2Impaired granulocyte-macrophage colony-stimulating factor bioactivity accelerates surgical recurrence in ileal crohn's disease显示文摘AIM To examine the relationship between elevated granulocyte-macrophage colony-stimulating factor(GMCSF) auto-antibodies(Ab) level and time to surgical recurrence after initial surgery for Crohn's disease(CD). METHODS We reviewed 412 charts from a clinical database at tertiary academic hospital. Patients included in the study had ileal or ileocolonic CD and surgical resection of small bowel or ileocecal region for management of disease. Serum samples were analyzed for serological assays including GM-CSF cytokine, GM-CSF Ab, ASCA Ig G and Ig A, and genetic markers including SNPs rs2066843, rs2066844, rs2066845, rs2076756 and rs2066847 in NOD2, rs2241880 in ATG16 L1, and rs13361189 in IRGM. Cox proportional-hazards models were used to assess the predictors of surgical recurrence.RESULTS Ninety six percent of patients underwent initial ileocecal resection(ICR) or ileal resection(IR) and subsequently 40% of patients required a second ICR/IR for CD. GMCSF Ab level was elevated at a median of 3.81 mcg/mL. Factors predicting faster time to a second surgery included elevated GM-CSF Ab [hazard ratio(HR) 3.52, 95%CI: 1.45-8.53, P = 0.005] and elevated GM-CSF cytokine(HR = 2.48, 95%CI: 1.31-4.70, P = 0.005). Factors predicting longer duration between first and second surgery included use of Immunomodulators(HR = 0.49, 95%CI: 0.31-0.77, P = 0.002), the interaction effect of low GM-CSF Ab levels and smoking(HR = 0.60, 95%CI: 0.45-0.81, P = 0.001) and the interaction effect of low GM-CSF cytokine levels and ATG16 L1(HR = 0.65, 95%CI: 0.49-0.88, P = 0.006).CONCLUSION GM-CSF bioavailability plays a critical role in maintaining intestinal homeostasis. 3High-performance all-polymer solar cells enabled by a novel low bandgap non-fully conjugated polymer acceptor显示文摘The non-fully conjugated polymer as a new class of acceptor materials has shown some advantages over its small molecular counterpart when used in photoactive layers for all-polymer solar cells(all-PSCs),despite a low power conversion efficiency(PCE)caused by its narrow absorption spectra.Herein,a novel non-fully conjugated polymer acceptor PFY-2TS with a low bandgap of~1.40 eV was developed,via polymerizing a largeπ-fused small molecule acceptor(SMA)building block(namely YBO)with a non-conjugated thioalkyl linkage.Compared with its precursor YBO,PFY-2TS retains a similar low bandgap but a higher LUMO level.Moreover,compared with the structural analog of YBO-based fully conjugated polymer acceptor PFY-DTC,PFY-2TS shows a similar absorption spectrum and electron mobility,but significantly different molecular crystallinity and aggregation properties,which results in optimal blend morphology with a polymer donor PBDB-T and physical processes of the device in all-PSCs.As a result,PFY-2TS-based all-PSCs achieved a PCE of 12.31%with a small energy loss of 0.56 eV enabled by the reduced non-radiative energy loss(0.24 eV),which is better than that of 11.08%for the PFY-DTC-based ones.Our work clearly demonstrated that non-fully conjugated polymers as a new class of acceptor materials are very promising for the development of high-performance all-PSCs.Qunping Fan Ruijie Ma Tao Liu Jianwei Yu Yiqun Xiao Wenyan Su Guilong Cai Yuxiang Li Wenhong Peng Tao Guo Zhenghui Luo Huiliang Sun Lintao Hou Weiguo Zhu Xinhui Lu Feng Gao Ellen Moons Donghong Yu He Yan Ergang Wang 2021Science China Chemistry2021,64,8:3
4Feeding Biochar to Cows:An Innovative Solution for Improving Soil Fertility and Farm Productivity显示文摘Addition of biochar produced through thermal decomposition of biomass has been seen as a strategy to improve soils and to sequester carbon(C), but wide scale implementation of the technology requires to devise innovative profitable solutions. To develop biochar utilisation with an integrated system approach, an innovative program was implemented in 2012 on a 53-ha farm in Western Australia to determine the costs and benefits of integrating biochar with animal husbandry and improvement of pastures. Biochar was mixed with molasses and fed directly to cows. The dung-biochar mixture was incorporated into the soil profile by dung beetles.We studied the changes in soil properties over 3 years. Biochar extracted from fresh dung and from the soil to a depth of 40 cm was characterised. A preliminary financial analysis of the costs and benefits of this integrated approach was also undertaken. The preliminary investigation results suggested that this strategy was effective in improving soil properties and increasing returns to the farmer. It was also concluded that the biochar adsorbed nutrients from the cow's gut and from the dung. Dung beetles could transport this nutrient-rich biochar into the soil profile. There was little evidence that the recalcitrant component of the biochar was reduced through reactions inside the gut or on/in the soil. 6Medical training fails to prepare providers to care for patients with chronic hepatitis B infection显示文摘AIM: To investigate physicians' knowledge including chronic hepatitis B(CHB) diagnosis, screening, and management in various stages of their training.METHODS: A voluntary 20-question survey was administered in Santa Clara County, CA where Asian and Pacific Islanders(API) account for a third of the population. Among the 219 physician participants,there were 63 interns, 60 second-year residents, 26 chief residents and 70 attending physicians. The survey asked questions regarding respondents' demographics,general hepatitis B virus knowledge questions(i.e.,transmission, prevalence, diagnostic testing, prevention,and treatment options), as well as, self-reported practice behavior and confidence in knowledge.RESULTS: Knowledge about screening and managing patients with CHB was poor: only 24% identified the correct tests to screen for CHB, 13% knew the next steps for patients testing positive for CHB, 18% knew the high prevalence rate among API, and 31% knew how to screen for liver cancer. Wald chi-square analysis determined the effect of training level on knowledge; in all cases except for knowledge of liver cancer screening(p = 0.0032), knowledge did not significantly increase with length in residency training or completion of residency.CONCLUSION: Even in a high-risk region, both medical school and residency training have not adequatelyprepared physicians in the screening and management of CHB.Stephanie D Chao Bing-Mei Wang Ellen T Chang Li Ma Samuel K So 2015World Journal of Gastroenterology2015,21,22:2
