2关节镜下盂唇固定加关节囊热挛缩治疗肩关节前方不稳的回顾性分析显示文摘目的对创伤性、单方向、复发性肩关节前方不稳患者实施关节镜下单纯肩关节盂唇固定术和肩关节盂唇固定加关节囊热挛缩术,探讨关节囊热挛缩术的作用。方法回顾性病例对照、序列临床研究。共有72例符合标准的病例由同一医生施行肩关节镜下前方稳定术。A组32例在1996年至1999年间完成,采用SuretacⅡ组织铆钉固定;B组40例,采用SuretacⅡ组织铆钉加射频关节囊热挛缩术,在1999年至2002年间进行。两组病例的术后康复程序一致,分别在术前与术后3、6、12、24个月进行患者决定因素评价和医生决定因素评价。结果66例获得完全随访,其中A组28例平均随访58个月,B组38例平均随访30个月。所有患者均有Bankart损伤。两组病例在患者决定因素和医生决定因素的疗效评定中结果相同。惟一不良结果是复发性不稳,A组28例中,5例再脱位,1例复发性半脱位;B组38例中,6例再脱位,2例复发性半脱位。大部分复发性不稳发生在术后6~24个月,运动创伤是复发性脱位的主要原因。“Kaplan-Meier”时间-再脱位生存分析显示:两组间的复发性再脱位发生率没有明显差别。结论用可吸收组织铆钉进行盂唇修复术治疗肩关节前方不稳时,加用关节镜下关节囊热挛缩术,既不增强术后关节囊强度,也未加重不稳结果。陈世益 George AC Murrell 2005中华骨科杂志2005,25,11:4
