3Cardiovascular drugs in the treatment of infantile hemangioma显示文摘Since the introduction of propranolol in the treatment of complicated infantile hemangiomas(IH) in 2008, other different beta-blockers, including timolol, acetabutolol, nadolol and atenolol, have been successfully used for the same purpose. Various hypotheses including vasoconstriction, inhibition of angiogenesis and the induction of apoptosis in proliferating endothelial cells have been advanced as the potential beta-blockerinduced effect on the accelerated IH involution, although the exact mechanism of action of beta-blockers remains unknown. This has generated an extraordinary interest in IH research and has led to the discovery of the role of the renin-angiotensin system(RAS) in the biology of IH, providing a plausible explanation for the beta-blocker induced effect on IH involution and the development of new potential indications for RAS drugs such as angiotensin-converting enzyme inhibitors and angiotensin-receptor blockers in the treatment of IH. 5Extramural vascular invasion and response to neoadjuvant chemoradiotherapy in rectal cancer: influence of the CpG island methylator phenotype显示文摘AIM To identify whether Cp G island methylator phenotype(CIMP) is predictive of response to neoadjuvant chemoradiotherapy(NACRT) and outcomes in rectal cancer.METHODS Patients undergoing NACRT and surgical resection for rectal cancer in a tertiary referral centre between 2002-2011 were identified. Pre-treatment tumour biopsies were analysed for CIMP status(high, intermediate or low) using methylation specific PCR. KRAS and BRAF status were also determined using pyrosequencing analysis. Clinical information was extracted from case records and cancer services databases. Response to radiotherapy was measured by tumour regression scores determined uponhistological examination of the resected specimen. The relationship between these molecular features, response to NACRT and oncological outcomes were analysed.RESULTS There were 160 patients analysed with a median followup time of 46.4 mo. Twenty-one(13%) patients demonstrated high levels of CIMP methylation(CIMP-H) and this was significantly associated with increased risk of extramural vascular invasion(EMVI) compared with CIMP-L [8/21(38%) vs 15/99(15%), P = 0.028]. CIMP status was not related to tumour regression after radiotherapy or survival, however EMVI was significantly associated with adverse survival(P < 0.001). Intermediate CIMP status was significantly associated with KRAS mutation(P = 0.01). There were 14(9%) patients with a pathological complete response(pC R) compared to 116(73%) patients having no or minimal regression after neoadjuvant chemoradiotherapy. Those patients with pC R had median survival of 106 mo compared to 65.8 mo with minimal regression, although this was not statistically significant(P = 0.26). Binary logistic regression analysis of the relationship between EMVI and other prognostic features revealed, EMVI positivity was associated with poor overall survival, advanced 'T' stage and CIMP-H but not nodal status, age, sex, KRAS mutation status and presence of local or systemic recurrence.CONCLUSION We report a novel association of pre-treatment characterisation of CIMP-H with EMVI status which has prognostic implications and is not readily detectable on pre-treatment histological examination.Jeremy Stuart Williamson Huw Geraint Jones Namor Williams Anthony Paul Griffiths Gareth Jenkins John Beynon Dean Anthony Harris 2017World Journal of Gastrointestinal Oncology2017,9,5:4

7Dynamics of soil organic carbon following land-use change:insights from stable C-isotope analysis in black soil of Northeast China显示文摘Intensive soil tillage is a significant factor in soil organic matter decline in cultivated soils. Both cultivation abandonment and foregoing tillage have been encouraged in the past 30 years to reduce greenhouse gas emissions and soil erosion. However, the dynamic processes of soil organic carbon (SOC) in areas of either continuous cultivation or abandonment remain unclear and inconsistent.Our aims were to assess and model the dynamic processes of SOC under continuous tillage and after cultivation abandonment in the black soil of Northeast China. Soil profiles were collected of cultivated or abandoned land with cultivation history of 0–100 years. An isotope mass balance equation was used to calculate the proportion of SOC derived from corn debris (C_4) and from natural vegetation (C_3) to deduce the dynamic process. Approximately 40% of SOC in the natural surface soil (0–10 cm) was eroded in the first 5 years of cultivation, increasing to about 75% within 40 years, before a slow recovery. C_4 above 30 cm soil depth increased by 4.5%–5% or 0.11–0.12 g·kg^(-1) on average per year under continuous cultivation, while it decreased by approximately 0.34% annually in the surface soil after cultivation abandonment.The increase in the percentage of C_4 was fitted to a linear equation with given intercepts in the upper 30 cm of soil in cultivated land. A significant relationship between the change of C_4 and time was found only in the surface soil after abandonment of cultivation.
