Safety and efficacy of tenofovir in chronic hepatitis B-related decompensated cirrhosis显示文摘AIM To evaluate the safety and efficacy of tenofovir disoproxil fumarate(TDF) as a first-line therapy in decompensated liver disease. METHODS We enrolled 174 chronic hepatitis B-related liver cirrhosis patients treated with 300 mg/d TDF at six Korean centers. Of the 174 cirrhosis patients, 57 were assigned to the decompensated cirrhosis group and 117 were assigned to the compensated cirrhosis group. We followed the patients for 12 mo and evaluated clinical outcomes, including biochemical, virological, and serological responses. We also evaluated changes in hepatic and renal function and compared the decompensated and compensated cirrhosis groups. RESULTS The 1-year complete virological response(CVR) and Hepatitis B e antigen(HBe Ag) seroconversion were seen in 70.2% and 14.2% in the decompensated cirrhosis group, respectively. The rates of HBe Ag seroconversion/loss and ALT normalization at month 12 were similar in both groups. TDF treatment was also effective for decreasing the level of hepatitis B virus(HBV) DNA in both groups, but CVR was higher in the compensated group(88.9% vs 70.2%, P = 0.005). Tenofovir treatment for 12 mo resulted in improved Child-Turcotte-Pugh(CTP) and model for end-stage liver disease(MELD) scores in decompensated group(P < 0.001). Of the 57 decompensated patients, 39(68.4%) achieved CTP class A and 27(49.1%) showed improvement in the CTP score of 2 points after 12 mo of TDF. The observed rate of confirmed 0.5 mg/d L increases in serum levels of creatinine in the decompensated and compensated cirrhosis group were 7.0% and 2.5%, respectively(P < 1.000).CONCLUSION TDF therapy in decompensated cirrhosis patients was effective for decreasing HBV DNA levels and improving hepatic function with relatively lower CVR than in compensated cirrhosis. Thus, physicians should carefully monitor not only renal function but also treatment responses when using TDF in decompensated cirrhosis patients. | Soon Kyu Lee Myeong Jun Song Seok Hyun Kim Byung Seok Lee Tae Hee Lee Young Woo Kang Suk Bae Kim Il Han Song Hee Bok Chae Soon Young Ko Jae Dong Lee | 2017 | World Journal of Gastroenterology2017,23,13: | 14
Conventional endoscopic features are not sufficient to differentiate small, early colorectal cancer显示文摘AIM:To evaluate the depth of invasion of small, early colorectal cancers(ECCs)using conventional endoscopic features.METHODS:From January 2005 to September 2011,colonoscopy cohort showed that a total of 72 patients with small colorectal cancers with the size less than 20mm underwent colonoscopy at the Yonsei University College of Medicine,Seoul,South Korea.Among them,8 patients were excluded due to incomplete medical records. Finally,a total of 64 ECCs with submucosa(SM)invasion and size less than 20 mm were included. One hundred fifty-two adenomas with size less than 20 mm were included as controls.Nine endoscopic features, including seven morphological findings(i.e.,loss of lobulation, excavation, demarcated and depressed areas, stalk swelling, fullness, fold convergence, and bleeding ulcers),pit patterns, and non-lifting signs, were evalu-ated retrospectively. All endoscopic features were evaluated by two experienced endoscopists who have each performed over 1000 colonoscopies annually for more than five years without knowledge of the histology.RESULTS:Among the morphological findings, the size of deep submucosal cancers was bigger than that of superficial lesions(16.9 mm vs 12.3 mm,P<0.001).Also,demarcated depressed areas, stalk swelling, and fullness were more common in deep SM cancers than in superficial tumors(demarcated depressed areas:52.0%vs 15.7%,P<0.001;stalk swelling:100%vs4.2%,P<0.001;fullness:25.0%vs 0%,P=0.001).Among deep SM cancers,96%of polyps showed invasive pit patterns, whereas 19.4%of superficial tumors showed invasive pit patterns(P<0.001).A positive non-lifting sign was more common in deep SM cancers(85.0%vs 28.6%,P<0.001).Diagnostic accuracy of invasive morphology, invasive pit patterns, and nonlifting signs for deep SM cancers were 71%,82%,and75%,respectively.CONCLUSION:Conventional endoscopic findings were insufficient to discriminate small, deep SM cancers from superficial SM cancers by white light, standard colonoscopy. | Wan Park Bun Kim Soo Jung Park Jae Hee Cheon Tae Il Kim Won Ho Kim Sung Pil Hong | 2014 | World Journal of Gastroenterology2014,20,21: | 11
Characterization of smooth muscle, enteric nerve, interstitial cells of Cajal, and fibroblast-like cells in the gastric musculature of patients with diabetes mellitus显示文摘AIM To investigate histologic abnormalities in the gastric smooth muscle of patients with diabetes mellitus(DM).METHODS Full-thickness gastric specimens were obtained from patients undergoing surgery for gastric cancer. H&E stain and Masson's Trichrome stain were performed to assess the degree of fibrosis. Immunohistochemical staining using various antibodies was also performed [antibodies against protein gene product 9.5(PGP9.5), neuronal nitric oxide synthase(n NOS), vasoactive intestinal peptide(VIP), neurokinin-1(NK1) receptor, c-Kit, and platelet-derived growth factor receptor-alpha,(PDGFRα)]. Immunofluorescent staining and evaluation with confocal microscopy were also conducted.RESULTS Twenty-six controls and 35 diabetic patients(21 shortduration patients and 14 long-duration patients) were included. There were no significant differences in basic demographics between the two groups except in mean body mass index(BMI)(higher in the DM group). Proportions of moderate-to-severe intercellular fibrosis in the muscle layer were significantly higher in the DM group than in the control group(P < 0.01). On immunohistochemical staining, c-Kit- and PDGFRα-positive immunoreactivity were significantly decreased in the DM group compared with the control group(P < 0.05). There were no statistically significant differences in PGP9.5, n NOS, VIP, and neurokinin 1 expression. On immunofluorescent staining, cellularity of interstitial cells of Cajal(ICC) was observed to decrease with increasing duration of DM.CONCLUSION Our study suggests that increased intercellular fibrosis, loss of ICC, and loss of fibroblast-like cells are found in the smooth muscle of DM patients. These abnormalities may contribute to changes in gastric motor activity in patients with DM. | Kyung Sik Park Kwang Bum Cho Il Seon Hwang Jae Hyung Park Byung Ik Jang Kyeong Ok Kim Sung Woo Jeon Eun Soo Kim Chang Sik Park Joong Goo Kwon | 2016 | World Journal of Gastroenterology2016,22,46: | 11
Pretreated quercetin protects gerbil hippocampal CA1 pyramidal neurons from transient cerebral ischemic injury by increasing the expression of antioxidant enzymes显示文摘Quercetin(QE; 3,5,7,3′,4′-pentahydroxyflavone), a well-known flavonoid, has been shown to prevent against neurodegenerative disorders and ischemic insults. However, few studies are reported regarding the neuroprotective mechanisms of QE after ischemic insults. Therefore, in this study, we investigated the effects of QE on ischemic injury and the expression of antioxidant enzymes in the hippocampal CA1 region of gerbils subjected to 5 minutes of transient cerebral ischemia. QE was pre-treated once daily for 15 days before ischemia. Pretreatment with QE protected hippocampal CA1 pyramidal neurons from ischemic injury, which was confirmed by neuronal nuclear antigen immunohistochemistry and Fluoro-Jade B histofluorescence staining. In addition, pretreatment with QE significantly increased the expression levels of endogenous antioxidant enzymes Cu/Zn superoxide dismutase, Mn superoxide dismutase, catalase and glutathione peroxidase in the hippocampal CA1 pyramidal neurons of animals with ischemic injury. These findings demonstrate that pretreated QE displayed strong neuroprotective effects against transient cerebral ischemia by increasing the expression of antioxidant enzymes. | Bai Hui Chen Joon Ha Park Ji Hyeon Ahn Jeong Hwi Cho In Hye Kim Jae Chul Lee Moo-Ho Won Choong-Hyun Lee In Koo Hwang Jong-Dai Kim Il Jun Kang Jun Hwi Cho Bich Na Shin Yang Hee Kim Yun Lyul Lee Seung Min Park | 2017 | Neural Regeneration Research2017,12,2: | 9
Aerodynamic design optimization of helicopter rotor blades including airfoil shape for hover performance显示文摘This study proposes a process to obtain an optimal helicopter rotor blade shape for aerodynamic performance in hover flight. A new geometry representation algorithm which uses the class function/shape function transformation (CST) is employed to generate airfoil coordinates. With this approach, airfoil shape is considered in terms of design variables. The optimization process is constructed by integrating several programs developed by author. The design variables include twist, taper ratio, point of taper initiation, blade root chord, and coefficients of the airfoil distribution function. Aerodynamic constraints consist of limits on power available in hover and forward flight. The trim condition must be attainable. This paper considers rotor blade configuration for the hover flight condition only, so that the required power in hover is chosen as the objective function of the optimization problem. Sensitivity analysis of each design variable shows that airfoil shape has an important role in rotor performance. The optimum rotor blade reduces the required hover power by 7.4% and increases the figure of merit by 6.5%, which is a good improvement for rotor blade design. | Ngoc Anh Vu Jae Woo Lee Jung Il Shu | 2013 | Chinese Journal of Aeronautics2013,26,1: | 8
Double layered self-expanding metal stents for malignant esophageal obstruction, especially across the gastroesophageal junction显示文摘AIM: To evaluate the clinical outcomes of double-layered self-expanding metal stents (SEMS) for treatment of malignant esophageal obstruction according to whether SEMS crosses the gastroesophageal junction (GEJ). METHODS: Forty eight patients who underwent the SEMS insertion for malignant esophageal obstruction were enrolled. Patients were classified as GEJ group (SEMS across GEJ, 18 patients) and non-GEJ group (SEMS above GEJ, 30 patients) according to SEMS position. Double layered (outer uncovered and inner covered stent) esophageal stents were placed. RESULTS: The SEMS insertion and the clinical improvement were achieved in all patients in both groups. Stent malfunction occurred in seven patients in the GEJ group and nine patients in the non-GEJ group. Tumor overgrowth occurred in five and eight patients, respectively, food impaction occurred in one patient in each group, and stent migration occurred in one and no patient, respectively. There were no significant differences between the two groups. Reflux esophagitis occurred more frequently in the GEJ group (eight vs five patients, P = 0.036) and was controlled by proton pump inhibitor. Aspiration pneumonia occurred in zero and five patients, respectively, and tracheoesophageal fistula occurred in zero and two patients, respectively. CONCLUSION: Double-layered SEMS are a feasible and effective treatment when placed across the GEJ for malignant esophageal obstruction. Double-layered SEMS provide acceptable complications, especially migration, although reflux esophagitis is more common in the GEJ group. | Min Dae Kim Su Bum Park Dae Hwan Kang Jae Hyung Lee Cheol Woong Choi Hyung Wook Kim Chung Uk Chung Young Il Jeong | 2012 | World Journal of Gastroenterology2012,18,28: | 5
Development of tenofovir disoproxil fumarate resistance after complete viral suppression in a patient with treatment-na?ve chronic hepatitis B: A case report and review of the literature显示文摘Tenofovir disoproxil fumarate(TDF) is a potent nucleo-tide analogue that is recommended as first-line therapy for patients with chronic hepatitis B. The results of a longitudinal study of TDF treatment demonstrated no development of resistance. We observed one treatment-na?ve chronic hepatitis B(CHB) patient who developed TDF resistance after complete viral suppression during long-term TDF treatment. A 37-year-old HBe Ag-positive man received TDF 300 mg/d for 43 mo. The hepatitis B virus(HBV) DNA titer was 8 log_(10) copies/m L at baseline and became undetectable at 16 mo after treatment. However, the HBV DNA titer rebounded to 7.5 log_(10) copies/m L at 43 mo after treatment. We performed full sequencing to find mutation sites associated with virologic breakthrough. The results showed 9 mutation sites, most of which had not been well-known as mutation sites. We changed the therapy from tenofovir to entecavir with a regimen of 0.5 mg once daily. After 4 mo, the HBV DNA titer decreased to 267 copies/m L, and the liver enzyme levels were normalized. | Woo Hee Cho Hyun Jae Lee Ki Bae Bang Seok Bae Kim Il Han Song | 2018 | World Journal of Gastroenterology2018,24,17: | 5
Correlations between endoscopic and clinical disease activity indices in intestinal Behcet's disease显示文摘AIM:To develop a novel endoscopic severity model of intestinal Behcet's disease(BD) and to evaluate its feasibility by comparing it with the actual disease activity index for intestinal Behcet's disease(DAIBD).METHODS:We reviewed the medical records of 167 intestinal BD patients between March 1986 and April 2011.We also investigated the endoscopic parameters including ulcer locations, distribution, number, depth, shape, size and margin to identify independent factors associated with DAIBD.An endoscopic severity model was developed using significant colonoscopic variables identified by multivariate regression analysis and its correlation with the DAIBD was evaluated. To determine factors related to the discrepancy between endoscopic severity and clinical activity, clinical characteristics and laboratory markers of the patients were analyzed.RESULTS:A multivariate regression analysis revealed that the number of intestinal ulcers(≥ 2,P = 0.031) and volcanoshaped ulcers(P = 0.001) were predictive factors for the DAIBD.An endoscopic severity model(Y) was developed based on selected endoscopic variables as follows:Y = 47.44 + 9.04 × non-Ileocecal area + 11.85 ×≥ 2 of intestinal ulcers + 5.03 × shallow ulcers + 12.76 × deep ulcers + 4.47 × geographicshaped ulcers + 26.93 × volcano-shaped ulcers + 8.65 ×≥ 20 mm of intestinal ulcers. However, endoscopic parameters used in the multivariate analysis explained only 18.9% of the DAIBD variance.Patients with severe DAIBD scores but with moderately predicted disease activity by the endoscopic severity model had more symptoms of irritable bowel syndrome(21.4% vs 4.9%,P = 0.026) and a lower rate of corticosteroid use(50.0% vs 75.6%,P = 0.016) than those with severe DAIBD scores and accurately predicted disease by the model.CONCLUSION:Our study showed that the number of intestinal ulcers and volcano-shaped ulcers were predictive factors for severe DAIBD scores. However, the correlation between endoscopic severity and DAIBD(r = 0.434) was weak. | Hyun Jung Lee Youn Nam Kim Hui Won Jang Han Ho Jeon Eun Suk Jung Soo Jung Park Sung Pil Hong Tae Il Kim Won Ho Kim Chung Mo Nam Jae Hee Cheon | 2012 | World Journal of Gastroenterology2012,18,40: | 5
Clinical outcome of Fitz-Hugh-Curtis syndrome mimicking acute biliary disease显示文摘AIM: To analyze the clinical characteristics of patients diagnosed with Fitz-Hugh-Curtis syndrome.METHODS: The clinical courses of patients that visited St. Mary's Hospital with abdominal pain from January 2005 to December 2006 and were diagnosed with Fitz-Hugh-Curtis syndrome were examined.RESULTS: Fitz-Hugh-Curtis syndrome was identified in 22 female patients of childbearing age; their mean age was 31.0 ± 8.1 years. Fourteen of these cases presented with pain in the upper right abdomen alone or together with pain in the lower abdomen, and six patients presented with pain only in the lower abdomen. The first impression at the time of visit was acute cholecystitis or cholangitis in 10 patients and acute appendicitis or pelvic inflammatory disease in eight patients. Twenty-one patients were diagnosed by abdominal computer tomograghy (CT), and the results of abdominal sonography were normal for 10 of these patients. Chlamydia trichomatis was isolated from 18 patients. Two patients underwent laparoscopic adhesiotomy and 20 patients were completely cured by antibiotic treatment.CONCLUSION: For women of childbearing age with acute pain in the upper right abdomen alone or together with pain in the lower abdomen, Fitz-Hugh-Curtis syndrome should be considered during differential diagnosis. Moreover, in cases suspected to be Fitz-Hugh-Curtis syndrome, abdominal CT, rather than abdominal sonography, assists in the diagnosis. | Seong Yong Woo Jin Il Kim Dae Young Cheung Se Hyun Cho Soo-Heon Park Joon-Yeol Han Jae Kwang Kim | 2008 | World Journal of Gastroenterology2008,14,45: | 5
Sex-dependent difference in the effect of metformin on colorectal cancer-specific mortality of diabetic colorectal cancer patients显示文摘AIM To assess factors associated with the higher effect of metformin on mortality in diabetic colorectal cancer(CRC) patients, since the factors related to the effectiveness of metformin have not been identified yet.METHODS Between January 2000 and December 2010,413 patients diagnosed with both stage 3/4 CRC and diabetes mellitus were identified. Patients' demographics and clinical characteristics were analyzed. The effect of metformin on CRC-specific mortality and the interactions between metformin and each adjusted factor were evaluated.RESULTS Total follow-up duration was median 50 mo(range: 1-218 mo). There were 85 deaths(45.9%) and 72 CRC-specific deaths(38.9%) among 185 patients who used metformin, compared to 130 total deaths(57.0%) and 107 CRC-specific deaths(46.9%) among 228 patients who did not use metformin. In multivariate analysis, survival benefit associated with metformin administration was identified(HR = 0.985,95%CI: 0.974-0.997,P = 0.012). Interaction test between metformin and sex after adjustment for relevant factors revealed that female CRC patients taking metformin exhibited a significantly lower CRC-specific mortality rate than male CRC patients taking metformin(HR = 0.369,95%CI: 0.155-0.881,P = 0.025). Furthermore, subgroup analysis revealed significant differences in CRC-specific mortality between the metformin and non-metformin groups in female patients(HR = 0.501,95%CI: 0.286-0.879,P = 0.013) but not male patients(HR = 0.848,95%CI: 0.594-1.211,P = 0.365). There were no significant interactions between metformin and other adjusted factors on CRC-specific mortality.CONCLUSION We showed a strong sex-dependent difference in the effect of metformin on CRC-specific mortality in advanced stage CRC patients with diabetes. | Jung Won Park Jin Ha Lee Ye Hyun Park Soo Jung Park Jae Hee Cheon Won Ho Kim Tae Il Kim | 2017 | World Journal of Gastroenterology2017,23,28: | 3

Risk of sedation for diagnostic esophagogastroduodenoscopy in obstructive sleep apnea patients显示文摘AIM: To investigate whether patients with obstructive sleep apnea (OSA) are at risk of sedation-related complications during diagnostic esophagogastroduodenoscopy (EGD). METHODS: A prospective study was performed in consecutive patients with OSA, who were confirmed with full-night polysomnography between July 2010 and April 2011. The occurrence of cardiopulmonary complications related to sedation during diagnostic EGD was compared between OSA and control groups. RESULTS: During the study period, 31 patients with OSA and 65 controls were enrolled. Compared with the control group, a higher dosage of midazolam was administered (P = 0.000) and a higher proportion of deep sedation was performed (P = 0.024) in the OSA group. However, all adverse events, including sedation fail-ure, paradoxical responses, snoring or apnea, hypoxia, hypotension, oxygen or flumazenil administration, and other adverse events were not different between the two groups (all P > 0.1). Patients with OSA were not predisposed to hypoxia with multivariate logistic regression analysis (P = 0.068). CONCLUSION: In patients with OSA, this limited sized study did not disclose an increased risk of cardiopulmonary complications during diagnostic EGD under sedation. | Jae Myung Cha Jung Won Jeun Kwan Mi Pack Joung Il Lee Kwang Ro Joo Hyun Phil Shin Won-Chul Shin | 2013 | World Journal of Gastroenterology2013,19,29: | 2
