1Surgical outcomes for unilateral superior oblique palsy in Chinese population: a retrospective study显示文摘AIM: To evaluate the outcome after surgery for unilateral superior oblique(SO) palsy in Chinese.METHODS: The medical records of 39 patients that underwent surgery for unilateral SO palsy between January 2003 and December 2012 at Caritas Medical Centre, Hong Kong, were retrospectively reviewed. All surgeries were performed by a single surgeon. Pre-operative assessments for vertical deviation, cyclo-deviation, and Knapp's classification were obtained to determine the nature and degree of surgical correction.Vertical deviation was measured at 1wk; 1, 6mo and on last follow-up day post-operatively. Cyclo-deviation was measured on last follow-up day post-operatively.RESULTS: During the 10 y period, 39 subjects were recruited. The most common etiology was congenital(94.9%). Knapp's Type III(66.7%) and Type I(12.8%)classifications were the most common subtypes. To treat SO palsy, the most common surgical procedures were:isolated inferior oblique(IO) anteriorization(41.0%),isolated IO myectomy(10.3%), and isolated IO recession(10.3%). At 3.5 ±2.1y post-operatively, the vertical deviation was significantly reduced(15.1 ±6.2 PD versus0.5±1.4 PD, P <0.0001) without significant improvement in cyclo-deviation(P =0.5). Initial vertical deviation was correlated with cyclo-torsion(r =0.4, P =0.007). Those with over-correction had greater initial vertical deviation(19.4±7.2 PD versus 13.2±4.3 PD, P =0.003). After a single operation, 84.6% of subjects achieved a vertical deviation within ±3 PD.· CONCLUSION: The majority of subjects achieved corrected vertical deviation after a single surgery although there was no improvement in cyclo-deviation.Those with over-correction of primary position deviation had greater preoperative vertical deviation and it may be related to simultaneous multiple muscle surgery.Gordon Shing Kin Yau Victor Tak Yau Tam Jacky Wai Yip Lee Theo Tak Kwong Chan Can Yin Fun Yuen 2015International Journal of Ophthalmology(English edition)2015,8,1:4
2Clinicopathological survey of 23 patients with lacrimal fossa lesions in Hong Kong显示文摘AIM: To survey the clinicopathological correlations of lacrimal fossa lesions in the Hong Kong population.METHODS: A total of 23 patients with lacrimal fossa lesions were identified radiologically.The mean age at the time of diagnosis was 52 year-old.The medical records of patients diagnosed with lacrimal fossa lesions and subsequently underwent radiological investigations were collected and reviewed retrospectively throughout a six years period from 2000 to 2006 from three regional hospitals (Tung Wah Eastern Hospital,Pamela Youde Nethersole Eastern Hospital and Queen Mary Hospital) belonging to the Hong Kong Mega Cluster.Patient demographics,presenting signs and symptoms,imaging modalities,pathological reports and subsequent management were recorded.RESULTS: The most common initial presenting symptom was upper lid swelling (83%) followed by proptosis (13%).19 patients (83%) had benign lacrimal fossa lesion and 4 patients (17%) had malignancy.Three of the malignancies were lymphomas and one was adenoid cystic carcinoma.CONCLUSION: Age is an important factor in considering the differential diagnosis of lacrimal fossa lesion.Lymphoproliferative diseases have the highest incidence in patients over 60 years of age while inflammatory lesions are more common in younger patients in Hong Kong.Danny Siu-Chun Ng Edwin Chan Kin Yau Clement Wai-Nang Chan 2011International Journal of Ophthalmology(English edition)2011,4,3:3

4COVID-19 outbreaks in residential care homes in Hong Kong and effectiveness of vaccine against severe outcomes显示文摘Background:COVID-19 outbreaks in residential care homes for the elderly(RCHEs)and for persons with disability(RCHDs)have caused significant morbidity and mortality during 5th epidemic in Hong Kong.This article reviewed COVID-19 outbreaks situation and estimated the effectiveness of receiving at least two-dose of COVID-19 vaccine in preventing severe outcomes.Methods:To estimate attack rates and vaccination coverage,documentation on COVID-19 infection and their vaccination records of residential care homes(RCH)residents reported between December 31,2021 and May 31,2022 were reviewed,and infected cases were follow-up for 4 weeks for severe outcomes or death.Correlation between vaccination coverage against attack rate by types of homes was examined.Infected RCH residents with available information were included in the analysis of vaccine effectiveness against severe outcomes and death.Results:COVID-19 vaccination coverage was low in RCHDs(median 0.46,IQR:0.24-0.76)and very low in RCHEs(median 0.08,IQR:0.00-0.19).Higher attack rates were recorded among RCHE residents(median 0.84,IQR:0.64-0.93)and higher case fatality rate(CFR:28.1%)than in RCHDs(median 0.58,IQR:0.31-0.84;CFR:3.9%).The attack rate decreased when vaccination coverage increased for both RCHEs(ρ=−0.131,p<0.001)and RCHDs(ρ=−0.333,p<0.001).Comparing with infected residents who were unvaccinated/vaccinated with one-dose,receiving at least two-dose was estimated to be effective in reducing severe outcomes in 31%and 36%of infected RCHE and RCHD residents respectively;with greater reduction in mortality among RCHD than RCHE residents(54%and 38%,respectively).Vaccine effectiveness of two-dose of BNT162b2 against severe outcomes and death are higher than that of CoronaVac.Conclusions:Increasing COVID-19 vaccination could have significant impact on reducing the risk of COVID-19 outbreaks in RCHs.At least two-dose of COVID-19 vaccine is still effective in reducing severe outcomes and death among infected residents in RCHs during Omicron epidemic.Mei Hung Joanna Leung Kin Hang Kung Ian Siu-kiu Yau Yan Yee Fung Kirran Nazesh Mohammad Cheong Chi Andrew Lau Ka Wing Albert Au Shuk Kwan Chuang 2023Infectious Medicine2023,2,1:0
5Characteristics of patients with erectile dysfunction in a family physicianled erectile dysfunction clinic:Retrospective case series显示文摘Objectives:1.To examine the characteristics of patients with erectile dysfunction in a family physician led erectile dysfunction clinic;2.To review association of chronic disease spectrum and erectile dysfunction;3.To review initial treatment pattern and outcome.Design:Retrospective case series review.Subjects:All consecutive patients seen in a regional hospital family physician led erectile dysfunction clinic from April 2014 to March 2015.Main outcome measures:1.The severity of erectile dysfunction,based on International Index of Erectile Function(IIEF-5).2.The associated chronic comorbidities of patients.3.Treatment patterns and patient outcomes.Results:One hundred and eighty three patients presented with erectile dysfunction(ED)with mean age 58.7(range 23 to 82)years old were seen during the study period.One hundred and twenty seven patients(69.4%)had comorbidity of chronic diseases,including 50.8%had hypertension,38.8%had diabetes mellitus and 33.9%had hyperlipidaemia.Their mean body mass index was 25.2 kg/m2,the mean blood pressure was 137.3/79.5 mm Hg(1 mm Hg=0.133 kPa).According to IIEF-5 score,50.3%,30.6%and 18.6%had severe,moderate and mild erectile dysfunction respectively.The average duration of ED before seeking medical help was 3.9 years.Phosphodiesterase 5(PDE5)inhibitors were prescribed to 119 patients(65%),and 57.1%of them achieved good response.Twenty nine patients(15.8%)were referred to other specialty for further management,including 27.6%had contraindication for PDE5 inhibitor.Conclusion:High proportion of erectile dysfunction patients had comorbitiy of chronic dis-eases.57.1%of those patients receiving PDE5 inhibitors showed good response.Lap Kin Chiang Cheuk-Wai Kam Kin-Chung Michael Yau Lorna Ng 2017Family Medicine and Community Health2017,5,1:0
