1Transjugular intrahepatic portosystemic shunt vs endoscopic therapy in preventing variceal rebleeding显示文摘AIM:To compare early use of transjugular intrahepatic portosystemic shunt(TIPS) with endoscopic treatment(ET) for the prophylaxis of recurrent variceal bleeding.METHODS:In-patient data were collected from 190 patients between January 2007 and June 2010 who suffured from variceal bleeding.Patients who were older than 75 years;previously received surgical treatment or endoscopic therapy for variceal bleeding;and complicated with hepatic encephalopathy or hepatic cancer,were excluded from this research.Thirty-five cases lost to follow-up were also excluded.Retrospective analysis was done in 126 eligible cases.Among them,64 patients received TIPS(TIPS group) while 62 patients received endoscopic therapy(ET group).The relevant data were collected by patient review or telephone calls.The occurrence of rebleeding,hepatic encephalopathy or other complications,survival rateand cost of treatment were compared between the two groups.RESULTS:During the follow-up period(median,20.7 and 18.7 mo in TIPS and ET groups,respectively),rebleeding from any source occurred in 11 patients in the TIPS group as compared with 31 patients in the ET group(Kaplan-Meier analysis and log-rank test,P = 0.000).Rebleeding rates at any time point(6 wk,1 year and 2 year) in the TIPS group were lower than in the ET group(Bonferroni correction α' = α/3).Eight patients in the TIPS group and 16 in the ET group died with the cumulative survival rates of 80.6% and 64.9%(Kaplan-Meier analysis and log-rank test c2 = 4.864,P = 0.02),respectively.There was no significant difference between the two groups with respect to 6-wk survival rates(Bonferroni correction α' = α/3).However,significant differences were observed between the two groups in the 1-year survival rates(92% and 79%) and the 2-year survival rates(89% and 64.9%)(Bonferroni correction α' = α/3).No significant differences were observed between the two treatment groups in the occurrence of hepatic encephalopathy(12 patients in TIPS group and 5 in ET group,KaplanMeier analysis and log-rank test,c2 = 3.103,P = 0.08).The average total cost for the TIPS group was higher than for ET group(Wilcxon-Mann Whitney test,52 678 RMB vs 38 844 RMB,P < 0.05),but hospitalization frequency and hospital stay during follow-up period were lower(Wilcxon-Mann Whitney test,0.4 d vs 1.3 d,P = 0.01;5 d vs 19 d,P < 0.05).CONCLUSION:Early use of TIPS is more effective than endoscopic treatment in preventing variceal rebleeding and improving survival rate,and does not increase occurrence of hepatic encephalopathy.Hui Xue Meng Zhang Jack XQ Pang Fei Yan Ying-Chao Li Liang-Shan Lv Jia Yuan Muna Palikhe Wei-Zhi Li Zhi-Lun Wang 2012World Journal of Gastroenterology2012,18,48:17

5Agricultural Risk Factors Influence Microbial Ecology in Honghu Lake显示文摘Agricultural activities, including stock-farming, planting industry, and fish aquaculture,can affect the physicochemical and biological characters of freshwater lakes. However, the effects of pollution producing by agricultural activities on microbial ecosystem of lakes remain unclear.Hence, in this work, we selected Honghu Lake as a typical lake that is influenced by agriculture activities. We collected water and sediment samples from 18 sites, which span a wide range of areas from impacted and less-impacted areas. We performed a geospatial analysis on the composition of microbial communities associated with physicochemical properties and antibiotic pollution of samples. The co-occurrence networks of water and sediment were also built and analyzed. Our results showed that the microbial communities of impacted and less-impacted samples of water were largely driven by the concentrations of TN, TP, NO_3^--N, and NO_2^--N, while those of sediment were affected by the concentrations of Sed-OM and Sed-TN. Antibiotics have also played important roles in shaping these microbial communities: the concentrations of oxytetracycline and tetracycline clearly reflected the variance in taxonomic diversity and predicted functional diversity between impacted and less-impacted sites in water and sediment samples, respectively. Furthermore, for samples from both water and sediment, large differences of network topology structures between impacted and less-impacted were also observed. 6Long-term safety of laser in situ keratomileusis in eyes with thin corneas: 5-year follow-up显示文摘● AIM: To investigate the long term(≥5 y) efficacy, predictability, and safety of laser in situ keratomileusis(LASIK) in eyes with thin corneas [central corneal thickness(CCT) <500 μm].● METHODS: A total of 339 patients met the criteria of this study. Finally, 175 eyes of 89 patients who had thin corneas and underwent LASIK≥5 y ago returned to our clinic and included in this study. Preoperative parameters recorded included uncorrected visual acuity(UCVA), corrected distance visual acuity(CDVA), manifest refraction, CCT and corneal topography. At returning visits, in addition to visual acuity and manifest refraction, ultrasound CCT and corneal topography were performed. Optical coherence tomography was used to measure the CCT, LASIK flap thickness, and residual stromal bed thickness(RSBT). Safety index, efficacy index, percentage of eyes within ±0.5 D and ±1.0 D of refraction, percent tissue altered(PTA), and percentage stromal bed thickness(PSBT) were calculated.● RESULTS: The safety index was 1.09 and efficacy index was 0.99. The percentages of eyes within ±0.5 D and ±1.0 D were 71.2% and 87.7%, respectively. The mean PTA was 40%±6%(range 20% to 55%); 76 eyes(43.4%) had PTA <40% and 99 eyes(56.6%) had PTA≥40%. The mean RSBT was 303±27 μm(range 240 to 390 μm), and 2 eyes had RSBT<250 μm. The mean PSBT was 61%±9%(range 51% to 85%). 8Relationship between mode of sport training and general cognitive performance显示文摘Purpose: To investigate whether athletes who engage in different modes of sports training correspondingly exhibit different patterns of performance on general cognition tasks.Methods: Sixty participants were recruited into an endurance, motorically complex, or control group, and were administered a series of physical tests and neuropsychological assessments.Results: Athletes in the endurance group demonstrated the highest levels of cardiovascular fitness and those in the motorically complex group exhibited the highest levels of motor fitness. Nonetheless, no differences in cognitive performance were observed between the 3 groups.Conclusion: These findings indicate that the mode of sport training, which results in either high cardiovascular or high motor fitness, bears no relationship to measures of general cognition in elite athletes. 15Unmet needs as indicator of improving chronic care delivery system in China显示文摘In 2009, China strengthened its public health service system. Since then, the country has made remarkable achievements in community-based chronic disease prevention and control;however, certain groups still have unmet needs. During 2019 to 2029, China will consolidate the top-level design of its medical health system. During this period, the coordination of department policies, improvement of service delivery mechanisms, building an integrated health service system, and other issues will be highlighted. This study will provide a basis for designing China’s chronic disease prevention and control system during the next stage of development. We will consider the unmet needs of patients with chronic diseases as an indicator for remodeling the prediction system in combination with the elements and structural theories of complex health systems. In this article, we first introduce the definition and measurement methods of unmet needs. Second, we identify the existing unmet needs found among patients with chronic diseases with reference to the chronic disease prevention and control policies of China as well as current service items. Finally, we propose the design of community chronic disease service package for the next development stage based on unmet needs of patients with chronic diseases. 17In vivo biocompatibility and degradability of a Zn-Mg-Fe alloy osteosynthesis system显示文摘Zinc is generally considered to be one of the most promising materials to be used in biodegradable implants,and many zinc alloys have been optimized to improve implant biocompatibility,degradation,and mechanical properties.However,long-term degradation leads to the prolonged presence of degradation products,which risks foreign body reactions.Herein,we investigated the in vivo biocompatibility and degradation of a biodegradable Zn-Mg-Fe alloy osteosynthesis system in the frontal bone,mandible,and femur in beagles for 1 year.Results of the routine blood,biochemical,trace element,and histological analyses of multiple organs,peripheral blood CD4/CD8a levels,and serum interleukin 2 and 4 levels showed good biocompatibility of the Zn-Mg-Fe alloy.Zinc content analysis revealed zinc accumulation in adjacent bone tissue,but not in the liver,kidney,and spleen,which was related to the degradation of the Zn-Mg-Fe alloy.The alloy demonstrated a uniform slowing degradation rate in vivo.No degradation differences in the frontal bone,mandible,and femur were observed.The degradation products included zinc oxide[ZnO],zinc hydroxide[Zn(OH)_(2)],hydrozincite[Zn_(5)(OH)_(6)(CO_(3))_(2)],and hopeite[Zn_(3)(PO_(4))_(2)⋅4H_(2)O].The good biocompatibility and degradation properties of the Zn-Mg-Fe alloy render it a very attractive osteosynthesis system for clinical applications.Xiaoxi Shao Xiang Wang Fangfang Xu Taiqiang Dai Jack G.Zhou Jiang Liu Kun Song Lei Tian Bin Liu Yanpu Liu 2022Bioactive Materials2022,7,1:1
