| 1 | Optimization of high-speed railway pantographs for improving pantograph-catenary contact显示文摘A crucial system for the operation of high-speed trains is the pantograph catenary interface as it is the sole responsible to deliver electrical power to the train. Being the catenary a stationary system with a long lifespan it is also less likely to be redesigned and upgraded than the pantographs that fit the train vehicles. This letter proposes an optimization procedure for the improvement of the contact quality between the pantograph and the catenary solely based on the redesign of the pantograph head suspension characteristics. A pantograph model is defined and validated against experimental dynamic characteristics of existing pantographs. An optimization strategy based on the use of a global optimization method, to find the vicinity of the optimal solution, followed by the use of a deterministic optimization algorithm, to fine tune the optimal solution, is applied here. The spring stiffness, damping characteristics and bow mass are the design variables used for the pantograph optimization. The objective of the optimal problem is the minimization of the standard deviation of the contact force history, which is the most important quantity to define the contact quality. The pantograph head suspension characteristics are allowed to vary within technological realistic limits. | 2 | Timely reperfusion for ST-segment elevation myocardial infarction:Effect of direct transfer to primary angioplasty on time delays and clinical outcomes显示文摘Primary percutaneous coronary intervention(PPCI) is the preferred reperfusion therapy for patients presenting with ST-segment elevation myocardial infarction(STEMI) when it can be performed expeditiously and by experienced operators. In spite of excellent clinical results this technique is associated with longer delays than thrombolysis and this fact may nullify the benefit of selecting this therapeutic option. Several strategies have been proposed to decrease the temporal delays to deliver PPCI. Among them,prehospital diagnosis and direct transfer to the cath lab,by-passing the emergency department of hospitals,has emerged as anattractive way of diminishing delays. The identification is based on input-output data collected from flight tests using both frequency and time domain techniques. The vehicle is an in-house 40 cm wingspan airplane. Because of the complex coupled, multivariable and nonlinear dynamics of the aircraft, linear SISO structures for both the lateral and longitudinal models around a reference state were derived. | 5 | Timing, method and discontinuation of hydrocortisone administration for septic shock patients显示文摘AIM To characterize the prescribing patterns for hydrocortisone for patients with septic shock and perform an exploratory analysis in order to identify the variables associated with better outcomes.METHODS This prospective cohort study included 59 patients with septic shock who received stress-dose hydrocortisone.It was performed at 2 critical care units in academic hospitals from June 1st, 2015, to July 31 st, 2016. Demographic data, comorbidities, medical management details, adverse effects related to corticosteroids, and outcomes were collected after the critical care physician indicated initiation of hydrocortisone. Univariate comparison between continuous and bolus administration of hydrocortisone was performed, including multivariate analysis, as well as Kaplan-Meier analysis to compare the proportion of shock reversal at 7 d after presentation. Receiver operating characteristic(ROC) curves determined the best cut-off criteria for initiation of hydrocortisone associated with the highest probability of shock reversal. We addressed the effects of the taper strategy for discontinuation of hydrocortisone, noting risk of shock relapse and adverse effects.RESULTS All-cause 30-d mortality was 42%. Hydrocortisone was administered as a continuous infusion in 54.2% of patients; time to reversal of shock was 49 h longer in patients who were given a bolus administration [59 h(range, 47.5-90.5) vs 108 h(range, 63.2-189); P = 0.001]. The maximal dose of norepinephrine after initiation of hydrocortisone was lower in patients on continuous infusion [0.19 μg/kg per minute(range, 0.11-0.28 μg)] compared with patients who were given bolus [0.34 μg/kg per minute(range, 0.16-0.49); P = 0.004]. Kaplan-Meier analysis revealed a higher proportion of shock reversal at 7 d in patients with continuous infusion compared to those given bolus(83% vs 63%; P = 0.004). There was a good correlation between time to initiation of hydrocortisone and time to reversal of shock(r = 0.80; P < 0.0001); ROC curve analysis revealed that the best criteria for prediction of shock reversal was a time to initiation of hydrocortisone of ≤ 13 h after administration of norepinephrine, with an area under the curve of 0.81(P < 0.001). The maximal dose of norepinephrine at initiation of hydrocortisone with the highest association with shock reversal was ≤ 0.28 μg/kg per minute, with an area under the curve of 0.75(P = 0.0002). On a logistic regression model, hydrocortisone taper was not associated with a lower risk of shock relapse(RR = 1.29; P = 0.17) but was related to a higher probability of hyperglycemia [odds ratio(OR), 5.3; P = 0.04] and hypokalemia(OR = 10.6; P = 0.01). CONCLUSION Continuous infusion of hydrocortisone could hasten the resolution of septic shock compared to bolus administration. Earlier initiation corresponds with a higher probability of shock reversal. | 6 | A model for strong attenuation and dispersion of seismic P-waves in a partially saturated fractured reservoir显示文摘In this work we interpret the data showing unusually strong velocity dispersion of P-waves (up to 30%) and attenuation in a relatively narrow frequency range. The cross-hole and VSP data were measured in a reservoir, which is in the porous zone of the Silurian Kankakee Limestone Formation formed by vertical fractures within a porous matrix saturated by oil, and gas patches. Such a medium exhibits significant attenuation due to wave-induced fluid flow across the interfaces between different types of inclusions (fractures, fluid patches) and background. Other models of intrinsic attenuation (in particular squirt flow models) cannot explain the amount of observed dispersion when using realistic rock properties. | 8 | Role of the HLA-DQ locus in the development of chronic gastritis and gastric carcinoma in Mexican patients显示文摘AIM: To determine the HLA-DQ locus in Mexican patients with Chronic gastritis and gastric adenocarcinoma. METHODS: Oligotyping for HLA-DQ locus was performed in 45 Mexican patients with chronic gastritis and 13 Mexican patients with diffuse-type gastric adenocarcinoma, and was then compared with 99 clinically healthy unrelated individuals. H pylori infection and CagA status were assessed in patients by enzyme-linked immunosor-bent assay (ELISA) method. RESULTS: We found a significant increased frequency of HLA-DQB1*0401 allele in H pylori-positive patients with chronic gastritis when compared with healthy subjects [19 vs 0%, P= 1 × 10-7, odds ratio (OR) = 4.96; 95% confidence interval (95% CI), 3.87-6.35]. We also found a significant increased frequency of HLA-DQB1*0501 in patients with diffuse-type gastric carcinoma in comparison with healthy individuals (P = 1 × 10-6, OR = 13.07; 95% CI, 2.82-85.14). | 10 | Conservative management of cholestasis with and without fever in acute biliary pancreatitis显示文摘The presence of cholestasis in both mild and severe forms of acute biliary pancreatitis(ABP)does not justify,of itself,early endoscopic retrograde cholangiography(ERC)or endoscopic sphincterotomy(ES).Clinical support treatment of acute pancreatitis for one to two weeks is usually accompanied by regression of pancreatic edema,of cholestasis and by stone migration to the duodenum in 60%-88%of cases.On the other hand,in cases with both cholestasis and fever,a condition usually characterized as ABP associated with cholangitis,early ES is normally indicated.However,in daily clinical practice,it is practically impossible to guarantee the coexistence of cholangitis and mild or severe acute pancreatitis.Pain,fever and cholestasis,as well as mental confusion and hypotension,may be attributed to inflammatory and necrotic events related to ABP. | 12 | Selective ligninolysis of wheat straw and wood chips by the white-rot fungus Lentinula edodes and its influence on in vitro rumen degradability显示文摘Background: The present work investigated the influence of lignin content and composition in the fungal treatment of lignocellulosic biomass in order to improve rumen degradability. Wheat straw and wood chips,differing in lignin composition, were treated with Lentinula edodes for 0, 2, 4, 8 and 12 wk and the changes occurring during fungal degradation were analyzed using pyrolysis-gas chromatography-mass spectrometry and detergent fiber analysis.Results: L. edodes preferentially degraded lignin, with only limited cellulose degradation, in wheat straw and wood chips, leaving a substrate enriched in cellulose. Syringyl(S)-lignin units were preferentially degraded than guaiacyl(G)-lignin units, resulting in a decreased S/G ratio. A decreasing S/G ratio(wheat straw: r =-0.72, wood chips: r =-0.75) and selective lignin degradation(wheat straw: r =-0.69, wood chips: r =-0.88) were correlated with in vitro gas production(IVGP), a good indicator for rumen degradability.Conclusions: L. edodes treatment increased the IVGP of wheat straw and wood chips. | 13 | 体外评估自酸蚀粘结剂的釉质封闭能力显示文摘目的:尽管有报道自酸蚀釉质粘合剂存在粘结不足的缺陷.自酸蚀粘结剂的应用在不断扩大.有些临床医师选择单独酸蚀釉质以弥补其缺陷。本研究目的是评估3种自酸蚀粘结剂在热疲劳后,酸蚀对釉质封闭的影响。材料与方法:取30颗牛切牙,在每颗牙上预备两个箱状洞型,分别用Adper Prompt L-Pop(3MESPE)、Clearfil SE Bond(Kuraray America)和iBond(Heraeus Kulzer)这三种自酸蚀粘结剂中的一种处理窝洞。其中部分先用磷酸酸蚀釉质,另一部分直接涂布自酸蚀粘结剂,然后用FiltekZ250(3MESPE)充填洞型。然后取一半的充填体置于1500个热循环中使之疲劳,再用50%硝酸氨银来检验材料的边缘封闭能力。银离子渗透按O~4分级.材料界面通过黑点探测器在发散区域扫描电镜(FESEM)下分析。用非参数统计计算出一个95%的可信区间。结果:Clearfil SE Bond的釉质微渗漏显著低于另两个自酸蚀粘结剂。对置于热循环后的标本,酸蚀能显著降低所有自酸蚀粘结剂的银离子渗透。对没有经过热循环的标本,酸蚀只能减少iBond的银离子渗透。在FESEM下.银离子沉淀可被发现位于Adper Prompt L-Pop和iBond的粘结层中.而Clearfil SE Bond处理后的10个标本中只有2个表现出银离子沿釉质粘结界面渗透。结论:标本在热循环疲劳状态.磷酸酸蚀能提高3种自酸蚀粘结剂的釉质边缘封闭能力。 | Jorge Perdigǎo Paulo Monteiro George Gomes 李珍(译) | 2009 | 中国口腔医学继续教育杂志2009,,4: | 2 |
