1Diagnosis of early gastric cancer using narrow band imaging and acetic acid显示文摘AIM: To determine whether the endoscopic findings of depressed-type early gastric cancers(EGCs) could precisely predict the histological type.METHODS: Ninety depressed-type EGCs in 72 patients were macroscopically and histologically identified. We evaluated the microvascular(MV) and mucosal surface(MS) patterns of depressed-type EGCs using magnifying endoscopy(ME) with narrow-band imaging(NBI)(NBI-ME) and ME enhanced by 1.5% acetic acid, respectively. First, depressed-type EGCs were classified according to MV pattern by NBI-ME. Subsequently, EGCs unclassified by MV pattern were classified according to MS pattern by enhanced ME(EME) images obtained from the same angle.RESULTS: We classified the depressed-type EGCs into the following 2 MV patterns using NBI-ME: a fine-network pattern that indicated differentiated adenocarcinoma(25/25, 100%) and a corkscrew pattern that likely indicated undifferentiated adenocarcinoma(18/23, 78.3%). However, 42 of the 90(46.7%) lesions could not be classified into MV patterns by NBI-ME. These unclassified lesions were then evaluated for MS patterns using EME, which classified 33(81.0%) lesions as MS patterns, diagnosed as differentiated adenocarcinoma. 3Clinical characteristics and management of gastric tube cancer with endoscopic submucosal dissection显示文摘AIM:To identify the characteristics of gastric tube cancer(GTC) and the complications associated with endoscopic submucosal dissection(ESD) for GTC.METHODS:Between 2007 and 2012,11 individualswith early gastric cancer in the reconstructed gastric tube after esophagectomy who underwent ESD in this hospital were studied.The characteristics of GTC were identified,and the complications of ESD for GTC were analyzed at three phases:preoperative,intraoperative,and postoperative.RESULTS:A total of 11 consecutive patients with 11 GTCs were selected for this study.All cases underwent en bloc resections by ESD.The median procedure time was 142 min.The average GTC diameter was 26.1 mm,and the average size of the resected lesions was 45.5 mm.The histopathological diagnosis in all cases was a differentiated adenocarcinoma.In the preoperative phase,anastomotic strictures(5/11,45%) and food residues(4/11,36.4%) in the gastric tube were the main complications.In the intraoperative phase,bleeding was observed in 5 cases(45%).The postoperative complications observed were delayed bleeding in 2 cases(18.2%) and stenosis in one case(9.1%).The case with stenosis was successfully treated using endoscopic balloon dilatation.CONCLUSION:Minor complications were frequently observed.However,all GTCs underwent en bloc resection with ESD without any serious complications.ESD is considered a useful treatment for GTC.Michita Mukasa Hidetoshi Takedatsu Ken Matsuo Hiroaki Sumie Hikaru Yoshida Atsushi Hinosaka Yasutomo Watanabe Osamu Tsuruta Takuji Torimura 2015World Journal of Gastroenterology2015,21,3:8

5Reimagining Modern Education: Contributions from Modern Japanese Philosophy and Practice?显示文摘Purpose:Amidst ongoing attempts to think beyondWestern frameworks for education,there is a tendency to overlook Japan,perhaps because it appears highly modern.This is striking given that some prominent strands of Japanese philosophy have formulated an explicit and exacting challenge to the core onto-epistemic premises of modern Western thought.It is also surprising because Japanese educational practices have resulted in some of the highest achievement outcomes—both cognitive and noncognitive—found anywhere in the world and inculcate a worldview that is distinct.Design/Approach/Methods:Herein,we thus attempt to make visible the potential contribution of modern Japanese philosophy by outlining some of the core ideas,then turn to sketch resonances with and responses to other projects outlined in this Special Issue.Our approach is elucidation through relational comparison.Findings:Through this process,we suggest that the notion of self-negation as a mode of learning may be helpful in explaining why—at the empirical level—the outlook of Japanese students,and perhaps other East Asian students,diverge markedly from their Western peers.Yet we also find that an attempt,such as ours,to link divergent onto-epistemic thought to alternative empirical hypotheses quickly gives rise to various doubts and discomforts,even among otherwise sympathetic scholars.Originality/Value:In directly responding to these doubts,one original contribution of our piece is to show just how difficult it may ultimately be to divest from the symbolic foundations already laid by Western liberalism:Even if divergent thought can be imagined and different cultural narratives explored,dominant readings of empirical“realities”continue to be entrapped in the logic laid by Western liberalism.Hikaru Komatsu Jeremy Rappleye 2020ECNU Review of Education2020,3,1:6
6A novel animal model for in vivo study of liver cancer metastasis显示文摘AIM:To establish an animal model with human hepatocyte-repopulated liver for the study of liver cancer metastasis.METHODS:Cell transplantation into mouse livers was conducted using alpha-fetoprotein(AFP)-producing hu-man gastric cancer cells(h-GCCs) and h-hepatocytes as donor cells in a transgenic mouse line expressing urokinase-type plasminogen activator(uPA) driven by the albumin enhancer/promoter crossed with a severe combined immunodeficient(SCID) mouse line(uPA/SCID mice).Host mice were divided into two groups(A and B).Group A mice were transplanted with h-GCCs alone,and group B mice were transplanted with h-GCCs and h-hepatocytes together.The replacement index(RI),which is the ratio of transplanted h-GCCs and h-hepatocytes that occupy the examined area of a histological section,was estimated by measuring h-AFP and h-albumin concentrations in sera,respectively,as well as by immunohistochemical analyses of h-AFP and human cytokeratin 18 in histological sections.RESULTS:The h-GCCs successfully engrafted,repopulated,and colonized the livers of mice in group A(RI = 22.0% ± 2.6%).These mice had moderately differentiated adenocarcinomatous lesions with disrupted glandular structures,which is a characteristics feature of gastric cancers.The serum h-AFP level reached 211.0 ± 142.2 g/mL(range,7.1-324.2 g/mL).In group B mice,the h-GCCs and h-hepatocytes independently engrafted,repopulated the host liver,and developed colonies(RI = 12.0% ± 6.8% and 66.0% ± 12.3%,respectively).h-GCC colonies also showed typical adenocarcinomatous glandular structures around the h-hepatocyte-colonies.These mice survived for the full 56 day-study and did not exhibit any metastasis of h-GCCs in the extrahepatic regions during the observational period.The mice with an h-hepatocyte-repopulated liver possessed metastasized h-GCCs and therefore could be a useful humanized liver animal model for studying liver cancer metastasis in vivo.CONCLUSION:A novel animal model of human liver cancer metastasis was established using the uPA/SCID mouse line.This model could be useful for in vivo testing of anti-cancer drugs and for studying the mechanisms of human liver cancer metastasis.Shinsuke Fujiwara Hikaru Fujioka Chise Tateno Ken Taniguchi Masahiro Ito Hiroshi Ohishi Rie Utoh Hiromi Ishibashi Takashi Kanematsu Katsutoshi Yoshizato 2012World Journal of Gastroenterology2012,18,29:6
7Comparison between bioartificial and artificial liver for the treatment of acute liver failure in pigs显示文摘AIM: To characterize and evaluate the therapeuti efficacy of bioartificial liver (BAL) as compared to tha of continuous hemodiafiltration (CHDF) with plasma exchange (PE), which is the current standard therapy fo fulminant hepatic failure (FHF) in Japan. METHODS: Pigs with hepatic devascularization were divided into three groups: (1) a non-treatment group (NT; n = 4); (2) a BAL treatment group (BAL; n = 4) (3) a PE + CHDF treatment group using 1.5 L of norma porcine plasma with CHDF (PE + CHDF, n = 4). Our BA system consisted of a hollow fiber module with 0.2 μm pores and 1 x 1010 of microcarrier-attached hepatocyte inoculated into the extra-fiber space. Each treatment wa initiated 4 h after hepatic devascularization. RESULTS: The pigs in the BAL and the PE + CHD groups survived longer than those in the NT group. The elimination capacity of blood ammonia by both BAL and PE + CHDF was significantly higher than that in NT Aromatic amino acids (AAA) were selectively eliminated by BAL, whereas both AAA and branched chain amino acids, which are beneficial for life, were eliminated by PE + CHDF. Electrolytes maintenance and acid-base balance were better in the CPE + CHDF group than that in the BAL group. 8Secular change in lifetime of granitic crust and the continental growth: A new view from detrital zircon ages of sandstones显示文摘U-Pb ages of detrital zircons were newly dated for 4 Archean sandstones from the Pilbara craton in Australia, Wyoming craton in North America, and Kaapvaal craton in Africa. By using the present results with previously published data, we compiled the age spectra of detrital zircons for 2.9, 2.6, 2.3,1.0, and0.6 Ga sandstones and modern river sands in order to document the secular change in age structure of continental crusts through time. The results demonstrated the following episodes in the history of continental crust:(1) low growth rate of the continents due to the short cycle in production/destruction of granitic crust during the Neoarchean to Paleoproterozoic(2.9-23 Ga),(2) net increase in volume of the continents during Paleo-to Mesoproterozoic(2.3-1.0 Ga), and(3) net decrease in volume of the continents during the Neoproterozoic and Phanerozoic(after 1.0 Ga). In the Archean and Paleoproterozoic, the embryonic continents were smaller than the modern continents, probably owing to the relatively rapid production and destruction of continental crust. This is indeed reflected in the heterogeneous crustal age structure of modern continents that usually have relatively small amount of Archean crusts with respect to the post-Archean ones. During the Mesoproterozoic, plural continents amalgamated into larger ones comparable to modern continental blocks in size. Relatively older crusts were preserved in continental interiors, whereas younger crusts were accreted along continental peripheries.In addition to continental arc magmatism, the direct accretion of intra-oceanic island arc around continental peripheries also became important for net continental growth. Since 1.0 Ga, total volume of continents has decreased, and this appears consistent with on-going phenomena along modern active arc-trench system with dominant tectonic erosion and/or arc subduction. 9Prediction of colorectal tumor grade and invasion depth through narrow-band imaging scoring显示文摘AIM To determine the usefulness of assigning narrow-band imaging(NBI) scores for predicting tumor grade and invasion depth in colorectal tumors.METHODS A total of 161 colorectal lesions were analyzed from138 patients who underwent endoscopic or surgical resection after conventional colonoscopy and magnifying endoscopy with NBI. The relationships between the surface and vascular patterns of the lesions, as visualized with NBI, and the tumor grade and depth of submucosa(SM) invasion were determined histopathologically.Scores were assigned to distinct features of the surface microstructures of tubular and papillary-type lesions.Using a multivariate analysis, a model was developed for predicting the tumor grade and depth of invasion based on NBI-finding scores.RESULTS NBI findings that correlated with a high tumor grade were associated with the 'regular/irregular'(P < 0.0001)surface patterns and the 'avascular area' pattern(P =0.0600). The vascular patterns of 'disrupted vessels'(P = 0.0714) and 'thick vessels'(P = 0.0133) but none of the surface patterns were associated with a depth of invasion of ≥ 1000 μm. In our model, a total NBIfinding score ≥ 1 was indicative of a high tumor grade(sensitivity: 0.97; specificity: 0.24), and a total NBIfinding score ≥ 9(sensitivity: 0.56; specificity: 1.0) was predictive of a SM invasion depth ≥ 1000 μm. Scores less than these cutoff values signified adenomas and a SM invasion depth < 1000 μm, respectively. 10Actively-controlled Beds for Ambulances显示文摘During transportation by ambulance, a patient is exposed to inertial acceleration when an ambulance decelerates or turns a corner. Such acceleration often gives a patient physical stress such as blood pressure variation or body sway, which causes strong pain, feeling of discomfort or sometimes critical damage for seriously injured persons. To reduce this undesirable effect of the acceleration, the authors developed the actively-controlled bed (ACB) which controls the posture of a stretcher in real time to reduce foot-to-head and lateral acceleration acting on a supine person. This paper describes development of the ACB, including control system design and performance evaluation. The control system is designed by Zakian s framework, which comprises the principle of matching and the method of inequalities, so that the design specifications on the tracking error and the motor torque are satisfied. From the results of driving experiments and simulation, it is estimated that the ACB can reduce the acceleration acting on a patient by 65% in the foot-to-head direction and by 75% in the lateral direction.Takahiko Ono Hikaru Inooka 2009International Journal of Automation and computing2009,6,1:4

15mPGES-1 expression in non-cancerous liver tissue impacts on postoperative recurrence of HCC显示文摘AIM:To investigate whether microsomal prostaglandin E synthase-1 (mPGES-1) expression in hepatocellular carcinoma (HCC) and in non-cancerous liver affects HCC prognosis after hepatectomy. METHODS: The relationship between patient clinical prof iles, tumor factors, surgical determinants, and mPGES-1 expression and the recurrence-free survival rate were examined in 64 patients who underwent curative hepatectomy between March 2003 and December 2006. RESULTS: The scores for mPGES-1 expression were higher in well differentiated and moderately differentiated HCC tissues than in poorly differentiated HCC tissues (well differentiated, 5.1 ± 2.7; moderately differentiated, 5.1 ± 1.7; poorly differentiated, 3.0 ± 1.8). In noncancerous liver tissues, the mPGES-1 levels were higher in injured liver tissues than in normal tissues. Cirrhotic livers had higher mPGES-1 levels than livers with chronic hepatitis (normal livers, 3.3 ± 0.7; chronic hepatitic livers, 5.4 ± 1.9; cirrhotic livers, 6.4 ± 1.6). A univariate analysis revealed that the recurrence-free survival rate was signif icantly lower in patients with vascular invasion,a higher mPGES-1 level in non-cancerous liver tissue,a larger tumor diameter (≥5 cm), and a lower serum albumin level (≤3.7 g/dL). The mPGES-1 expression in HCC tissues did not correlate well with postoperative recurrence.
