| 1 | Farnesoid X receptor,the bile acid sensing nuclear receptor,in liver regeneration显示文摘The liver is unique in regenerative potential, which could recover the lost mass and function after injury from ischemia and resection. The underlying molecular mechanisms of liver regeneration have been extensively studied in the past using the partial hepatectomy(PH) model in rodents, where 2/3 PH is carried out by removing two lobes. The whole process of liver regeneration is complicated, orchestrated event involving a network of connected interactions, which still remain fully elusive. Bile acids(BAs) are ligands of farnesoid X receptor(FXR), a nuclear receptor of ligand-activated transcription factor. FXR has been shown to be highly involved in liver regeneration. BAs and FXR not only interact with each other but also regulate various downstream targets independently during liver regeneration. Moreover, recent findings suggest that tissue-specific FXR also contributes to liver regeneration significantly. These novel findings suggest that FXR has much broader role than regulating BA, cholesterol, lipid and glucose metabolism. Therefore, these researches highlight FXR as an important pharmaceutical target for potentialuse of FXR ligands to regulate liver regeneration in clinic. | 4 | Clinical features and treatment outcomes of endogenous Klebsiella endophthalmitis:a 12-year review显示文摘AIM:To identify the clinical features and treatment outcomes of endogenous Klebsiella pneumoniae endophthalmitis and investigate prognostic factors of poor visual outcome.METHODS:The clinical records of all patients diagnosed with endogenous Klebsiella endophthalmitis between January 2007 to December 2018 in Prince of Wales Hospital,Hong Kong,China were retrospectively reviewed.Thorough ophthalmological examination findings were recorded in the case note,including visual acuity testing,slit-lamp examination,indirect ophthalmoscopy and B-scan ultrasonography if media opacity precluded fundus viewing.RESULTS:A total of 18 eyes in 14 patients were identified.Bilateral involvement was noted in 4 patients(28.6%).Hepatobiliary sepsis was the source in 9 patients(64.3%).Culture of intraocular fluid was positive in 5 out of 18 eyes(27.8%).Mortality was noted in 2 patients(14.3%).Mean final visual acuity was 20/1500.Six out of 16 eyes had total loss of sight(37.5%)and 3 eyes required evisceration(18.8%).Multivariate linear regression revealed poor presenting visual acuity(P=0.031)and lack of fundus view due to vitritis(P=0.02)as prognostic factors of poor visual outcome.CONCLUSION:Visual outcome of endogenous Klebsiella endophthalmitis is poor.Poor presenting visual acuity and lack of fundus view predict poor visual outcome.High index of suspicion for endophthalmitis is important in Klebsiella sepsis patients with complaints of ocular symptoms.Ophthalmological screening is recommended in noncommunicable patients with Klebsiella sepsis. | Chun Yue Mak Mary Ho Lawrence Pui-leung Iu Helena Pui-yee Sin Li Jia Chen Grace Lui Marten Erik Brelen Alvin Lerrmann Young | 2020 | International Journal of Ophthalmology(English edition)2020,13,12: | 5 |
| 5 | Impaired granulocyte-macrophage colony-stimulating factor bioactivity accelerates surgical recurrence in ileal crohn's disease显示文摘AIM To examine the relationship between elevated granulocyte-macrophage colony-stimulating factor(GMCSF) auto-antibodies(Ab) level and time to surgical recurrence after initial surgery for Crohn's disease(CD). METHODS We reviewed 412 charts from a clinical database at tertiary academic hospital. Patients included in the study had ileal or ileocolonic CD and surgical resection of small bowel or ileocecal region for management of disease. Serum samples were analyzed for serological assays including GM-CSF cytokine, GM-CSF Ab, ASCA Ig G and Ig A, and genetic markers including SNPs rs2066843, rs2066844, rs2066845, rs2076756 and rs2066847 in NOD2, rs2241880 in ATG16 L1, and rs13361189 in IRGM. Cox proportional-hazards models were used to assess the predictors of surgical recurrence.RESULTS Ninety six percent of patients underwent initial ileocecal resection(ICR) or ileal resection(IR) and subsequently 40% of patients required a second ICR/IR for CD. GMCSF Ab level was elevated at a median of 3.81 mcg/mL. Factors predicting faster time to a second surgery included elevated GM-CSF Ab [hazard ratio(HR) 3.52, 95%CI: 1.45-8.53, P = 0.005] and elevated GM-CSF cytokine(HR = 2.48, 95%CI: 1.31-4.70, P = 0.005). Factors predicting longer duration between first and second surgery included use of Immunomodulators(HR = 0.49, 95%CI: 0.31-0.77, P = 0.002), the interaction effect of low GM-CSF Ab levels and smoking(HR = 0.60, 95%CI: 0.45-0.81, P = 0.001) and the interaction effect of low GM-CSF cytokine levels and ATG16 L1(HR = 0.65, 95%CI: 0.49-0.88, P = 0.006).CONCLUSION GM-CSF bioavailability plays a critical role in maintaining intestinal homeostasis. | 6 | Palliative surgery for Krukenberg tumors—12-year experience and review of the literature显示文摘AIM To determine the clinical characteristics of patients undergoing palliative surgery for Krukenberg tumors,including disease presentation,outcomes,and prognostic factors.METHODS This was a retrospective clinical study of all patients who underwent palliative surgery for Krukenberg tumors between January 2004 and December 2015.Patient information was obtained from inpatient and outpatient case notes as well as the hospital electronic records.Patients who underwent potential y curative resection,and patients with Krukenberg tumors who did not undergo surgery were also excluded from the study.Palliative surgery was defined as those performed for either alleviation of symptoms or for asymptomatic patients for whom surgical removal of the tumors were deemed necessary following a multidisciplinary consensus.Tumors were diagnosed pre-operatively by computed tomography scans and all had histologic confirmation of the surgical specimens.RESULTS Over the study duration,38 female patients underwent palliative surgery for Krukenberg tumors at our institution.Mean age was 54.2±11.7 years.The colon was the most frequent primary source of metastases(n=21)followed by the stomach(n=4).Prophylactic palliative surgery was performed for eight(21.1%)asymptomatic patients.Median post-operative length of stay was 8 d(IQR 6-12 d).Five patients(13.2%)experienced postoperative complications,although high grade morbidity was only seen in one patient(2.6%).Median overall survival from surgery was 17 mo(95%CI:12.1-21.9)at a median follow-up duration of 12 mo(IQR 8-17mo).The median survival was shorter for patients who underwent emergency surgery,younger patients,those with a colorectal primary,larger tumors,or synchronous peritoneal or hepatic metastases.CONCLUSION Pal iative surgery for Krukenberg tumors can be performed safely with acceptable complication rates.Bilateral oophorectomy should be performed to prevent the risk of symptomatic contralateral tumors. | Isaac Seow-En Gwen Hwarng Grace Hwei Ching Tan Leonard Ming Li Ho Melissa Ching Ching Teo | 2018 | World Journal of Clinical Oncology2018,9,1: | 5 |
| 7 | Imaging findings of inflammatory pseudotumor-like follicular dendritic cell tumors of the liver:Two case reports and literature review显示文摘BACKGROUND Inflammatory pseudotumor-like follicular dendritic cell(IPT-like FDC)tumors of the liver is an uncommon tumor with extremely low incidence.To date,the radiologic findings of this tumor in multiphase computed tomography(CT)and magnetic resonance imaging(MRI)imaging have not been described.CASE SUMMARY Patient 1 is a 31-year-old Chinese female,whose complaining incidentally coincided with the finding of multiple liver masses.In the local hospital,an abdominal enhanced CT found two hypo-dense solid lesions,with heterogeneous sustained hypoenhancement,in the upper segment of the liver’s right posterior lobe.In our hospital,enhanced magnetic resonance imaging(MRI)with hepatocyte-specific contrast agents showed a similar enhanced pattern of lesions with patchy hyperintensity in the hepatobiliary phase(HBP).The patient underwent surgery and recovered well.The final pathology confirmed an IPTlike FDC tumor.No recurrence was found on the regular re-examination.Patient 2 is a 48-year-old Chinese male admitted to our hospital for a huge unexpected hepatic lesion.A dynamic enhanced abdominal CT revealed a huge heterogeneous enhanced solid tumor in the right lobe of the liver with a size of 100 mm×80 mm,which showed a heterogeneous sustained hypoenhancement.In addition,enlarged lymph nodes were found in the hilum of the liver.This patient underwent a hepatic lobectomy and lymph node dissection.The final pathology confirmed an IPT-like FDC tumor.No recurrence was found upon regular re-examination.CONCLUSION When a hepatic tumor shows heterogeneous sustained hypoenhancement with a patchy enhancement during HBP,an IPT-like FDC tumor should be considered in the differential diagnosis. | Hai-Lan Li Hua-Ping Liu Grace Wen-Jun Guo Zhi-Hong Chen Fu-Qing Zhou Peng Liu Jian-Bin Liu Ren Wan Zhi-Qun Mao | 2019 | World Journal of Gastroenterology2019,25,45: | 4 |

| 9 | Ca^(2+)-supplying black phosphorus-based scaffolds fabricated with microfluidic technology for osteogenesis显示文摘Effective osteogenesis remains a challenge in the treatment of bone defects.The emergence of artificial bone scaffolds provides an attractive solution.In this work,a new biomineralization strategy is proposed to facilitate osteogenesis through sustaining supply of nutrients including phosphorus(P),calcium(Ca),and silicon(Si).We developed black phosphorus(BP)-based,three-dimensional nanocomposite fibrous scaffolds via microfluidic technology to provide a wealth of essential ions for bone defect treatment.The fibrous scaffolds were fabricated from 3D poly(L-lactic acid)(PLLA)nanofibers(3D NFs),BP nanosheets,and hydroxyapatite(HA)-porous SiO2 nanoparticles.The 3D BP@HA NFs possess three advantages:i)stably connected pores allow the easy entrance of bone marrow-derived mesenchymal stem cells(BMSCs)into the interior of the 3D fibrous scaffolds for bone repair and osteogenesis;ii)plentiful nutrients in the NFs strongly improve osteogenic differentiation in the bone repair area;iii)the photothermal effect of fibrous scaffolds promotes the release of elements necessary for bone formation,thus achieving accelerated osteogenesis.Both in vitro and in vivo results demonstrated that the 3D BP@HA NFs,with the assistance of NIR laser,exhibited good performance in promoting bone regeneration.Furthermore,microfluidic technology makes it possible to obtain high-quality 3D BP@HA NFs with low costs,rapid processing,high throughput and mass production,greatly improving the prospects for clinical application.This is also the first BP-based bone scaffold platform that can self-supply Ca^(2+),which may be the blessedness for older patients with bone defects or patients with damaged bones as a result of calcium loss. | Zhanrong Li Xingcai Zhang Jiang Ouyang Dandan Chu Fengqi Han Liuqi Shi Ruixing Liu Zhihua Guo Grace X.Gu Wei Tao Lin Jin Jingguo Li | 2021 | Bioactive Materials2021,6,11: | 4 |

| 12 | Phantom motion intent decoding for transhumeral prosthesis control with fused neuromuscular and brain wave signals显示文摘In recent years,the electroencephalography(EEG)brain-computer interface(BCI)has been researched in the area of upper-limb prosthesis control due to the promise of being able to record neurological signals which follow activation patterns in the cortex directly from the brain with non-invasive electrodes.This is seen as a way of bypassing the limitation posed by acquiring neuromuscular signals predominantly with electromyography(EMG)directly from the stump,which possesses residual limb anatomy post-amputation.In this study,the sequential forward selection algorithm to form a 10-optimal-channel representation,alongside an extended signal feature vector was applied,to investigate the motion intent decoding performance of EMG-only,EEG-only,and a fused EMG-EEG sensing configuration for four transhumeral amputees with varying stump lengths.The results showed a considerable improvement for the EMG-only configuration with the advanced feature vector,but only a small increase for the EEG-only,and thus a marginal improvement when information from both signals was fused together.This is likely due to the EEG requiring a greater number of channels spread across the skull to provide a reliable intent decoding.Further work will now involve optimisation studies to find a greater representation of electrode representation and parsimony,to minimise the number of channels while boosting motion intent decoding accuracy. | Ejay Nsugbe Oluwarotimi Williams Samuel Mojisola Grace Asogbon Guanglin Li | 2021 | IET Cyber-Systems and Robotics2021,3,1: | 2 |
