1Clostridium difficile infections in China显示文摘Clostridium difficile (C. difficile) infection has become one of the major hospital-associated infections in West-ern countries in the last two decades. However, there is limited information on the status of C. difficile infection inChinese healthcare settings. Given the large and increasing elderly population and the well-recognized problem ofover-prescribing of broad spectrum antibiotics in China, it is critical to understand the epidemiology and potentialrisk factors that may contribute to C. difficile infection in China. A literature review of available published studies,including those in Chinese language-based journals, was conducted. A review of the currently available literaturesuggested the presence of C. difficile infections in China, but also suggested that these infections were not particu-larly endemic. This finding should lead to better designed and greatly expanded studies to provide a more reliableepidemiologically-based conclusion on the actual status of C. difficile infection in China, including the identifica-tion of any associated risk factors. 2Multiphase computed tomography radiomics of pancreatic intraductal papillary mucinous neoplasms to predict malignancy显示文摘BACKGROUND Intraductal papillary mucinous neoplasms(IPMNs)are non-invasive pancreatic precursor lesions that can potentially develop into invasive pancreatic ductal adenocarcinoma.Currently,the International Consensus Guidelines(ICG)for IPMNs provides the basis for evaluating suspected IPMNs on computed tomography(CT)imaging.Despite using the ICG,it remains challenging to accurately predict whether IPMNs harbor high grade or invasive disease which would warrant surgical resection.A supplementary quantitative radiological tool,radiomics,may improve diagnostic accuracy of radiological evaluation of IPMNs.We hypothesized that using CT whole lesion radiomics features in conjunction with the ICG could improve the diagnostic accuracy of predicting IPMN histology.AIM To evaluate whole lesion CT radiomic analysis of IPMNs for predicting malignant histology compared to International Consensus Guidelines.METHODS Fifty-one subjects who had pancreatic surgical resection at our institution with histology demonstrating IPMN and available preoperative CT imaging were included in this retrospective cohort.Whole lesion semi-automated segmentation was performed on each preoperative CT using Healthmyne software(Healthmyne,Madison,WI).Thirty-nine relevant radiomic features were extracted from each lesion on each available contrast phase.Univariate analysis of the 39 radiomics features was performed for each contrast phase and values were compared between malignant and benign IPMN groups using logistic regression.Conventional quantitative and qualitative CT measurements were also compared between groups,viaχ2(categorical)and Mann Whitney U(continuous)variables.RESULTS Twenty-nine subjects(15 males,age 71±9 years)with high grade or invasive tumor histology comprised the'malignant'cohort,while 22 subjects(11 males,age 70±7 years)with low grade tumor histology were included in the'benign'cohort.Radiomic analysis showed 18/39 precontrast,19/39 arterial phase,and 21/39 venous phase features differentiated malignant from benign IPMNs(P<0.05).Multivariate analysis including only ICG criteria yielded two significant variables:thickened and enhancing cyst wall and enhancing mural nodule<5 mm with an AUC(95%CI)of 0.817(0.709-0.926).Multivariable post contrast radiomics achieved an AUC(95%CI)of 0.87(0.767-0.974)for a model including arterial phase radiomics features and 0.834(0.716-0.953)for a model including venous phase radiomics features.Combined multivariable model including conventional variables and arterial phase radiomics features achieved an AUC(95%CI)of 0.93(0.85-1.0)with a 5-fold cross validation AUC of 0.90.CONCLUSION Multi-phase CT radiomics evaluation could play a role in improving predictive capability in diagnosing malignancy in IPMNs.Future larger studies may help determine the clinical significance of our findings.Stuart L Polk Jung W Choi Melissa J McGettigan Trevor Rose Abraham Ahmed Jongphil Kim Kun Jiang Yoganand Balagurunathan Jin Qi Paola T Farah Alisha Rathi Jennifer B Permuth Daniel Jeong 2020World Journal of Gastroenterology2020,26,24:5

5Pancreatic cancer serum biomarker PC-594:Diagnostic performance and comparison to CA19-9显示文摘AIM:To investigate serum PC-594 fatty acid levels as a potential biomarker in North American pancreatic cancer(PaC) patients,and to compare its performance to CA19-9.METHODS:Using tandem mass spectrometry,we evaluated serum PC-594 levels from 84 North American patients with confirmed PaC and 99 cancer-free control subjects.We determined CA19-9 levels by ELISA.Significance between Pa C patients and controls,and association with clinical variables was determined by analysis of variance and t-tests.Diagnostic performance was evaluated by receiver-operator characteristic(ROC)curve analysis,and PC-594 correlation with age and CA19-9 determined by regression analysis.RESULTS:Mean PC-594 levels were 3.7 times lower in Pa C patients compared to controls(P < 0.0001).The mean level in PaC patient serum was 0.76 ± 0.07 μmol/L,and the mean level in control subjects was 2.79 ± 0.15 μmol/L.There was no correlation between PC-594 and age,disease stage or gender(P > 0.05).Using 1.25 μmol/L as a PC-594 threshold produced a relative risk(RR) of 9.4(P < 0.0001,95%CI:5.0-17.7).The area under the receiver-operator characteristic curve(ROCAUC) was 0.93(95%CI:0.91-0.95) for PC-594 and 0.85(95%CI:0.82-0.88) for CA19-9.Sensitivity at 90% specificity was 87% for PC-594 and 71% for CA19-9.Six Pa C patients with CA19-9 above 35 U/m L showed normal PC-594 levels,while 24 Pa C patients with normal CA19-9 showed low PC-594 levels.Eighty-five of the 99 control subjects(86%) showed normal levels of both markers.CONCLUSION:PC-594 biomarker levels are significantly reduced in North American Pa C patients,and showed superior diagnostic performance compared to CA19-9.Shawn A Ritchie Bassirou Chitou Qingan Zheng Dushmanthi Jayasinghe Wei Jin Asuka Mochizuki Dayan B Goodenowe 2015World Journal of Gastroenterology2015,21,21:3
6Common non-synonymous polymorphisms in the BRCA1 Associated RING Domain (BARD1) gene are associated with breast cancer susceptibility:a case-control analysis显示文摘The BRCA1 Associated RING Domain(BARD1) gene has been identified as a high penetrance gene for breast cancer, whose germline and somatic mutations were reported in both non-BRCA1/2 hereditary site-specific and sporadic breast cancer cases. BARD1 plays a crucial role in tumor repression, along with its heterodimeric partner BRCA1. In the current study, we tested the hypothesis that common non-synonymous polymorphisms in BARD1 are associated with breast cancer susceptibility in a case-control study of 507 patients with incident breast cancer and 539 frequency-matched cancer-free controls in Chinese women. We genotyped all three common(minor allele frequency (MAF) > 0.10) non-synonymous polymorphisms(Pro24Ser, Arg378Ser, and Val507Met) in BARD1. We found that the BARD1 Pro24Ser variant genotypes(24Pro/Ser and 24Ser/Ser) and Arg378Ser variant homozygote 378Ser/Ser were associated with a significantly decreased breast cancer risk, compared with their wild-type homozygotes, respectively. Furthermore, a significant locus-locus interaction was evident between Pro24Ser and Arg378Ser (P-int = 0.032). Among the 378Ser variant allele carriers, the 24Pro/Pro wild-type homozygote was associated with a significantly increased breast cancer risk (adjusted OR = 1.81, 95% CI = 1.11-2.95), but the subjects having 24Pro/Ser or Ser/Ser variant genotypes had a significantly decreased risk(adjusted OR = 0.74, 95% CI = 0.56-0.99). In stratified analysis, this locus-locus interaction was more evident among subjects without family cancer history, those with positive estrogen receptor (ER) and individuals with negative progesterone receptor(PR). These findings indicate that the potentially functional polymorphisms Pro24Ser and Arg378Ser in BARD1 may jointly contribute to the susceptibility of breast cancer.Huo, X Hu, Z. B Zhai, X.
