	1	Ipsilateral proximal and distal radius fractures with unstable elbow joint: Which should we address first?显示文摘Simultaneous ipsilateral fractures involving radial head and distal end of radius are uncommon. We present our thoughts on which fracture should be addressed first. A 68-year-old lady sustained an ipsilateral fracture of the right radial head and distal end of radius following a fall. Clinically her right elbow was posteriorly dislocated and right wrist was deformed. Plain radiographs showed an intraarticular fracture of the distal end of radius and a comminution radial head fracture with a proximally migrated radius. Magnetic resonance imaging (MRI) showed no significant ligament injuries. We addressed her distal radius first with an an atomical lock ing plate followed by her radial head with a radial head replacement. Our rationale to treat the distal end radius: first was to obtain a correct alignment of Lister's tubercle and correct the distal radius height. Lister's tubercle was used to guide for the correct rotation of the radial head prosthesis. Correcting the distal end fracture radial height helped us with length selection of the radial head prosthesis and address the proximally migrated radial shaft and neck. Postoperative radiographs showed an acceptable reduction. The Cooney score was 75 at 3 months postoperatively, which was equivalent to a fair functional outcome. 	2	A mouse model of weight-drop closed head injury:emphasis on cognitive and neurological deficiency显示文摘Traumatic brain injury(TBI) is a leading cause of death and disability in individuals worldwide.Producing a clinically relevant TBI model in small-sized animals remains fairly challenging.For good screening of potential therapeutics,which are effective in the treatment of TBI,animal models of TBI should be established and standardized.In this study,we established mouse models of closed head injury using the Shohami weight-drop method with some modifications concerning cognitive deficiency assessment and provided a detailed description of the severe TBI animal model.We found that 250 g falling weight from 2 cm height produced severe closed head injury in C57BL/6 male mice.Cognitive disorders in mice with severe closed head injury could be detected using passive avoidance test on day 7 after injury.Findings from this study indicate that weight-drop injury animal models are suitable for further screening of brain neuroprotectants and potentially are similar to those seen in human TBI. 	12	Is more evidence needed for thrombectomy in basilar artery occlusion? The BASICS and BEST meta- analytical approaches显示文摘INTRODUCTION The benefit of mechanical thrombectomy for stroke associated with anterior circulation large vessel occlusion(LVO)is well established and among the most effective treatments in medicine.1 Stroke associated with basilar artery occlusion(BAO)was not included in the seminal thrombectomy randomised clin-ical trials(RCTs).BAO accounts for 1%of the ischaemic strokes and 5%-10%of the LVO,but 70%-80%of BAO victims evolve to death or full dependence.23 The terminal nature of the brainstem and thalamic circulations may predispose to such dismal outcomes.Only two RCT designed to assess the efficacy of early(<6-8 hours since symptoms onset)thrombectomy for BAO treatment,the BEST and BASICS trials.24 Given the prior evidence for thrombectomy benefit for anterior circu-lation stroke is well settled;BAO stroke shares a similar physiopathology with very poor prog-nosis.
