1Should antibiotics be administered before arthroscopic knee surgery? A systematic review of the literature显示文摘AIM To explore the current evidence surrounding the administration of prophylactic antibiotics for arthroscopic knee surgery. METHODS Databases were searched from inception through May of 2018 for studies examining prophylactic antibiotic use and efficacy in knee arthroscopy. Studies with patient data were further assessed for types of arthroscopic procedures performed,number of patients in the study,use of antibiotics,and outcomes with the intention of performing a pooled analysis. Data pertaining to 'deep tissue infection' or 'septic arthritis' were included in our analysis. Reported data on superficial infection were not included in our data analysis. For the pooled analysis,a relative risk ratio was calculated and χ~2 tests were used to assess for statistical significance between rates of infection amongst the various patient groups. Post hoc power analyses were performed to compute the statistical power obtained from our sample sizes. Number needed to treat analyses were performed for statistically significant differences by dividing 1 by the difference between the infection rates of the antibiotic and no antibiotic groups. An alpha value of 0.05 was used for our analysis. Study heterogeneity was assessed by Cochrane's Q test as well as calculation of the I^2 value.RESULTS A total of 49682 patients who underwent knee ar-throscopy for a diverse set of procedures across 19 studies met inclusion critera for pooled analysis. For those not undergoing graft procedures,there were 27 cases of post-operative septic arthritis in 34487 patients(0.08%) who received prophylactic antibiotics and 16 cases in 10911(0.15%) who received none [risk ratio(RR) = 0.53,95% confidence interval(CI): 0.29-0.99,P = 0.05]. A sub-group analysis in which bony procedures were excluded was performed which found no significant difference in infection rates between patients that received prophylactic antibiotics and patients that did not(P > 0.05). All anterior cruciate ligament reconstruction studies used prophylactic antibiotics,but two studies investigating the effect of soaking the graft in vancomycin in addition to standard intravenous(IV) prophylaxis were combined for analysis. There were 19 cases in 1095 patients(1.74%) who received IV antibioitics alone and no infections in 2034 patients who received IV antibiotics and had a vancomycin soaked graft(RR = 0.01,95%CI: 0.001-0.229,P < 0.01).CONCLUSION Prophylactic antibiotics are effective in preventing septic arthritis following simple knee arthroscopy. 3A Cambrian micro-lobopodian and the evolution of arthropod locomotion and reproduction显示文摘The evolutionary success of arthropods, the most abundant and diverse animal group, is mainly based on their segmented body and jointed appendages, features that had evolved most likely already before the Cambrian. The first arthropod-like animals, the lobopodians from the Early Cambrian, were unsclerotized and worm-like, and they had unjointed tubular legs. Here we describe the first three-dimensionally preserved Cambrian lobopodian. The material presented of Orstenotubulus evamuellerae gen. et sp. nov. is the smallest and youngest of a lobopodian known. O. evamuellerae shows strikingly detailed similarities to Recent tardigrades and/or onychophorans in its cellular-structured cuticle and the telescopic spines. It also shows similarities to other, longer known lobopodians, but which are ten times as large as the new form. These similarities include the finely annulated body and legs, which is characteristic also for Recent onychophorans, and paired humps continuing into spines situated dorsally to the leg insertions, a feature lacking in the extant forms. The morphology of O. evamuellerae not only elucidates our knowledge about lobopodians, but also aids in a clearer picture of the early evolution of arthropods. An example is the single ventral gonopore between a limb pair of O. evamuellerae, which indicates that a single gonopore, as developed in onychophorans, tardigrades, pentastomids, myriapods and insects, might represent the plesiomorphic state for Arthropoda, while the paired state in chelicerates and crustaceans was convergently achieved. Concerning life habits, the lateral orientation of the limbs and their anchoring spines of the new lobopodian imply that early arthropods were crawlers rather than walkers.Andreas Maas Georg Mayer Reinhardt M. 20Dawn of complex animal food webs:A new predatory anthozoan(Cnidaria)from Cambrian显示文摘Cnidarians diverged very early in animal evolution;therefore,investigations of the morphology and trophic levels of early fossil cnidarians may provide critical insights into the evolution of metazoans and the origin of modern marine food webs.However,there has been a lack of unambiguous anthozoan cnidarians from Ediacaran assemblages,and undoubted anthozoans from the Cambrian radiation of metazoans are very rare and lacking in ecological evidence.Here,we report a new polypoid cnidarian,Nailiana elegans gen.et sp.nov.,represented by multiple solitary specimens from the early Cambrian Chengjiang biota(-520 Ma)of South China.These specimens show eight unbranched tentacles surrounding a single opening into the gastric cavity,which may have born multiple mesenteries.Thus,N.elegans displays a level of organization similar to that of extant cnidarians.Phylogenetic analyses place N.elegans in the stem lineage of Anthozoa and suggest that the ancestral anthozoan was a soft-bodied,solitary polyp showing octoradial symmetry.Moreover,one specimen of the new polyp preserves evidence of predation on an epifaunal lingulid brachiopod.This case provides the oldest direct evidence of macrophagous predation,the advent of which may have triggered the emergence of complex trophic/ecological relationships in Cambrian marine communities and spurred the explosive radiation of animal body plans.Qiang Ou Degan Shu Zhifei Zhang Jian Han Heyo Van Iten Meirong Cheng Jie Sun Xiaoyong Yao Rong Wang Georg Mayer 2022The Innovation2022,3,1:1
