2苯中毒骨髓损伤的特征显示文摘目的对23例苯中毒病人的周围血及骨髓进行分析,探讨苯中毒骨髓损伤的特征和机制。方法23例住院苯中毒患者,平均离岗及诊断苯中毒时间2.7年;21例为慢性轻度苯中毒,2例为慢性重度苯中毒(苯中毒再生障碍性贫血);男8例,女15例,平均年龄(45.5±7.8)岁。按统一调查表询问职业史、毒物接触史、药物接触史、造血系统疾病史以及肿瘤史;实验室检查包括:血常规、骨髓常规以及骨髓活检、荧光原位杂交技术(FISH)、流式细胞仪和TCR受体基因重排分析。结果(1)23例苯中毒病人中,血常规检查6例正常、系减少1例、一系或二系减少16例(其中14例淋巴细胞数减少)。血中大颗粒淋巴细胞(LGL)百分比增加(平均29.6%,n=22)。(2)23例苯中毒病人骨髓检查均示有一系或二系发育不全,骨髓损害的特征包括:显著的红系病态造血,嗜酸细胞发育不良,中性粒细胞前体细胞中异常的胞浆颗粒,嗜血细胞生成,髓质退化及骨髓增生低下。(3)T细胞受体(TCR)可变区基因重排(14/23),CD4+/CD8+比例减低(0.77±0.48,n=23)。(4)未发现研究对象染色体结构异常。结论与MDS各亚型常见的骨髓表现不同,慢性苯中毒骨髓损伤的特征是:显著的红系病态造血,嗜酸细胞发育不良,中性粒细胞前体细胞中异常的胞浆颗粒,嗜血细胞生成,髓质退化及骨髓增生低下;脱离苯接触若干年后,骨髓的这些损害仍持续存在。吕玲 林国为 邹和建 王小钦 Liming Bao Richard D Irons 2007中国工业医学杂志2007,20,1:8
3细胞遗传学和间期FISH在淋巴瘤诊断中的应用显示文摘目的评估细胞遗传学和荧光原位杂交技术(FISH)对临床和(或)细针穿刺细胞学检查诊断为淋巴瘤或不能除外淋巴瘤病例的价值。方法同时采用常规组织学、免疫组织化学、细胞遗传学检查和间期FISH技术方法,对223例直径≥1.5cm淋巴结的手术活检标本进行系统性研究。FISH探针采用直接荧光标记的IGH LSI双色分开探针。结果2例患者的取材因坏死成分较多无法诊断。221例患者的病理诊断中,霍奇金淋巴瘤(HL)44例(19.9%),非霍奇金淋巴瘤(NHL)162例(73.3%),其他恶性病变4例(1.8%),良性病变11例(5.0%)。间期FISH结果显示,HL的免疫球蛋白重链基因(IGH)异常的发生率为13.6%(6/44),NHL为51.2%(83/162),良性病变为0%(0/11),差异有统计学意义(P〈0.001)。细胞遗传学和间期FISH联合检查时,HL和NHL的检出率分别提高到15.9%和77.8%,其中有3例为组织病理学和免疫组化检查不能定性者。结论间期FISH是检测IGH基因异常的快速和敏感的手段。细胞遗传学和间期FISH检测是淋巴瘤诊断的重要辅助手段。胡夕春 包黎明 朱雄增 Irons Richard D 傅华 陈燕 陈慧 赵欣旻 杨新苗 李进 2007中华肿瘤杂志2007,29,1:3
