2Epistaxis in end stage liver disease masquerading as severe upper gastrointestinal hemorrhage显示文摘AIM:To describe the prevalence,diagnosis,treatment,and outcomes of end stage liver disease(ESLD) patients with severe epistaxis thought to be severe upper gastrointestinal hemorrhage(UGIH).METHODS:This observational single center study included all consecutive patients with ESLD and epistaxis identified from consecutive subjects hospitalized with suspected UGIH and prospectively enrolled in our databases of severe UGIH between 1998 and 2011.RESULTS:A total of 1249 patients were registered for severe UGIH in the data basis,461(36.9%) were cirrhotics. Epistaxis rather than UGIH was the bleeding source in 20 patients. All patients had severe coagulopathy. Epistaxis was initially controlled in all cases. Fifteen(75%) subjects required posterior nasal packing and 2(10%) embolization in addition to correction of coagulopathy. Five(25%) patients died in the hospital,12(60%) received orthotopic liver transplantation(OLT),and 3(15%) were discharged without OLT. 3Robotic surgery of the pancreas显示文摘Pancreatic surgery is one of the most challenging and complex fields in general surgery.While minimally invasive surgery has become the standard of care for many intra-abdominal pathologies the overwhelming majority of pancreatic surgery is performed in an open fashion.This is attributed to the retroperitoneal location of the pancreas,its intimate relationship to major vasculature and the complexity of reconstruction in the case of pancreatoduodenectomy.Herein,we describe the application of robotic technology to minimally invasive pancreatic surgery.The unique capabilities of the robotic platform have made the minimally invasive approach feasible and safe with equivalent if not better outcomes(e.g.,decreased length of stay,less surgical site infections)to conventional open surgery.However,it is unclear whether the robotic approach is truly superior to traditional laparoscopy;this is a key point given the substantial costs associated with procuring and maintaining robotic capabilities.Daniel Joyce Gareth Morris-Stiff Gavin A Falk Kevin El-Hayek Sricharan Chalikonda R Matthew Walsh 2014World Journal of Gastroenterology2014,20,40:2

7Lymphoepithelial cysts of the pancreas:a management dilemma显示文摘Pancreatic lymphoepithelial cysts(LECs) are rare,benign lesions that are typically unexpected post-operative pathological findings. We aimed to review clinical, radiological and pathological features of LECs that may allow their preoperative diagnosis. Histopathology databases of two large pancreatic units were searched to identify LECs and notes reviewed to determine patient demographic details, mode of presentation, investigations, treatment and outcome. Five male and one female patients were identified. Their median age was 60 years. Lesions were identified on computed tomography performed for abdominal pain in two patients, and were incidentally observed in four patients. Five LECs were located in the tail and one in the body of the pancreas, with a median cyst size of 5 cm. Obtaining cyst fluid was difficult and a largely acellular aspirate was yielded. The pre-operative diagnosis was mucinous cystic neoplasm in all patients. This series of patients were treated distal pancreatectomy and splenectomy. A retrospective review of radiological examinations suggested that LECs have a relatively low signal on T2 imaging and a high signal intensity on T1 weighted images. LECs appear more common in elderly males, and are typically incidental, large, unilocular cysts. 11Appraisal of CO2 storage potential in compressional hydrocarbon-bearing basins: Global assessment and case study in the Sichuan Basin(China)显示文摘Carbon capture and storage(CCS)has been proposed as a potential technology to mitigate climate change.However,there is currently a huge gap between the current global deployment of this technology and that which will be ultimately required.Whilst CO2 can be captured at any geographic location,storage of CO2 will be constrained by the geological storage potential in the area the CO2 is captured.The geological storage potential can be evaluated at a very high level according to the tectonic setting of the target area.To date,CCS deployment has been restricted to more favourable tectonic settings,such as extensional passive margin and post-rift basins and compressional foreland basins.However,to reach the adequate level of deployment,the potential for CCS of regions in different tectonic settings needs to be explored and assessed worldwide.Surprisingly,the potential of compressional basins for carbon storage has not been universally evaluated according to the global and regional carbon emission distribution.Here,we present an integrated source-to-sink analysis tool that combines comprehensive,open-access information on basin distribution,hydrocarbon resources and CO2 emissions based on geographical information systems(GIS).Compressional settings host some of the most significant hydrocarbon-bearing basins and 36% of inland CO2 emissions but,to date,large-scale CCS facilities in compressional basins are concentrated in North America and the Middle East only.Our source-to-sink tool allows identifying five high-priority regions for prospective CCS development in compressional basins:North America,north-western South America,south-eastern Europe,the western Middle East and western China.We present a study of the characteristics of these areas in terms of CO2 emissions and CO2 storage potential.Additionally,we conduct a detailed case-study analysis of the Sichuan Basin(China),one of the compressional basins with the greatest CO2 storage potential.Our results indicate that compressional basins will have to play a critical role in the future of CCS if this technology is to be implemented worldwide.Xiaolong Sun Juan Alcalde Enrique Gomez-Rivas Lucía Struth Gareth Johnson Anna Travé 2020Geoscience Frontiers2020,11,6:1
