| 1 | 基于IMRT时代的第八版AJCC/UICC鼻咽癌临床分期建议显示文摘目的准确的分期系统对癌症的治疗至关重要。随着癌症分期和治疗方法的演变,需要不断评价分期的适用性和改进性。方法基于第7版AJCC/UICC分期回顾性分析香港和中国大陆2个肿瘤中心收治的1609例接受调强放射治疗的首诊无转移鼻咽癌患者临床资料,所有患者治疗前均行核磁共振分期评估。结果无其他T3、T4期解剖结构受侵患者中,伴有咀嚼肌间隙(翼内肌和/或翼外肌)侵犯、椎前肌侵犯及咽旁间隙侵犯的三组患者之间0s相近。伴广泛软组织(上述侵犯结构以外的软组织)受侵患者OS与伴有颅内侵犯或颅神经侵犯相似。仅2%患者锁骨上窝以上淋巴结转移者直径〉6cm,其0s率与下颈淋巴结转移者类似。用下颈(环状软骨尾侧缘水平以下)代替锁骨上窝并不影响N分期之间的风险差异性。采用推荐的T、N分期,T。N。、T。N,期0s相近。结论经AJCC/UICC分期筹备委员会审阅后,建议第8版分期应将翼内肌/翼外肌从T。降到T:期,增加椎前肌为L期,用下颈取代锁骨上窝,将淋巴结最大直径〉6cm合并归为N,期,将T。、N,期统一归为Ⅳ。期。这些改变不仅使得相邻分期间风险差异性更好,而且使得临床实践性与全球适用性之间达到最佳平衡。 | 潘建基 Wai Tong Ng 宗井凤 Lucy L.K.Chan Brian O' Sullivan 林少俊 Henry C.K.Sze 陈韵彬 Horace C.W.Choi 郭巧娟 Wai Kuen Kan 肖友平 Xu Wei Quynh Thu Le Christine M.Glastonbury A.Dimtrios Colevas Randal S.Weber Jatin P.Shah Anne W.M.Lee | 2016 | 中华放射肿瘤学杂志2016，25，3： | 65 |
| 2 | Effect of alcohol consumption on liver stiffness measured by transient elastography显示文摘AIM:To determine the evolution of transient elastography(TE) in patients with alcoholic liver disease according to alcohol cessation or continuation.METHODS:We retrospectively selected in our local database all patients who had two TE between June 2005 and November 2010 with chronic alcohol excessive consumption and excluded those with associated cause of liver disease.TE was performed at least one week apart by senior operator.TE examinations with less than ten successful measures or with an interquartile range above 30% were excluded.We retrospectively reviewed file of all patients to include only patient followed up by trained addictologist and for which definite information on alcohol consumption was available.Concomitant biological parameters [aspartate amino transferase(AST),alanine amino transferase and gamma-glutamyl transpeptidase(GGT)] within 4 wk of initial and final TE were recorded.Putative fibrosis score according to initial and final TE were determined with available cut-off for alcoholic liver disease and hepatitis C.Initial and final putative fibrosis score were compared according to alcohol consumption during follow-up.RESULTS:During the study period 572 patients had TE examination for alcoholic liver disease and 79 of them had at least two examinations.Thirty-seven patients met our criteria with a median follow-up of 32.5 wk.At the end of the study,13(35%) were abstinent,and 24(65%) relapsers.Eight patients had liver biopsy during follow-up.TE decreased significantly during follow-up in 85% of abstinent patients [median(range):-4.9(-6.1,-1.9)],leading to a modification of the putative fibrosis stage in 28%-71% of patient according to different cut-off value.In relapsers TE increased in 45% and decreased in 54% of patient.There was no statistical difference between initial and final TE in relapsers.In the overall population,using 22.6 kPa as cut-off for cirrhosis,4 patients had cirrhosis at initial TE and 3 patients had cirrhosis at final TE.Using 19.5 kPa as cut-off for cirrhosis,7 patients had cirrhosis at initial TE and 5 patients had cirrhosis at final TE.Using 12.5 kPa as cut-off for cirrhosis,16 patients had cirrhosis at initial TE and 15 patients had cirrhosis at final TE.Evolution of biological data was in accordance with the relapse or abstinent status:abstinence ratio(duration of abstinence/duration follow-up) was correlated with AST ratio(r =-0.465,P = 0.007) and GGT ratio(r =-0.662,P<0.0001).GGT was correlated with initial(r = 0.488,P = 0.002) and final TE(r = 0.49,P<0.005).Final TE was correlated with AST(r = 0.362,P<0.05).Correlation between TE ratio and AST ratio(r = 0.44,P = 0.01) revealed that TE varied proportionally to AST for all patients irrespective of their alcohol status.The same relationship was observed between TE ratio and GGT ratio(r = 0.65,P<0.0001).Evolution of TE was significantly correlated with the ratio of time of abstinence to observation time(r =-0.387,P = 0.016) and the evolution of liver enzymes.CONCLUSION:TE significantly decreased with abstinence.Results of TE in alcoholic liver disease cannot be interpreted without taking into account alcohol consumption and liver enzymes. | Edouard Bardou-Jacquet Ludivine Legros Draman Soro Marianne Latournerie Anne Guillygomarc'h Caroline Le Lan Pierre Brissot Dominique Guyader Romain Moirand | 2013 | World Journal of Gastroenterology2013，19，4： | 20 |

| 4 | Evolution of germination strategy in the invasive species Ulex europaeus显示文摘Aims The study of the adaptive potential of the germination patterns of invading species enables us to identify some traits linked with their capacity to colonize new sites,and to gain a better understanding of their area of distribution.The aim of this study is to determine the germination pattern of Ulex europaeus,a cosmopolitan invasive species,in relation to temperature and to explore its potential evolu-tion and invasiveness in a tropical region,following its introduction from Europe.Methods We studied the germination pattern of U.europaeus(the com-mon gorse)to test both physical dormancy and germination capacity within the range of temperatures found in the native and invasive regions.To understand its germination pattern and its evolution,the rate and the speed of germination,as well as the percentage of seeds that became mouldy during the experi-ment,have been compared between a native habitat,France and a habitat into which it has been introduced,the tropical island of La Reunion.Important Findings The results show that gorse seeds germinate in large quantities,pos-sess the ability to germinate under a wide range of temperatures and they confirm the physical dormancy of the seeds(caused by seed coat impermeability).The decrease in germination from 25℃ upwards,coupled with an increase in the rate of moulding help to explain its restricted distribution at altitude in tropical environments.For scari-fied seeds,we have not detected any difference between the two regions,neither in the percentage of germinated seeds,nor in the per-centage of mouldy seeds.However,seeds from Reunion germinate faster at 20℃ than seeds from France and a greater number of seeds from Reunion are able to germinate without scarification(10-60%for Reunion versus 0-10%for France).These results suggest that while preserving the advantages of the native habitat,in Reunion gorse develops a strategy which favours the rapid occupation of new sites. | Nathalie Udo Michèle Tarayre Anne Atlan | 2017 | Journal of Plant Ecology2017，10，2： | 4 |
| 5 | Relationship between the severity of hepatitis C virus-related liver disease and the presence of Helicobacter species in the liver: A prospective study显示文摘AIM: To determine the presence of Helicobacter species DNA in the liver of chronic hepatitis C (CHC) patients with and without cirrhosis as compared to controls, and to identify the bacterial species involved. METHODS: Seventy-nine consecutive patients (HBV and HIV negative) with a liver sample obtained after liver biopsy or hepatic resection were studied: 41 with CHC without cirrhosis, 12 with CHC and cirrhosis, and 26 controls (HCV negative). Polymerase chain reactions (PCRs) targeting Helicobacter 16S rDNA and species- specific were performed on DNA extracted from the liver. A gastric infection with H pylori was determined by serology and confirmed by 13C-urea breath test. RESULTS: Overall, Helicobacter 16S rDNA was found in 16 patients (20.2%). Although positive cases tended to be higher in CHC patients with cirrhosis (41.6%) than in those without cirrhosis (17.0%) or in controls (15.4%), the difference was not statistically significant (P =0.08). H pylori-like DNA was identified in 12 cases and H. pullorum DNA in 2, while 2 cases remained unidenti- fied. Gastric infection with H pylori was found in only 2 of these patients. CONCLUSION: Our results do not confirm the associ- ation of Helicobacter species DNA in the liver of CHC patients with advanced liver disease.
