| 1 | 全髋关节置换非骨水泥型股骨柄假体髓腔准备的精确性研究显示文摘目的观察非骨水泥型股骨柄假体在股骨近端髓腔中与骨组织的接触情况及使用机器人辅助手术与传统手术结果的比较。方法在10具新鲜成人尸体股骨标本上,分别采用传统手术及机器人辅助手术行股骨近端髓腔准备后植入近端固定的非骨水泥型股骨柄假体(Osteolock)。经一系列处理后,用钻石刀按层厚3mm行硬组织切片,摄片,扫描输入计算机并用特殊软件(IDL)分析假体近端情况。分别测量假体与骨组织间缝隙的宽度以及假体表面缝隙的长度及其所占假体周径的百分比,假体表面与骨直接接触所占的百分比。结果机器人辅助手术组(实验组)平均假体近端表面与骨直接接触率达93.2%(87.6%~99.7%),假体表面与骨组织间存在由手术造成的缝隙率仅为2.9%(0.3%~7.8%),最大缝隙宽度小于1mm;而传统手术组(对照组)骨直接接触率为60.1%(49.2%~70.4%),由手术造成的缝隙率达32.8%(25.1%~39.9%),最大缝隙宽度达3mm。采用机器人辅助手术可使假体表面与骨直接接触率较传统手术增加约33%,手术误差造成的缝隙率仅为传统手术的9%,最大缝隙宽度为传统手术的26%。结论机器人辅助手术可使股骨近端髓腔与非骨水泥型股骨柄的直接接触率明显提高(P<0.001)。 | 吴立东 HJ Hahne J Hassenpflug | 2002 | 中华骨科杂志2002，22，5： | 15 |
| 2 | 胫骨高位截骨术治疗膝内侧室骨关节炎的长期随访显示文摘目的 观察胫骨高位截骨术治疗膝内侧室骨关节炎的远期治疗效果。方法 随访了1985~ 1996年 12年间在德国基尔大学骨科医院行胫骨高位截骨术的 194例 (2 15膝 )膝内侧室骨关节炎的患者。结果 有 16 1膝获得随访 ,平均随访时间 7 5年 (1 5~ 12年 )。手术后 2年内的优良率为 97 3% ,5年内的优良率为 93 6 % ,手术后 5年以上的优良率为 78 2 %。 12膝发生手术并发症(5 6 % ) ,主要为感染 (1例深部感染 ,5例浅表感染 ) ,骨不连或骨延迟愈合 5例 ,腓总神经损伤 1例。手术后有 19膝 (11 8% )因治疗效果的下降须再次行全膝关节置换手术。以全膝置换为失败标准的手术成功率分别为 :2年 98 7% ,5年 95 0 %及 10年为 84 1%。结论 胫骨高位截骨术是治疗膝关节内侧骨关节炎的一种有效手段 ,术中下肢力线的适当过度纠正是手术成功的关键 ,远期疗效呈下降趋势 ,有一部分患者须再次行全膝关节置换手术。 | 吴立东 HJ Hahne J Hassenpflug | 2004 | 中华外科杂志2004，42，8： | 8 |
| 3 | Pioglitazone, a specific ligand of peroxisome proliferator-activated receptor-gamma, protects pancreas against acute cerulein-induced pancreatitis显示文摘ABM: To determine the effect of pioglitazone, a specific peroxisome proliferator-activated receptor-γ(PPARγ) ligand, on the development of acute pancreatitis (AP) and on the expression of heat shock protein 70 (HSP70) in the pancreas. METHODS: AP was induced in rats by subcutaneous infusion of cerulein for 5 h. Pancreatic blood flow was measured by laser Doppler flowmetry. Plasma lipase activity, interleukin-1β (IL-1β) and IL-10 were determined, Pancreatic weight and histology were evaluated and pancreatic DNA synthesis and blood flow as well as pancreatic mRNA for IL-1β and HSP70 were assessed in rats treated with pioglitazone alone or in combination with cerulein. RESULTS: Pioglitazone administered (10-100 mg/kg i.g.) 30 min before cerulein, attenuated dose-dependently the pancreatic tissue damage in cerulein-induced pancreatitis (CIP) as demonstrated by the improvement of pancreatic histology, reduction in plasma lipase activity, plasma concentration of pro-inflammatory IL-1β and its gene expression in the pancreas and attenuation of the pancreatitis-evoked fall in pancreatic blood flow. CIP increased pancreatic HSP70 mRNA and protein expression in the pancreas and this effect was enhanced by pioglitazone treatment. CONCLUSION: Pioglitazone attenuates CIP and the beneficial effect of this pioglitazone is multifactorial probably due to its anti-inflammatory activities, to the suppression of IL-1β and to the over-expression of HSP70. | 4 | A long-term follow-up study of high tibial osteotomy for medial compartment osteoarthrosis显示文摘Objective: To observe the long-term outcome of high tibial osteotomy (HTO) in treating medial compartment osteoarthrosis of knees. Methods: A retrospective study was carried out on 194 patients (215 knees) treated with HTO for medial compartment osteoarthritis at the Orthopaedic Hospital of Kiel University between 1985 and 1996. Results: One hundred and sixty-one knees (144 patients) were followed up for (1.5)-12 years with an average of (7.5) years and their data were reviewed. The proportion of excellent outcome were (97.3)%, (93.6)% and (78.2)% two, five and over five years after HTO, respectively. The revision rate of total knee arthroplasty (TKA) was (11.8)% (19 knees retreated with TKA for HTO failure). The survivorship analysis of the 19 knees retreated with TKA showed an expected survival rate of (98.7%), (95.0%) and (84.1)% 2, 5 and 10 years after HTO, respectively. There were (5.6)% complications (12 /161), including five superficial wound infections, one deep infection, five delayed bone healing, and one peroneal nerve palsy. Fifty patients (54 knees) missed follow-up, among them 10 patients (11 knees) died.Conclusions: HTO is an effective method in treating medial compartment osteoarthritis with a varus knee. Appropriate overcorrection of femorotibial alignment is the key for the success of the operation.
