1Evolution of disease phenotype in adult and pediatric onset Crohn's disease in a population-based cohort显示文摘AIM:To investigate the evolution of disease phenotypein adult and pediatric onset Crohn's disease(CD) populations,diagnosed between 1977 and 2008.METHODS:Data of 506 incident CD patients were analyzed(age at diagnosis:28.5 years,interquartile range:22-38 years).Both in-and outpatient records were collected prospectively with a complete clinical follow-up and comprehensively reviewed in the population-based Veszprem province database,which included incident patients diagnosed between January 1,1977 and December 31,2008 in adult and pediatric onset CD populations.Disease phenotype according to the Montreal classification and long-term disease course was analysed according to the age at onset in time-dependent univariate and multivariate analysis.RESULTS:Among this population-based cohort,seventy-four(12.8%) pediatric-onset CD patients were identified(diagnosed ≤ 17 years of age).There was no significant difference in the distribution of disease behavior between pediatric(B1:62%,B2:15%,B3:23%) and adult-onset CD patients(B1:56%,B2:21%,B3:23%) at diagnosis,or during follow-up.Overall,the probability of developing complicated disease behaviour was 49.7% and 61.3% in the pediatric and 55.1% and 62.4% in the adult onset patients after 5-and 10-years of follow-up.Similarly,time to change in disease behaviour from non stricturing,non penetrating(B1) to complicated,stricturing or penetrating(B2/B3) disease was not significantly different between pediatric and adult onset CD in a Kaplan-Meier analysis.Calendar year of diagnosis(P = 0.04),ileal location(P < 0.001),perianal disease(P < 0.001),smoking(P = 0.038) and need for steroids(P < 0.001) were associated with presence of,or progression to,complicated disease behavior at diagnosis and during follow-up.A change in disease location was observed in 8.9% of patients and it was associated with smoking status(P = 0.01),but not with age at diagnosis.CONCLUSION:Long-term evolution of disease behavior was not different in pediatric-and adult-onset CD patients in this population-based cohort but was associated to location,perianal disease and smoking status.Barbara Dorottya Lovasz Laszlo Lakatos Agnes Horvath Istvan Szita Tunde Pandur Michael Mandel Zsuzsanna Vegh Petra Anna Golovics Gabor Mester Mihaly Balogh Csaba Molnar Erzsebet Komaromi Lajos Sandor Kiss Peter Laszlo Lakatos 2013World Journal of Gastroenterology2013,19,14:2
2Autonomic and sensory nerve dysfunction in primary biliary cirrhosis显示文摘AIM: Cardiovascular autonomic and peripheral sensory neuropathy is a known complication of chronic alcoholic and non-alcoholic liver diseases. We aimed to assess the prevalence and risk factors for peripheral sensory nerve and autonomic dysfunction using sensitive methods in patients with primary biliary cirrhosis (PBC). METHODS: Twenty-four AMA M2 positive female patients with clinical, biochemical and histological evidence of PBC and 20 age matched healthy female subjects were studied. Five standard cardiovascular reflex tests and 24-h heart rate variability (HRV) analysis were performed to define autonomic function. Peripheral sensory nerve function on median and peroneal nerves was characterized by current perception threshold (CPT), measured by a neuroselective diagnostic stimulator (Neurotron, Baltimore, MD). RESULTS: Fourteen of 24 patients (58%) had at least one abnormal cardiovascular reflex test and thirteen (54%) had peripheral sensory neuropathy. Lower heart rate response to deep breathing (P=0.001), standing (P=0.03) and Valsalva manoeuvre (P=0.01), and more profound decrease of blood pressure after standing (P=0.03) was found in PBC patients than in controls. As a novel finding we proved that both time domain and frequency domain parameters of 24-h HRV were significantly reduced in PBC patients compared to controls. Each patient had at least one abnormal parameter of HRV. Lower CPT values indicated hyperaesthesia as a characteristic feature at peroneal nerve testing at three frequencies (2000 Hz: P=0.005; 250 Hz: P=0.002; 5 Hz: P=0.004) in PBC compared to controls. Correlation of autonomic dysfunction with the severity and duration of the disease was observed. Lower total power of HRV correlated with lower CPT values at median nerve testing at 250 Hz (P = 0.0001) and at 5 Hz (P=0.002), as well as with those at peroneal nerve testing at 2000 Hz (P=0.01). CONCLUSION: Autonomic and sensory nerve dysfunctions are frequent in PBC. Twenty-four-hour HRV analysis is more sensitive than standard cardiovascular tests for detecting of both parasympathetic and sympathetic impairments. Our novel data suggest that hyperaesthesia is a characteristic feature of peripheral sensory neuropathy and might contribute to itching in PBC. 14Infrared neural stimulation and inhibition using an implantable silicon photonic microdevice显示文摘Brain is one of the most temperature sensitive organs.Besides the fundamental role of temperature in cellular metabolism,thermal response of neuronal populations is also significant during the evolution of various neurodegenerative diseases.For such critical environmental factor,thorough mapping of cellular response to variations in temperature is desired in the living brain.So far,limited efforts have been made to create complex devices that are able to modulate temperature,and concurrently record multiple features of the stimulated region.In our work,the in vivo application of a multimodal photonic neural probe is demonstrated.Optical,thermal,and electrophysiological functions are monolithically integrated in a single device.The system facilitates spatial and temporal control of temperature distribution at high precision in the deep brain tissue through an embedded infrared waveguide,while it provides recording of the artefact-free electrical response of individual cells at multiple locations along the probe shaft.Spatial distribution of the optically induced temperature changes is evaluated through in vitro measurements and a validated multi-physical model.The operation of the multimodal microdevice is demonstrated in the rat neocortex and in the hippocampus to increase or suppress firing rate of stimulated neurons in a reversible manner using continuous wave infrared light(λ=1550 nm).Our approach is envisioned to be a promising candidate as an advanced experimental toolset to reveal thermally evoked responses in the deep neural tissue.Agoston Csaba Horvath Sandor Borbely Ors Csanad Boros Lili Komaromi Pal Koppa Peter Bartho Zoltan Fekete 2020Microsystems & Nanoengineering2020,6,1:0
15Pediatric dog bite injuries in the USA: a systematic review显示文摘Introduction Dog bites are one of the leading causes of non-fatal emergency room visits in children.These injuries not only cause physical harm but can lead to long-term psychological stress.This study evaluated the current literature related to pediatric dog bite injuries to identify research gaps which should be prioritized to improve a major public health concern.Methods We performed a keyword search of PubMed,Scopus,and OVID Medline databases(January 1980-March 2020)for all published studies focused on dog bite injuries in the pediatric population(≤18 years of age)using the Preferred Reporting Items for Systematic Reviews and Meta-Analyses.Results Out of 1859 abstracts screened,43 studies involving 86880 patients were included.Twenty-nine studies were retrospective chart reviews characterizing the epidemiology of dog bites and their associated treatment outcomes;six were prospective cohort studies;two were cross-sectional studies;and six were experimental studies.Synthesized results demonstrate that children<9 years of age suffer the greatest burden of injuries,with children<6 years of age at higher risk of more severe injuries involving the head,neck,and face.Conclusion Studies analyzing the prevention or psychosocial consequences of dog bites injuries are needed.Kelli Nicole Patterson Kyle Z Horvath Peter C Minneci Rajan Thakkar LeeAnn Wurster Dana L Noffsinger Tran Bourgeois Katherine J Deans 2022World Journal of Pediatric Surgery2022,5,2:0
