| 2 | Treatment of biliary tract cancer with NVP-AEW541:Mechanisms of action and resistance显示文摘AIM:To investigate in vitro treatment with NVPAEW541,a small molecule inhibitor of insulin-like growth factor-1 receptor(IGF-1R),in biliary tract cancer (BTC),since this disease is associated with a poor prognosis due to wide resistance to chemotherapeutic agents and radiotherapy.METHODS:Cell growth inhibition by NVP-AEW541 was studied in vitro in 7 human BTC cell lines by automated cell counting.In addition,the anti-tumoral mechanism of NVP-AEW541 was studied by Western blotting,cell cycle analysis and reverse transcription-polymerase chain reaction(RT-PCR).Anti-tumoral drug effect in combination with gemcitabine,5-fluorouracil(5-FU)and Polo-like kinase 1 inhibitor BI2536 was also studied. RESULTS:In vitro treatment with NVP-AEW541 suppressed growth in all human BTC cell lines,however response was lower in gallbladder cancer.Treatment withNVP-AEW541 was associated with dephosphorylation of IGF-1R and AKT.In contrast,phosphorylation of p42/p44 and Stat3 and expression of Bcl-xL were inconsistently downregulated.In addition,treated cells showed cell cycle arrest at the G1/S-checkpoint and an increase in sub-G1 peak.Moreover,IGF-1R and its ligands IGF-1 and IGF-2 were co-expressed in RT-PCR,suggesting an autocrine loop of tumor cell activation.Combined with gemcitabine,NVP-AEW541 exerted synergistic effects, particularly at low concentrations,while effects of combination with 5-FU or BI 2536 were only additive. | 3 | A randomized open-label trial of on-demand rabeprazole vs ranitidine for patients with non-erosive reflux disease显示文摘AIM:To compare the efficacy of the proton-pump inhibitor,rabeprazole,with that of the H 2-receptor antagonist,ranitidine,as on-demand therapy for relieving symptoms associated with non-erosive reflux disease(NERD).METHODS:This is a single center,prospective,randomized,open-label trial of on-demand therapy with rabeprazole(group A) vs ranitidine(group B) for 4 wk.Eighty-three patients who presented to the American University of Beirut Medical Center with persistent gastroesophageal reflux disease(GERD) symptoms and a normal upper gastrointestinal endoscopy were eligible for the study.Patients in group A(n = 44) were allowed a maximum rabeprazole dose of 20 mg twice daily,while those in group B(n = 39) were allowed a maximum ranitidine dose of 300 mg twice daily.Efficacy was assessed by patient evaluation of global symptom relief,scores of the SF-36 quality of life(QoL) questionnaires,total number of pills used,and number of medication-free days.RESULTS:Among the 83 patients who were enrolled in the study,76 patients(40 in the rabeprazole group and 36 in the ranitidine group) completed the 4-wk trial.Baseline characteristics were comparable between both groups.After 4 wk,there was no significant difference in the subjective global symptom relief between the rabeprazole and the ranitidine groups(71.4% vs 65.4%,respectively;P = 0.9).There were no statistically significant differences between mean cumulative scores of the SF-36 QoL questionnaire for the two study groups(rabeprazole 22.40 ± 27.53 vs ranitidine 17.28 ± 37.06;P = 0.582).There was no significant difference in the mean number of pills used(rabeprazole 35.70 ± 29.75 vs ranitidine 32.86 ± 26.98;P = 0.66).There was also no statistically significant difference in the mean number of medication-free days between both groups.CONCLUSION:Rabeprazole has a comparable efficacy compared to ranitidine when given on-demand for the treatment of NERD.Both medications were associated with improved quality of life. | Abdallah A Kobeissy Jana G Hashash Faek R Jamali Assaad M Skoury Reham Haddad Sarah El-Samad Rami Ladki Rola Aswad Assaad M Soweid | 2012 | World Journal of Gastroenterology2012，18，19： | 4 |
| 4 | Value of a comprehensive geriatric assessment for predicting one-year outcomes in patients undergoing transcatheter aortic valve implantation: results from the CGA-TAVI multicentre registry显示文摘Background In a three-month report from the CGA-TAVI registry, we found the Multidimensional Prognostic Index (MPI) and Short Physical Performance Battery (SPPB) to be of value for predicting short-term outcomes in elderly patients undergoing transcatheter aortic valve implantation (TAVI). In the present analysis, we examined the association of these tools with outcomes up to one year post-TAVI. Methods CGA-TAVI is an international, observational registry of geriatric patients undergoing TAVI. Patients were assessed using the MPI and SPPB. Efficacy of baseline values and any postoperative change for predicting outcome were established using logistic regression. Kaplan- Meier analysis was carried out for each comprehensive geriatric assessment tool, with survival stratified by risk category. Results One year after TAVI, 14.1% of patients deceased, while 17.4% met the combined endpoint of death and/or non-fatal stroke, and 37.7% the combined endpoint of death and/or hospitalisation and/or non-fatal stroke. A high-risk MPI score was associated with an increased risk of all-cause mortality (aOR = 36.13, 95% CI: 2.77–470.78, P = 0.006) and death and/or non-fatal stroke (aOR = 10.10, 95% CI: 1.48–68.75, P = 0.018). No significant associations were found between a high-risk SPPB score and mortality or two main combined endpoints. In contrast to a worsening SPPB, an aggravating MPI score at three months post-TAVI was associated with an increased risk of death and/or non-fatal stoke at one year (aOR = 95.16, 95% CI: 3.41–2657.01).
