1中国在2100年前消除子宫颈癌的预算优化策略研究显示文摘2018年,世界卫生组织总干事公布了全球消除子宫颈癌的目标,即发病率降至4/10万以下。中国目前人群人乳头瘤病毒(HPV)疫苗接种率和宫颈癌筛查覆盖率均较低,子宫颈癌发病率以年均10.2%的速度增长。本研究开发了一种适应中国人群并经校准的杂交模型,在预测老龄化、城市化和性行为可能改变的趋势下,估计2015—2100年城市和农村每年的子宫颈癌发病率,并确定在不同预算投入下实现消除子宫颈癌目标的最优防控策略。研究结果显示,若不增加子宫颈癌防控预算,到2100年,子宫颈癌发病率预计将达到2015年的3倍;若2020年开始采用优化的防控策略,即95%的12岁女孩接种HPV疫苗,扩大45岁女性终生一次宫颈癌筛查的覆盖率,使城市女性覆盖率达90%、农村女性覆盖率达33%,则预计2072(95%CI:2070—2074)年在城市、2074(95%CI:2072—2076)年在农村可实现消除子宫颈癌的目标;若预算不受限制,最早在2057年(2053—2060年)和2060年(2057—2063年)将分别在城市、农村消除子宫颈癌。中国可通过优化宫颈癌综合防控策略,尽早在适龄人群中引入HPV疫苗来实现消除宫颈癌的目标;并可通过增加预算来加速消除进程,大幅减少罹患子宫颈癌的人数。赵宇倩 乔良(编译) Xia CF Hu SY Xu Xq 2019中华预防医学杂志2019,53,11:15
