug resistance.This vicious cycle is difficult to avoid and leads to a scenario of increased complexity and narrowed therapeutic options.Infection by MDR pathogens is more frequently associated with a failure to start appropriate empiric antimicrobial ther-apy.The lack of appropriate treatment may contribute to the high mortality occurring in transplant recipients with MDR infections.Furthermore,high therapeutic failure rates have been observed in patients infected with extensively-resistant pathogens,such as carbapenemresistant Enterobacteriaceae,for which optimal treatment remains undefined.In such a context,the careful implementation of preventive strategies is of utmost importance to minimize the negative impact that MDR infections may have on the outcome of liver transplant recipients.This article reviews the current literature regarding the incidence and outcome of MDR infections in liver transplant recipients,and summarizes current preventive and therapeutic recommendations. | Guilherme Santoro-Lopes Erika Ferraz de Gouvêa | 2014 | World Journal of Gastroenterology2014,20,20: | 24 |

| 9 | Specific probiotics alleviate allergic rhinitis during the birch pollen season显示文摘AIM:To investigate whether birch pollen allergy symptoms are linked with gut microbiota changes and whether probiotics have an effect on these.METHODS:Forty seven children with confirmed birch pollen allergy were randomized to receive either a probiotic combination of Lactobacillus acidophilus (L.acidophilus) NCFMTM (ATCC 700396) and Bifidobacterium lactis (B.lactis) Bl-04 (ATCC SD5219) or placebo in a double-blind manner for 4 mo,starting prior to onset of the birch pollen season.Symptoms were recorded in a diary.Blood samples were taken for analysis of cytokines and eosinophils.Fecal samples were analysed for microbiota components,calprotectin and IgA.Nasal swabs were taken for analysis of eosinophils.RESULTS:The pollen season induced a reduction in Bifidobacterium,Clostridium and Bacteroides which could not be prevented by the probiotic intervention.During the intervention,significantly higher numbers of B.lactis 11.2×107 ± 4.2×107 vs 0.1×107 ± 0.1×107 bacteria/g feces (P<0.0001) and L.acidophilus NCFMTM 3.5×106 ± 1.3×106 vs 0.2×106 ± 0.1×106 bacteria/g feces (P<0.0001) were observed in the probiotic group compared to the placebo group.During May,there was a tendency for fewer subjects,(76.2% vs 95.2%,P=0.078) to report runny nose,while during June,fewer subjects,11.1% vs 33.3%,reported nasal blocking in the probiotics group (P =0.101).Concomitantly,fewer subjects in the probiotic group had infiltration of eosinophils in the nasal mucosa compared to the placebo group,57.1% vs 95% (P=0.013).Eye symptoms tended to be slightly more frequent in the probiotic group,12.5 d [interquartile range (IQR) 6-18] vs 7.5 d (IQR 0-11.5) (P=0.066) during May.Fecal IgA was increased in the placebo group during the pollen season;this increase was prevented by the probiotics (P=0.028).CONCLUSION:Birch pollen allergy was shown to be associated with changes in fecal microbiota composition.The specific combination of probiotics used was shown to prevent the pollen-induced infiltration of eosinophils into the nasal mucosa,and indicated a trend for reduced nasal symptoms. | Arthur C Ouwehand Merja Nermes Maria Carmen Collado Nina Rautonen Seppo Salminen Erika Isolauri | 2009 | World Journal of Gastroenterology2009,15,26: | 19 |

| 11 | Pneumatosis cystoides intestinalis following alpha-glucosidase inhibitor treatment:A case report and review of the literature显示文摘A 69-year-old man was diagnosed as having myasthenia gravis (MG) in September 2004,and treated with thymectomy and prednisolone. He was then diagnosed as having steroid-induced diabetes mellitus,and received sulfonylurea (SU) therapy in May 2005. An alpha-glucosidase inhibitor (αGI) was added in March 2006,resulting in good glycemic control. He experienced symptoms of abdominal distention,increased flatus,and constipation in October 2007,and was admitted into our hospital in late November with hematochezia. Plain abdominal radiography revealed small linear radiolucent clusters in the wall of the colon. Computed tomography (CT) showed intramural air in the sigmoid colon. Colonoscopy revealed multiple smooth surfaced hemispherical protrusions in the sigmoid colon. The diagnosis of pneumatosis cystoides intestinalis (PCI) was made on the basis of these findings. As the αGI voglibose was suspected as the cause of this patient's PCI,treatment was conservative,ceasing voglibose,with fasting and fluid supplementation. The patient progressed well,and was discharged 2 wk later. Recently,several reports of PCI associated with αGI therapy have been published,predominantly in Japan where αGIs are commonly used. If the use of αGIs becomes more widespread,we can expect more reports of this condition on a global scale. | 13 | 紫坪铺水库造成孔隙弹性耦合变化及其对2008年汶川地震触发作用显示文摘位于川西地区龙门山断裂带附近的紫坪铺水库于2005年9月开始蓄水.约2.7年后,2008年5月12日,Mw7.9级汶川地震在龙门山断裂带上发生,两事件在时间和空间上的接近,揭示其可能相互关联,但前人的诸多研究给出了不同甚至是相反的结果.本研究基于完全耦合孔隙弹性理论,利用二维有限元模型(FEM),模拟水库蓄水造成的区域孔隙压力场和应力场的演化过程,基于库仑应力演化探讨其对龙门山断裂带活动的影响.模拟结果显示紫坪铺水库蓄水打破了原来的区域孔隙压力平衡,形成孔隙压力梯度源,向周围地壳传播;进而造成龙门山断裂带上库仑应力正值范围不断扩大,由浅入深影响到整条断层,尤其对浅层范围的加载作用明显,达上百千帕,为整个断层面的失稳提供了基础.震源区域库仑应力呈持续增长趋势,汶川地震发震前,增长了约数千帕-数十千帕,即使初期库仑应力为负,在未来某时刻库仑应力仍可能由负转正,并不断增大.通过计算汶川地震震源及其附近区域内相对于多种可能断层倾角的库仑应力,发现库仑应力随断层倾角增大而增加.因此整体来看,紫坪铺水库蓄水对龙门山断裂带起加载作用,有可能触发地震.对紫坪铺库区周围的小震分析也显示,蓄水以来小震明显增多、且不同时段小震发生密集区与水库距离逐渐增大,与孔隙压力扩散趋势一致.以上结果表明紫坪铺水库的蓄水增加了汶川地震的危险性. | 陶玮 Timothy Masterlark 沈正康 Erika Ronchin 张永 | 2014 | 地球物理学报2014,57,10: | 13 |

| 16 | Role of non-steroidal anti-inflammatory drugs on intestinal permeability and nonalcoholic fatty liver disease显示文摘The use of non-steroidal anti-inflammatory drugs(NSAIDs) is widespread worldwide thanks to their analgesic, anti-inflammatory and antipyretic effects. However, even more attention is placed upon the recurrence of digestive system complications in the course of their use. Recent data suggests that the complications of the lower gastro-intestinal tract may be as frequent and severe as those of the upper tract. NSAIDs enteropathy is due to enterohepatic recycling of the drugs resulting in a prolonged and repeated exposure of the intestinal mucosa to the compound and its metabolites. Thus leading to so-called topical effects, which, in turn, lead to an impairment of the intestinal barrier. This process determines bacterial translocation and toxic substances of intestinal origin in the portal circulation, leading to an endotoxaemia. This condition could determine a liver inflammatory response and might promote the development of nonalcoholic steatohepatitis, mostly in patients with risk factors such as obesity, metabolic syndrome and a high fat diet, which may induce a small intestinal bacterial overgrowth and dysbiosis. | 18 | Curcumin and Apigenin – novel and promising therapeutics against chronic neuroinflammation in Alzheimer's disease显示文摘Alzheimer's disease is a progressive neurodegenerative disorder, characterized by deposition of amyloid beta, neurofibrillary tangles, astrogliosis and microgliosis, leading to neuronal dysfunction and loss in the brain. Current treatments for Alzheimer's disease primarily focus on enhancement of cholinergic transmission. However, these treatments are only symptomatic, and no disease-modifying drug is available for Alzheimer's disease patients. This review will provide an overview of the proven antioxidant, anti-inflammatory, anti-amyloidogenic, neuroprotective, and cognition-enhancing effects of curcumin and apigenin and discuss the potential of these compounds for Alzheimer's disease prevention and treatment. | 19 | Genetic, metabolic and environmental factors involved in the development of liver cirrhosis in Mexico显示文摘Liver cirrhosis(LC) is a chronic illness caused by inflammatory responses and progressive fibrosis. Globally, the most common causes of chronic liver disease include persistent alcohol abuse, followed by viral hepatitis infections and nonalcoholic fatty liver disease. However, regardless of the etiological factors, the susceptibility and degree of liver damage may be influenced by genetic polymorphisms that are associated with distinct ethnic and cultural backgrounds. Consequently, metabolic genes are influenced by variable environmental lifestyle factors, such as diet, physical inactivity, and emotional stress, which are associated with regional differences among populations. This Topic Highlight will focus on the genetic and environmental factors that may influence the metabolism of alcohol and nutrients in the setting of distinct etiologies of liver disease. The interaction between genes and environment in the current-day admixed population, Mestizo and Native Mexican, will be described. Additionally, genes involved in immune regulation, insulin sensitivity, oxidative stress and extracellular matrix deposition may modulate the degree of severity. In conclusion, LC is a complex disease. The onset, progression, and clinical outcome of LC among the Mexican population are influenced by specific genetic and environmental factors. Among these are an admixed genome with a heterogenic distribution of European, Amerindian and African ancestry; a high score of alcohol consumption; viral infections; a hepatopathogenic diet; and a high prevalence of obesity.
