| 1 | Association between low molecular polypeptide 7 single nucleotide polymorphism and response to therapy in hepatitis C virus infection显示文摘AIM: To investigate the relationship between low molecular polypeptide-7 (LMP-7) gene polymorphism and response to interferon (IFN) therapy in chronic hepatitis C virus (HCV) patients. METHODS: LMP-7 polymorphism at codon 49 with nucleotide substitution from A to C was amplified in 104 chronic HCV patients of genotype 4. The amplicons were digested with restriction endonuclease Bsm I and the produced restriction fragment length polymorphism was analyzed. Patients received IFN + regional blood volume therapy for 48 wk and the frequency of thissingle nucleotide polymorphism (SNP) was statistically correlated with treatment response. The exclusion criteria for these patients were stated by the national health program for treating viral hepatitis. Main exclusion criteria included co-infection with hepatitis B virus or schistosomiasis, thyroid dysfunction, uncontrolled diabetes mellitus, history of long term drug or alcohol intake and autoimmune hepatitis. Multivariate analyses were done to correlate LMP-7 SNP plus several factors such as age, gender, weight, serum alpha-fetoprotein (AFP) and alanine aminotransferase levels, liver activity, fibrosis score and viral load with response to therapy. RESULTS: The data presented in this study clearly demonstrated statistically significant differences between sustained virological response (SVR) (defined as the absence of HCV RNA levels in the patient's sera at least 6 mo after discontinuation of treatment) and non-response (NR) (where HCV RNA levels in the patient's sera never become undetectable for 6 mo during or after treatment). Variables were described as odds ratio with 95%CI. The data were considered significant if P values were ≤ 0.05; highly significant if P < 0.01 and very highly significant if P < 0.001. Current data showed that 91.7% of patients carrying LMP-7 C/C allele were associated with SVR, while the other two genotypes C/A and A/A were associated with NR patients, 83.3% and 64.3% respectively, showing that genotype CC was strongly associated with response to interferon (95%CI: 12.0719-134.6572, P = 0.0001). The majority of parameters recorded in SVR and NR patients included higher values of mean age (P = 0.004), alanine aminotransferase (P = 0.001), AFP (P = 0.001), body weight (P = 0.025), viral load (P = 0.025), higher fibrosis and histological activity index indices among NR vs SVR patients. Also, the multivariate statistical analysis of the different factors of fibro-sis score, liver activity grade, genotypes and alleles of LMP-7 gene polymorphism in responders and NRs of HCV patients in this study showed that HCV patients with A allele had a very highly significant association with the NRs, high fibrosis and higher liver activity, while the C allele had a very highly significant association with the responders, low fibrosis and lower liver activity (95%CI: 3.5800-13.2519, P = 0.0001).CONCLUSION: LMP-7 SNP is a candidate gene that should be considered when designing a mathematical model for predicting response to therapy and disease progression in HCV patients. | Moataza H Omran Basma E Fotouh Samar S Youssef Noha E Ibrahim Wael Nabil EL-Sayed M Mahdy Wafaa G Shosha Mostafa K El-Awady | 2013 | World Journal of Hepatology2013,5,3: | 4 |

| 10 | Online Postcolumn Indirect Detection for Determination of Ibandronate in Pharmaceutical Tablets by HPLC/DAD显示文摘Ibandronate sodium monohydrate is a highly polar aliphatic compound that belongs to the bisphosphonate class,a widely used bone resorption inhibitor.The aliphatic nature of ibandronate precludes direct photometric detection and its high polar-ity urged analysts to use ion-pairing agents in the mobile phase to promote retention in reversed-phase columns.In this work,a reversed-phase method for determination of ibandronate sodium monohydrate in pharmaceutical tablets is introduced by employing an online postcolumn ligand exchange reaction to enable photometric detection.The method offers for the first time the ability to separate the drug from its well-known impurities on conventional reversed-phase columns.The detection reac-tion depends on the ability of ibandronate and its degradation products to displace salicylate in the iron(III)salicylate complex,forming various colorless iron(III)complexes and showing a negative chromatographic signal for ibandronate and its degra-dants atλ_(max)=525 nm.The chromatographic separation was achieved using a Hypersil BDS C_(8) column(5μm particle size,150 mm×4.6 mm i.d.)and a mobile phase consisting of a mixture of aqueous formate buffer pH 3(pH 3,5 mM)and methanol in a ratio of 80:20%(v/v),delivered at a flow rate of 0.8 mL/min.The postcolumn iron(III)salicylate reagent was prepared at 250μM in aqueous formate buffer,delivered using a syringe pump at a flow rate of 0.5 mL/min.The proposed method was validated as per the guidelines of the International Conference on Harmonization Q2(R1)and was found linear over the range 40-120μg/mL(r=0.9995)with a limit of detection of 3.19μg/mL.Compared to currently reported methods,the proposed method is considerably simpler,faster,cheaper,and adequately sensitive and makes use of the most popular LC detectors. | Mokhtar Mabrouk Sherin F.Hammad Mohamed A.Abdelaziz Fotouh R.Mansour | 2019 | Journal of Analysis and Testing2019,3,4: | 0 |
| 11 | The safety,efficacy and pharmaceutical quality of male enhancement nutraceuticals bought online:Truth versus claim显示文摘Objective:Nutraceutical products are widely used for their claimed therapeutic benefits.However,falsified or adulterated nutraceuticals present a major health threat to consumers.This study investigates the pharmaceutical quality,safety and anti-inflammatory effects of six male enhancement nutraceuticals that claim to be 100%natural.Methods:Three batches of six male enhancement products were tested to detect the presence and levels of adulterants via high-performance liquid chromatography(HPLC).The pharmaceutical quality of the selected nutraceuticals was tested with near infrared spectroscopy(NIR)and Se De M.The cytotoxic effects of these products on HepG2 cells were determined through cell proliferation(XTT)and lactate dehydrogenase(LDH)cytotoxicity assays.Lastly,the in vitro inflammatory effects of these products were investigated using murine J774 macrophages through cytokine release analysis.Results:HPLC analysis detected the presence of sildenafil citrate,a vasodilator,and the active ingredient in Viagra and Revatio,in all batches of the products we analyzed.Amount of sildenafil citrate ranged from 0.45 mg to 51.85 mg among different batches.NIR assessment showed inter-and intra-batch heterogeneity in product composition.Results of the XTT and LDH assays showed significant cytotoxic effects of the analyzed products.XTT analysis revealed that the viability of HepG2 treated with tested products varied from 27.57%to 41.43%.Interestingly,the male enhancement products also showed anti-inflammatory effects.Conclusion:Despite their labeling as 100%natural,all products tested in this study contained levels of sildenafil citrate,which was not reported on the packaging.There was a lack of pharmaceutical uniformity among products of the same batch and across different batches.Additionally,the products we tested had cytotoxic effects.These study findings highlight the adulteration,poor quality and hazard of these nutraceuticals.Therefore,strict regulation of these products and standardization of the definition of nutraceuticals are urgently needed.Further,these falsely advertised products should be withdrawn from the market due to potential adverse effects on the health of their consumers. | Mohamed Ismail Nounou Heba AEassa Nada AHelal Khaled Aboul Fotouh Ihab Mansoor Isabel KLatz Cindy Zheng Hadeer AEassa Dina Mohamed Diana MHuynh Abigail RWiss Jessica Sweeney Monica TOakes Mark MMikhail Nadine Amine Harshvir Kaur Natalia Echeverry Kamila Orzechowski Doreen Szollosi | 2021 | Journal of Integrative Medicine2021,19,3: | 0 |
