6How to identify which patients with asymptomatic carotid stenosis could benefit from endarterectomy or stenting显示文摘Offering routine carotid endarterectomy(CEA)or carotid artery stenting(CAS)to patients with asymptomatic carotid artery stenosis(ACS)is no longer considered as the optimal management of these patients.Equally suboptimal,however,is the policy of offering only best medical treatment(BMT)to all patients with ACS and not considering any of them for prophylactic CEA.In the last few years,there have been many studies aiming to identify reliable predictors of future cerebrovascular events that would allow the identification of patients with high-risk ACS and offer a prophylactic carotid intervention only to these patients to prevent them from becoming symptomatic.All patients with ACS should receive BMT.The present article will summarise the evidence suggesting ways to identify these high-risk asymptomatic individuals,namely:(1)microemboli detection on transcranial Doppler,(2)plaque echolucency on Duplex ultrasound,(3)progression in the severity of ACS,(4)silent embolic infarcts on brain CT/MRI,(5)reduced cerebrovascular reserve,(6)increased size of juxtaluminal hypoechoic area,(7)identification of intraplaque haemorrhage using MRI and(8)carotid ulceration.The evidence suggests that approximately 10%-15%of patents with asymptomatic stenosis might benefit from intervention;this will become more clear after publication of ongoing studies comparing stenting or endarterectomy with best medical therapy.In the meantime,no patient should be offered intervention unless there is evidence of high risk of ipsilateral stroke,from modalities such as those discussed here.Kosmas I Paraskevas Frank J Veith J David Spence 2018Stroke & Vascular Neurology2018,3,2:10

9Biodegradable magnesium barrier membrane used for guided bone regeneration in dental surgery显示文摘Barrier membranes are commonly used as part of the dental surgical technique guided bone regeneration(GBR)and are often made of resorbable collagen or non-resorbable materials such as PTFE.While collagen membranes do not provide sufficient mechanical protection of the covered bone defect,titanium reinforced membranes and non-resorbable membranes need to be removed in a second surgery.Thus,biodegradable GBR membranes made of pure magnesium might be an alternative.In this study a biodegradable pure magnesium(99.95%)membrane has been proven to have all of the necessary requirements for an optimal regenerative outcome from both a mechanical and biological perspective.After implantation,the magnesium membrane separates the regenerating bone from the overlying,faster proliferating soft tissue.During the initial healing period,the membrane maintained a barrier function and space provision,whilst retaining the positioning of the bone graft material within the defect space.As the magnesium metal corroded,it formed a salty corrosion layer and local gas cavities,both of which extended the functional lifespan of the membrane barrier capabilities.During the resorption of the magnesium metal and magnesium salts,it was observed that the membrane became surrounded and then replaced by new bone.After the membrane had completely resorbed,only healthy tissue remained.The in vivo performance study demonstrated that the magnesium membrane has a comparable healing response and tissue regeneration to that of a resorbable collagen membrane.Overall,the magnesium membrane demonstrated all of the ideal qualities for a barrier membrane used in GBR treatment.Patrick Rider Zeljka Peric Kacarevic Akiva Elad Drazen Tadic Daniel Rothamel Gerrit Sauer Fabien Bornert Peter Windisch David Botond Hangyasi Balint Molnar Emely Bortel Bernhard Hesse Frank Witte 2022Bioactive Materials2022,7,8:7

12Under the surface:Pressure-induced planetary-scale waves,volcanic lightning,and gaseous clouds caused by the submarine eruption of Hunga Tonga-Hunga Ha'apai volcano显示文摘We present a narrative of the eruptive events culminating in the cataclysmic January 15, 2022 eruption of Hunga Tonga-Hunga Ha’apai Volcano by synthesizing diverse preliminary seismic, volcanological, sound wave, and lightning data available within the first few weeks after the eruption occurred. The first hour of eruptive activity produced fast-propagating tsunami waves, long-period seismic waves, loud audible sound waves, infrasonic waves, exceptionally intense volcanic lightning and an unsteady volcanic plume that transiently reached-at 58km-the Earth’s mesosphere. Energetic seismic signals were recorded worldwide and the globally stacked seismogram showed episodic seismic events within the most intense periods of phreatoplinian activity, and they correlated well with the infrasound pressure waveform recorded in Fiji. Gravity wave signals were strong enough to be observed over the entire planet in just the first few hours, with some circling the Earth multiple times subsequently. These large-amplitude, long-wavelength atmospheric disturbances come from the Earth’s atmosphere being forced by the magmatic mixture of tephra, melt and gasses emitted by the unsteady but quasicontinuous eruption from 0402±1–1800 UTC on January 15, 2022. Atmospheric forcing lasted much longer than rupturing from large earthquakes recorded on modern instruments, producing a type of shock wave that originated from the interaction between compressed air and ambient(wavy) sea surface. This scenario differs from conventional ideas of earthquake slip, landslides, or caldera collapse-generated tsunami waves because of the enormous(~1000x) volumetric change due to the supercritical nature of volatiles associated with the hot,volatile-rich phreatoplinian plume. The time series of plume altitude can be translated to volumetric discharge and mass flow rate. For an eruption duration of ~12 h, the eruptive volume and mass are estimated at 1.9 km^(3) and~2 900 Tg, respectively, corresponding to a VEI of 5–6 for this event. The high frequency and intensity of lightning was enhanced by the production of fine ash due to magma-seawater interaction with concomitant high charge per unit mass and the high pre-eruptive concentration of dissolved volatiles. Analysis of lightning flash frequencies provides a rapid metric for plume activity and eruption magnitude. Many aspects of this eruption await further investigation by multidisciplinary teams.
