| 1 | Isolation of circulating tumor cells in non-small-cell-lung-cancer patients using a multi-flow microfluidic channel显示文摘Circulating tumor cells(CTCs)carry a wealth of information on primary and metastatic tumors critical for precise cancer detection,monitoring,and treatment.Numerous microfluidic platforms have been developed in the past few years to capture these rare cells in patient bloodstream for deciphering the critical information needed.However,the practical need for a high-quality method of CTC isolation remains to be met.Herein,we demonstrate a novel multi-flow microfluidic device that is able to sensitively provide high purity(>87%)of separation outcome without labeling.Our device is constructed and configured based on the phenomenal effect of size-dependent inertial migration.The recovery rate of>93%has been achieved using spiked cancer cells at clinically relevant concentrations(10 cells per 5 mL and above).We have also successfully detected CTCs from 6 out of 8 non-small-cell-lung-cancer(NSCLC)patients,while none for 5 healthy control subjects.With these results,we envision our approach is a promising alternative for reliable CTC capture,and thus for facilitating the progress of extracting information from CTCs to personalize treatment strategies for solid tumor patients. | Jian Zhou Arutha Kulasinghe Amanda Bogseth Ken O’Byrne Chamindie Punyadeera Ian Papautsky | 2019 | Microsystems & Nanoengineering2019，5，1： | 9 |
| 2 | Anti-tumour necrosis factor agent and liver injury:literature review,recommendations for management显示文摘Abnormalities in liver function tests,including transient and self-limiting hypertransaminasemia,cholestatic disease and hepatitis,can develop during treatment with anti-tumour-necrosis-factor(TNF)therapy.The optimal management of liver injury related to antiTNF therapy is still a matter of debate.Although some authors recommend discontinuing treatment in case of both a rise of alanine aminotransferase more than5 times the upper limit of normal,or the occurrence of jaundice,there are no standard guidelines for the management of anti-TNF-related liver injury.Bibliographical searches were performed in Pub Med,using the following key words:inflammatory bowel disease(IBD);TNF inhibitors;hypertransaminasemia;drugrelated liver injury;infliximab.According to published data,elevation of transaminases in patients with IBD treated with anti-TNF is a common finding,but resolution appears to be the usual outcome.Anti-TNF agents seem to be safe with a low risk of causing severe drugrelated liver injury.According to our centre experience,we found that hypertransaminasemia was a common,mainly self-limiting finding in our IBD cohort and was not correlated to infliximab treatment on both univariate and multivariate analyses.An algorithm for the management of liver impairment occurring during antiTNF treatment is also proposed and this highlights the need of a multidisciplinary approach and suggests liver biopsy as a key-point in the management decision in case of severe rise of transaminases.However,hepatic injury is generally self-limiting and drug withdrawal seems to be an exception. | Roberta Elisa Rossi Ioanna Parisi Edward John Despott Andrew Kenneth Burroughs James O'Beirne Dario Conte Mark Ian Hamilton Charles Daniel Murray | 2014 | World Journal of Gastroenterology2014，20，46： | 7 |

| 5 | Cytoplasmic expression of p27^(kip1) is associated with a favourable prognosis in colorectal cancer patients显示文摘AIM: To evaluate the prognostic significance of p27kip1 in colorectal cancer patients. METHODS: Cytoplasmic and nuclear p27kip1 expression was evaluated in 418 colorectal cancers using tissue microarrays. Data were associated with known patient and tumor variables and long-term patient outcomes, providing further insight into the mechanisms by which p27kip1 may influence tumor development. RESULTS: Nuclear and cytoplasmic p27Kip1 expressions were detected in 59% and 19% of tumors respectively. Cytoplasmic p27Kip1 was almost invariably associated with positive nuclear p27Kip1 expression. Neither case correlat- ed with known clinical or pathological variables, includ- ing tumor stage, grade or extramural vascular invasion. Furthermore, nuclear p27kip1 expression had no impact on survival. However, we identified a significant correla- tion between expression of cytoplasmic p27kip1 and longer disease-specific survival times. | 8 | Acute colonic pseudo-obstruction: A systematic review of aetiology and mechanisms显示文摘AIM To critically review the literature addressing the definition, epidemiology, aetiology and pathophysiology of acute colonic pseudo-obstruction(ACPO).METHODS A systematic search was performed to identify articles investigating the aetiology and pathophysiology of ACPO. A narrative synthesis of the evidence was undertaken. RESULTS No consistent approach to the definition or reporting of ACPO has been developed, which has led to overlapping investigation with other conditions. A vast array of risk factors has been identified, supporting a multifactorial aetiology. The pathophysiological mechanisms remain unclear, but are likely related to altered autonomic regulation of colonic motility, in the setting of other predisposing factors. CONCLUSION Future research should aim to establish a clear and consistent definition of ACPO, and elucidate the pathophysiological mechanisms leading to altered colonic function.
