1世界家庭医生组织(WONCA)研究论文摘要汇编——全科医生如何停止或减少不当处方用药以改善多药联合治疗的身体虚弱的超高龄老年慢性病共病患者健康结局的LESS研究显示文摘背景全科医生是否以及采取何种措施为接受多药联合治疗的超高龄(>80岁)老年慢性病共病患者,停止或减少不当处方用药,仍然鲜为人知。本研究旨在探究瑞士全科医生是否以及如何为此类人群停止或减少不当处方用药,并调查其背后的原因。方法本项网络调查研究在来自瑞士的全科医生中开展,调查内容为:对采用了7种药物进行治疗,有或无心血管疾病病史、身体虚弱的超高龄老年慢性病共病患者,全科医生是否为其停用或者减量使用某些药物。分别计算愿意为有或无心血管疾病病史的患者停用或者减量使用至少1种处方药物的全科医生的比例。使用开放性问题调查全科医生为患者停用或者减量使用处方药物的原因。调查影响全科医生为患者停用或者减量使用处方药物的因素。结果共邀请了282例全科医生参加调查,其中157例(56%)做了有效应答,73%为男性,平均年龄为56岁。对于无心血管疾病病史的患者,98%的全科医生为其停用或减量使用了至少1种处方药物(通常为预防心血管疾病的药物),原因是未给患者带来获益。为了减少不良反应,全科医生会为患者减少止痛药用量,或者仅在必要时开止痛药。对于有心血管疾病病史的患者,近乎相同比例的全科医生也做了以上处理。全科医生称,考虑到药物治疗的风险和获益、患者的生活质量和预期寿命,并且重视患者自身的想法及其关注事项,为患者停用或减量使用某些药物。结论在患者未得到获益的情况下,瑞士全科医生愿意为其停用或者减量使用预防心血管疾病的药物,但是倾向于让患者继续使用止痛药。开发工具协助全科医生权衡为患者进行药物治疗的风险和获益,有助于其在临床中为患者停用不当药物或者减量使用某些药物进行治疗。MANTELLI S JUNGO K T ROZSNYAI Z 本刊编辑部 2018中国全科医学2018,21,35:2
