1New therapeutic perspectives in irritable bowel syndrome: Targeting low-grade inflammation, immuno-neuroendocrine axis, motility, secretion and beyond显示文摘Irritable bowel syndrome(IBS)is a chronic,recurring,and remitting functional disorder of the gastrointestinal tract characterized by abdominal pain,distention,and changes in bowel habits.Although there are several drugs for IBS,effective and approved treatments for one or more of the symptoms for various IBS subtypes are needed.Improved understanding of pathophysiological mechanisms such as the role of impaired bile acid metabolism,neurohormonal regulation,immune dysfunction,the epithelial barrier and the secretory properties of the gut has led to advancements in the treatment of IBS.With regards to therapies for restoring intestinal permeability,multiple studies with prebiotics and probiotics are ongoing,even if to date their efficacy has been limited.In parallel,much progress has been made in targeting low-grade inflammation,especially through the introduction of drugs such as mesalazine and rifaximin,even if a better knowledge of the mechanisms underlying the low-grade inflammation in IBS may allow the design of clinical trials that test the efficacy and safety of such drugs.This literature review aims to summarize the findings related to new and investigational therapeutic agents for IBS,most recently developed in preclinical as well as Phase 1 and Phase 2clinical studies.Emanuele Sinagra Gaetano Cristian Morreale Ghazaleh Mohammadian Giorgio Fusco Valentina Guarnotta Giovanni Tomasello Francesco Cappello Francesca Rossi Georgios Amvrosiadis Dario Raimondo 2017World Journal of Gastroenterology2017,23,36:14

3Efficacy and safety of Amla(Phyllanthus emblica L.) in non-erosive reflux disease:a double-blind,randomized,placebo-controlled clinical trial显示文摘Background: Gastroesophageal reflux disease(GERD) is one of the most common gastrointestinal complaints. GERD, caused by the reflux of stomach contents into the esophagus, leads to troublesome symptoms such as heartburn and regurgitation. It is classified into two types: erosive esophagitis,characterized by visible esophageal mucosa erosion in endoscopy, and non-erosive reflux disease(NERD). GERD is a chronic and recurrent disease that impairs the quality of life and imposes socioeconomic and therapeutic burdens to both patients and society.Objective: Due to the failure of the conventional treatments for GERD and to the traditional use of Amla(Phyllanthus emblica L.), in addition to beneficial effects shown in recent studies, we evaluated the safety and efficacy of Amla tablet for improvement of symptoms of patients with NERD.Design, setting, participants and interventions: We designed a double-arm, randomized, double-blind,placebo-controlled clinical trial. Sixty-eight patients who had classic symptoms of GERD(heartburn,regurgitation and epigastralgia) for at least three months before the start of the trial were randomized in two parallel groups. Patients in the Amla group received two 500 mg Amla tablets twice a day, after meals,for 4 weeks. In the control group, patients received placebo tablets similar to the Amla prescription.Main outcome measures: The patients were visited at baseline, and at the end of the 2 nd and 4 th weeks of intervention; their symptoms were measured on a frequency and severity scale for the symptoms of NERD,according to the quality of life in reflux-associated disease questionnaire.Results: Frequencies of heartburn and regurgitation in both groups of the study were significantly reduced after intervention(P < 0.001). 4无溶剂条件下可重复使用的担载的酸性离子液体催化剂高效催化酮和芳香醛选择性Cross-Aldol缩合反应(英文)显示文摘A newly prepared catalyst consisting of acidic ionic liquid 1-(4-sulfonic acid)butylpyridinium hydrogen sulfate supported on silica was used to catalyze the cross-aldol condensation of ketones with aromatic aldehydes under solvent-free conditions. The highly active and selective catalyst gave good to excellent yields of the desired cross-aldol products without the occurrence of any self-condensation reactions. Reaction times were short, the procedure and work-up were simple, and no volatile or hazardous organic solvents were necessary. 7Efficacy of a traditional Persian medicine preparation for radiation-induced xerostomia:a randomized,open-label,active-controlled trial显示文摘BACKGROUND: Xerostomia is one of the most common side effects of radiation therapy among patients with head and neck cancers(HNC). However, conventional medicine lacks an effective treatment for radiation-induced xerostomia.OBJECTIVE: Synthesizing the traditional use of Alcea digitata and Malva sylvestris with their known beneficial effects from recent studies, we evaluated the efficacy of the herbs in the quality of life(QOL) of HNC patients with radiation-induced xerostomia.DESIGN, SETTING, PARTICIPANTS AND INTERVENTIONS: This study is a randomized, double-arm, open-label active-controlled clinical trial. We evaluated the effect of A. digitata and M. sylvestris on QOL of HNC patients with radiation-induced xerostomia compared with Hypozalix(artificial saliva). Patients were enrolled from the Imam Hossein Hospital's oncology clinic in Shahid Beheshti University of Medical Sciences, Tehran, Iran.MAIN OUTCOME MEASURES: Primary outcome measures in this trial were changes in patients' QOL assessed by the European Organization for Research and Treatment of Cancer Quality of Life Questionnaire, Head and Neck Module(EORTC QLQ-H&N 35).RESULTS: Between-group analysis showed that the intervention group patients obtained significantly lower(better) total EORTC QLQ-H&N 35 scores as compared to the control group at the end of the intervention period(P = 0.007). 8Family history and disease outcomes in patients with Crohn's disease:A comparison between China and the United States显示文摘AIM To investigate the differences in family history of inflammatory bowel disease(IBD) and clinical outcomes among individuals with Crohn's disease(CD) residing in China and the United States.METHODS We performed a survey-based cross-sectional study of participants with CD recruited from China and the United States.We compared the prevalence of IBD family history and history of ileal involvement,CD-related surgeries and IBD medications in China and the United States,adjusting for potential confounders.RESULTS We recruited 49 participants from China and 145 from the United States.The prevalence of family history of IBD was significantly lower in China compared with the United States(China:4.1%,United States:39.3%).The three most commonly affected types of relatives were cousin,sibling,and parent in the United States compared with child and sibling in China.Ileal involvement(China:63.3%,United States:63.5%) and surgery for CD(China:51.0%,United States:49.7%) were nearly equivalent in the two countries.CONCLUSION The lower prevalence of familial clustering of IBD in China may suggest that the etiology of CD is less attributed to genetic background or a family-shared environment compared with the United States.Despite the potential difference in etiology,surgery and ileal involvement were similar in the two countries.Examining the changes in family history during the continuing rise in IBD may provide further insight into the etiology of CD.Pei-Qi Wang Jun Hu Elie S Al Kazzi Eboselume Akhuemonkhan Min Zhi Xiang Gao Raquel Holand de Paula Pessoa Sami Ghazaleh Tuhina Cornelius Suhel Abbas Sabunwala Shadi Ghadermarzi Kartikeya Tripathi Mark Lazarev Pin-Jin Hu Susan Hutfless 2016World Journal of Gastrointestinal Pharmacology and Therapeutics2016,7,4:2
