9Mental health of perinatal women显示文摘Pregnancy and childbirth are major stressors for some women. They can be followed by deterioration in mental health status and cause mental illnesses during perinatal period. Undetected and untreated perinatal mental illnesses can have negative unexpected impacts on parenting skills of the women and children's development. Mentally ill mothers may not effectively attend their children's needs in a timely manner and may experience an unfavourable mother-child attachment affecting the child's language, social, emotional and cognitive development. The rate of pregnancy and postnatal health complications and interventions is higher among mentally ill women with some certain risk factors. The mentally ill mothers along with their partners need comprehensive support and counselling to be able to care for their infants and establish strong parent-child bond and attachment. Mental health campaigns across the world have endeavoured to increase the knowledge and awareness of the public towards perinatal mental health illnesses. To this aim, a routine screening is recommended in order to identify the women who are at risk of mood or anxiety disorder during perinatal period. The development of knowledge on perinatal mental illnesses among public and the health professionals has resulted in timely recognition and treatment of perinatal mental illnesses. Although great volumes of research show high prevalence of perinatal mental illnesses and their impacts on parenting confidence and competence as well as child's developmental process, there is still lack of research on various aspects of perinatal mental illnesses. To enable early prevention, diagnosis and intervention, it is crucial to identify families who are at an increased risk of perinatal mental illnesses and provide support and intervention to minimise the adverse outcomes. The children's needs may not be met by providing treatment to parental mental illnesses alone. 10Mechanical behavior of embankments overlying on loose subgrade stabilized by deep mixed columns显示文摘Deep mixed column(DMC) is known as one of the effective methods for stabilizing the natural earth beneath road or railway embankments to control stability and settlements under traffic loads.The load distribution mechanism of embankment overlying on loose subgrades stabilized with DMCs considerably depends on the columns' mechanical and geometrical specifications.The present study uses the laboratory investigation to understand the behavior of embankments lying on loose sandy subgrade in three different conditions:(1) subgrade without reinforcement.(2) subgrade reinforced with DMCs in a triangular pattern and horizontal plan,and(3) subgrade reinforced with DMCs in a square pattern and horizontal plan.For this purpose,by adopting the scale factor of 1:10.a reference embankment with20 cm height,250 cm length,and 93%maximum dry density achieved in standard Proctor compaction test was constructed over a 70 cm thick loose sandy bed with the relative density of 50%in a loading chamber,and its load-displacement behavior was evaluated until the failure occurred.In the next two tests.DMCs(with 10 cm diameter.40 cm length,and 25 cm center-to-center spacing) were placed in groups in two different patterns(square and triangular) in the same sandy bed beneath the embankment and.consequently,the embankments were constructed over the reinforced subgrades and gradually loaded until the failure happened.In all the three tests,the load-displacement behaviors of the embankment and the selected DMCs were instrumented for monitoring purpose.The obtained results implied 64%increase in failure load and 40%decrease in embankment crest settlement when using the square pattern of DMCs compared with those of the reference embankment,while these values were 63%and 12%,respectively,for DMCs in triangular pattern.This confirmed generally better performance of DMCs with a triangular pattern.Morteza Esmaeili Hamid Khajehei 2016Journal of Rock Mechanics and Geotechnical Engineering2016,8,5:1

13Why is there a need for an interdisciplinary approach to assess erectile dysfunction?显示文摘Erectile dysfunction is a prevalent complaint among men. The majority of patients suffering erectile dysfunction exhibit various risk factors of vascular diseases. Erectile dysfunction used to be recognised as one of the consequences of vascular diseases in patients suffering heart attack or myocardial infarction. During the last decade, however, the role of endothelial dysfunction in the occurrence of erectile dysfunction has been signifi ed, and it has been suggested that erectile dysfunction may not be simply a consequence of vascular diseases but an indicator of future vascular problems. Erectile dysfunction has been known as 'the tip of iceberg' of a generalised vascular dysfunction, which typically happens before serious vascular problems. Considerable evidence shows a link between erectile dysfunction and vascular disorders. Several theories have been considered for the association between erectile dysfunction and vascular diseases. One of them is the 'artery size' theory focusing on the differences between the diameter of the penile artery and other arteries. Another theory is based on 'endothelial dysfunction', which highlights inappropriate vasoconstriction as a cause of erectile dysfunction and vascular diseases. 'Age' has also been reported to have pivotal role in the development of vascular dysfunction resulting in erectile dysfunction and ultimately vascular diseases. Another theory explaining the pathophysiology of erectiledysfunction and its relationship with vascular diseases focuses on the formation of atherosclerosis plaques. 15Sexuality after childbirth: Gaps and needs显示文摘Childbirth is a stressful event for a majority of women and can have many consequences one of which is female sexual dysfunction. The main aim of pre- and postnatal health services is to fulfil physical and emotional needs of mothers and babies but not sexual function of women. Also, the fact that sexual satisfaction is part of general well being and mental health is generally neglected. Sexual function of women not only is affected by childbirth, but also is influenced by many other factors. One of these factors is culture and religion. Women's sexual life after childbirth has different meaning in different cultures. In many conservative societies with certain cultural and religious beliefs women are prohibited from having sex after childbirth. In these societies, women hear conflicting stories about risks and benefits of having sexual intercourse during postpartum period the majority of which may not be true. It has been reported that some women may be at greater risk of postpartum sexual dysfunction as neurobiological factors and genetics have been recently suggested to impact female sexual functioning. Considering the multidimensional nature of female sexual dysfunction, this problem cannot be resolved by a simple solution and not all postpartum women can be treated by the same protocol. Various treatment options, such as the use of medications, behavioural interventions and psychotherapy have been investigated in research studies and there is still controversy over the issue. 16Use of iron in perinatal anaemia:Indications for women’s health care policies and procedure显示文摘This paper reviews management of obstetric anaemia and the role of intravenous iron for the treatment of obstetric anaemia.Red blood cell transfusions are routinely used for haemoglobin restoration in anaemic women.The decision for red blood cell transfusion is made on a combination of haemoglobin level and clinical status,and it is suggested that transfusions are not necessary in those who are well compensated or when alternative therapy is available.To reduce the risk,intravenous iron infusion is proposed as a bloodless therapeutic approach.There are a variety of iron preparations.Intravenous iron infusion can reduce the requirement for blood transfusion in hemodynamically stable women with perinatal anaemia,especially in resource-scarce settings.It a cost-effective bloodless approach for the treatment of anaemia than can enhance patient outcomes.According to the literature,when haemoglobin is greater than 90 g/L,blood transfusion is not often required.In perinatal women with anaemia,the decision whether to administer blood or iron is based on patient preferences,haemoglobin levels,clinical symptoms,past and present medical conditions and the clinician’s judgement.Nevertheless,due to the lack of rigid criteria for blood transfusions in the majority of clinical settings,it is considered the default treatment for anaemia in perinatal women.Mike Etemady Melika Hajizadeh Beata Gidaszewski Julie Ann Swain Seng Chai Chua Marjan Khajehei 2023World Journal of Obstetrics and Gynecology2023,12,4:0
17Mental fitness during transition to fatherhood显示文摘Transition into fatherhood is often marked by a period of adjustment,uncertainty and psychological distress and challenges for many men,along with social isolation and relationship problems.Risk factors for paternal mental health issues are maternal depression,marital distress,parenting stress,gender role stress,mismatched expectations for pregnancy and after childbirth,poor physical health,inadequate self-care behaviours,avoiding seeking help for mental health issues,and having a child with sleeping,feeding and temperament problems.Paternal depression,anxiety and post-traumatic stress disorder can have negative impacts on the social and emotional wellbeing of fathers,their partners and their children.Nevertheless,these issues are not widely acknowledged,recognised or treated.Men’s mental health illness is a silent crisis.They often fail to seek help due to their feeling of shame,stigma for a lack of emotional control,distress or anxiety related to utilising mental health support services,and underrating the severity of their symptoms.These necessitate the need for timely attention,psychological support and proper education to minimise their risk of mental health issues.Although research has indicated fathers’inclination toward being included in practices such as the mental health assessment,perinatal education and postnatal educational approaches need to be inclusive of fathers and encourage them to seek support for their paternal mental health issues and parenting difficulties.Marjan Khajehei Julie Ann Swain Elmira Behroozpour Negar Hajizadeh Ali Parvaneh 2022World Journal of Obstetrics and Gynecology2022,11,1:0
18Endorphins, oxytocin, sexuality and romantic relationships: An understudied area显示文摘Endorphins are the body's natural opioids that are created and released by the central nervous system, hypothalamus and pituitary gland. Endorphins have a reputation for pain reduction, enhancing excitement or satisfaction, boosting confidence, enabling control of emotions and generating feelings of euphoria, and are involved in the natural reward cycle. There is also evidence in the literature suggesting the role of endorphins in sexuality(including sexual function and sexual behaviours), as they may regulate the release of sex hormones, prolactin and growth hormone, which are involved in sexual function and love. Endogenous oxytocin is another intrinsic hormone whose role in inducing labour contractions, the delivery of the baby and stimulating lactation has been well studied. However, the potential impact of endorphins and oxytocin on sexuality and romantic relationships is not well understood. This article reviews the research on endorphins and endogenous oxytocin and how they relate to human sexuality and romantic relationships. Some animal studies report the effect of endorphin and oxytocin on sex hormones and mating behaviours, but these findings have not been supported by research into human behaviour, indicating many gaps in knowledge relating to the association between these hormones and human sexuality.Marjan Khajehei Elmira Behroozpour 2018World Journal of Obstetrics and Gynecology2018,7,2:0
