1Prognostic value of increased carbohydrate antigen in patients with heart failure显示文摘AIM:To study the prognostic value of carbohydrateantigen 125(CA125) and whether it adds prognostic information to N-terminal pro-brain natriuretic peptide(NT-proBNP) in stable heart failure(HF) patients.METHODS:The predictive value of CA125 was retrospectively assessed in 156 patients with stable HF remitted to the outpatient HF unit for monitoring from 2009 to 2011.Patients were included in the study if they had a previous documented episode of HF and received HF treatment.CA125 and NT-proBNP concentrations were measured.The independent association between NT-proBNP or CA125 and mortality was assessed with Cox regression analysis,and their combined predictive ability was tested by the integrated discrimination improvement(IDI) index.RESULTS:The mean age of the 156 patients was 72 ± 12 years.During follow-up(17 ± 8 mo),27 patients died,1 received an urgent heart transplantation and 106 required hospitalization for HF.Higher CA125 values were correlated with outcomes:58 ± 85 KU/L if hospitalized vs 34 ± 61 KU/L if not(P < 0.05),and 94 ± 121 KU/L in those who died or needed urgent heart transplantation vs 45 ± 78 KU/L in survivors(P < 0.01).After adjusting for propensity scores,the highest risk was observed when both biomarkers were elevated vs not elevated(HR = 8.95,95%CI:3.11-25.73; P < 0.001) and intermediate when only NT-proBNP was elevated vs not elevated(HR = 4.15,95%CI:1.41-12.24; P < 0.01).Moreover,when CA125 was added to the clinical model with NT-proBNP,a 4%(P < 0.05) improvement in the IDI was found.CONCLUSION:CA125 > 60 KU/L identified patients in stable HF with poor survival.Circulating CA125 level adds prognostic value to NT-proBNP level in predicting HF outcomes.Ana B Méndez Jordi Ordonez-Llanos Andreu Ferrero Mariana Noguero Teresa Mir Josefina Mora Antoni Bayes-Genis Sònia Mirabet Juan Cinca Eulàlia Roig 2014World Journal of Cardiology2014,6,4:13
2Renin angiotensin system in liver diseases: Friend or foe?显示文摘In the last three decades,the understanding of the renin angiotensin system(RAS)has been changed by the discoveries of functional local systems,novel biologically active peptides,additional specific receptors,alternative pathways of angiotensin(Ang)?Ⅱ?generation,and new roles for enzymes and precursor components other than those in Ang?Ⅱ?synthesis.In this regard,the discovery that Ang-(1-7)opposes the pressor,proliferative,pro-fibrotic,and pro-inflammatory effects mediated by Ang?Ⅱ?has contributed to the realization that the RAS is composed of two axes.The first axis consists of the angiotensin-converting enzyme(ACE),with Ang?Ⅱ?as the end product,and the angiotensin type 1(AT1)receptor as the main effector mediating the biological actions of Ang?Ⅱ.The second axis results from ACE2-mediated hydrolysis of Ang?Ⅱ,leading to the production of Ang-(1-7),with the Mas receptor as the main effector conveying the vasodilatory,antiproliferative,anti-fibrotic,and anti-inflammatory effects of Ang-(1-7).Experimental and clinical studies have shown that both axes of the RAS may take part in the pathogenesis of liver diseases.In this manuscript,we summarize the current evidence regarding the role of RAS in hepatic cirrhosis and its complications,including hemodynamic changes and hepatorenal syndrome.The therapeutic potential of the modulation of RAS molecules in liver diseases is also discussed.Ana Cristina Simoes e Silva Aline S Miranda Natália P Rocha Antonio L Teixeira 2017World Journal of Gastroenterology2017,23,19:12
3Hepatitis B virus prevalence and transmission risk factors in inflammatory bowel disease patients at Clementino Fraga Filho university hospital显示文摘AIM: To evaluate the prevalence of hepatitis B virus (HBV) infection in inflammatory bowel disease (IBD) patients that followed up in our hospital and try to identify the possible risk factors involved in this infection transmission. METHODS: This was a cross-sectional study for which 176 patients were selected according to their arrival for the medical interview. All these patients had already IBD diagnosis. The patient was interviewed and a questionnaire was filled out. RESULTS: In the group of 176 patients whom we examined, we found that 17% (30) were anti-HBc positive. Out of 30 patients with positive anti-HBc, 2.3% (4) had positive HBsAg and negative HBV-DNA. In an attempt to identify the possible HBV infection transmission risk factors in IBD patients, it was observed that 117 patients had been submitted to some kind of surgical procedure, but only 24 patients had positive anti-HBc (P = 0.085). It was also observed that surgery to treat IBD complications was not a risk factor for HBV infection transmission, since we did not get a statically significant P value. However, IBDpatients that have been submitted to surgery to treat IBD complications received more blood transfusions then patients submitted to other surgical interventions (P = 0.015). 4Hepatic abscess induced by foreign body:Case report and literature review显示文摘Hepatic abscess due to perforation of the gastrointestinal tract caused by ingested foreign bodies is uncommon. Pre-operative diagnosis is difficult as patients are often unaware of the foreign body ingestion and symptoms and imagiology are usually non-specific. The authors report a case of 62-year-old woman who was admitted with fever and abdominal pain. Further investigation revealed hepatic abscess, without resolution despite antibiotic therapy. A liver abscess resulting from perforation and intra-hepatic migration of a bone coming from the pilorum was diagnosed by surgery. 5Proteoglycans in liver cancer显示文摘Proteoglycans are a group of molecules that contain at least one glycosaminoglycan chain,such as a heparan,dermatan,chondroitin,or keratan sulfate,covalently attached to the protein core.These molecules arecategorized based on their structure,localization,and function,and can be found in the extracellular matrix,on the cell surface,and in the cytoplasm.Cell-surface heparan sulfate proteoglycans,such as syndecans,are the primary type present in healthy liver tissue.However,deterioration of the liver results in overproduction of other proteoglycan types.The purpose of this article is to provide a current summary of the most relevant data implicating proteoglycans in the development and progression of human and experimental liver cancer.A review of our work and other studies in the literature indicate that deterioration of liver function is accompanied by an increase in the amount of chondroitin sulfate proteoglycans.The alteration of proteoglycan composition interferes with the physiologic function of the liver on several levels.This article details and discusses the roles of syndecan-1,glypicans,agrin,perlecan,collagen XVIII/endostatin,endocan,serglycin,decorin,biglycan,asporin,fibromodulin,lumican,and versican in liver function.Specifically,glypicans,agrin,and versican play significant roles in the development of liver cancer.Conversely,the presence of decorin could potentially provide protective effects.Kornélia Baghy Péter Tátrai Eszter Regős Ilona Kovalszky 2016World Journal of Gastroenterology2016,22,1:8

7MGMT and MLH1 methylation in Helicobacter pylori-infected children and adults显示文摘AIM:To evaluate the association between Helicobacter pylori(H.pylori) infection and MLH1 and MGMT methylation and its relationship with microsatellite instability(MSI).METHODS:The methylation status of the MLH1 and MGMT promoter region was analysed by methylation specific methylation-polymerase chain reaction(MSPPCR) in gastric biopsy samples from uninfected or H.pylori-infected children(n = 50),from adults with chronic gastritis(n = 97) and from adults with gastric cancer(n = 92).MLH1 and MGMT mRNA expression were measured by real-time PCR and normalised to a constitutive gene(β actin).MSI analysis was performed by screening MSI markers at 4 loci(Bat-25,Bat-26,D17S250 and D2S123) with PCR;PCR products were analysed by single strand conformation polymorphism followed by silver staining.Statistical analyses were performed with either the χ 2 test with Yates continuity correction or Fisher's exact test,and statistical significance for expression analysis was assessed using an unpaired Student's t-test.RESULTS:Methylation was not detected in the promoter regions of MLH1 and MGMT in gastric biopsy samples from children,regardless of H.pylori infection status.The MGMT promoter was methylated in 51% of chronic gastritis adult patients and was associated with H.pylori infection(P < 0.05);this region was methylated in 66% of gastric cancer patients,and the difference in the percentage of methylated samples between these patients and those from H.pylori-infected chronic gastritis patients was statistically significant(P < 0.05).MLH1 methylation frequencies among H.pylori-infected and non-infected chronic gastritis adult patients were 13% and 7%,respectively.We observed methylation of the MLH1 promoter(39%) and increased MSI levels(68%) in samples from gastric cancer patients in comparison to samples from H.pylori-infected adult chronic gastritis patients(P < 0.001 and P < 0.01,respectively).The frequency of promoter methylation for both genes was higher in gastric cancer samples than in H.pylori-positive chronic gastritis samples(P < 0.05).The levels of MLH1 and MGMT mRNA were significantly reduced in chronic gastritis samples that were also hypermethylated(P < 0.01).CONCLUSION:In summary,MGMT and MLH1 methylation did not occur in earlier-stage H.pylori infections and thus might depend on the duration of infection.Marisa C Alvarez Juliana C Santos Nathália Maniezzo Marcelo S Ladeira Artur LC da Silva Isabel CA Scaletsky José Pedrazzoli Jr Marcelo L Ribeiro 2013World Journal of Gastroenterology2013,19,20:5
8Sex-specific effects of Eugenia punicifolia extract on gastric ulcer healing in rats显示文摘AIM To evaluate the sex-specific effects of a hydroalcoholic extract from Eugenia punicifolia(HEEP) leaves on gastric ulcer healing.METHODS In this rat study involving males, intact(cycling) females, and ovariectomized females, gastric ulcers were induced using acetic acid. A vehicle, lansoprazole, or HEEP was administered for 14 d after ulcer induction. Body weight was monitored throughout the treatment period. At the end of treatment, the rats were euthanized and the following in vivo and in vitro investigations were performed: macroscopic examination of the lesion area and organ weights, biochemical analysis, zymography, and evaluation of protein expression levels. Additionally, the concentration-dependent effect of HEEP was evaluated in terms of subacute toxicity and cytotoxicity.RESULTS Compared to the vehicle, HEEP demonstrated a great healing capacity by substantially reducing the ulcerative lesion area in males(52.44%), intact females(85.22%), and ovariectomized females(65.47%), confirming that HEEP accelerates the healing of acetic acidinduced gastric lesions and suggesting that this effect is modulated by female sex hormones. The antiulcer effect of HEEP was mediated by prostaglandin E2 only in male rats. Overall, the beneficial effect of HEEP was the highest in intact females. Notably, HEEP promoted the expression of vascular endothelial growth factor(intact vs ovariectomized females) and decreased the expression of Caspase-8 and Bcl-2(intact female vs male or ovariectomized female). Additionally, HEEP enhanced fibroblast proliferation and migration into a wounded area in vitro, confirming its healing effect. 9Elevated miR-33a and miR-224 in steatotic chronic hepatitis Cliver biopsies显示文摘AIM:To assess the expression of selected microRNAs(miRNA) in hepatitis C,steatotic hepatitis C,noninfected steatotic and normal liver tissues.METHODS:The relative expression levels of miR-21,miR-33 a,miR-96,miR-122,miR-125 b,miR-221 and miR-224 were determined in 76 RNA samples isolated from 18 non-steatotic and 28 steatotic chronic hepatitis C(CHC and CHC-Steatosis,respectively) cases,18 non-infected,steatotic liver biopsies of metabolic origin(Steatosis) and 12 normal formalin-fixed paraffin-embedded liver tissues using TaqMan MicroRNA Assays.All CHC biopsy samples were obtained prior to initiating therapy.Patients' serum biochemical values,which included glucose,triglyceride,cholesterol,alanine aminotransferase(ALT),aspartate aminotransferase(AST),gamma-glutamyl-transferase(GGT),alkaline phosphatase(AP),were obtained and correlated with relative miRNA expression.RESULTS:When compared with control non-infected liver samples,miR-122 and miR-221 levels were reduced in CHC-Steatosis(P < 0.03) and in CHC,CHCSteatosis and Steatosis(P < 0.01).Alternatively,the expression of miR-33 a and miR-224 were elevated in CHC-Steatosis and Steatosis in comparison to control tissue(P < 0.01).The levels of miR-33 a and miR-224 in CHC-Steatosis(P < 0.02) and miR-224 in Steatosis(P < 0.001) were increased in comparison to CHC samples.By contrast,the expression of miR-21 did not differ statistically between diseased and normal liver samples.Levels of miR-33 a correlated negatively with serum AST and AP levels in Steatosis as well as with necroinflammatory grade in CHC,whereas miR-21 correlated positively with AST in Steatosis and displayed negative correlation with triglyceride level in CHC-Steatosis.In contrast,miRNA levels were not correlated with ALT,GGT,cholesterol levels or fibrosis stage.CONCLUSION:Differences in miRNA expression were observed between CHC and steatotic CHC,CHC and steatotic liver,but not between steatotic CHC and steatotic liver of metabolic origin.Gabor Lendvai Katalin Jármay Gizella Karácsony Tünde Halász Ilona Kovalszky Kornélia Baghy Tibor Wittmann Zsuzsa Schaff András Kiss 2014World Journal of Gastroenterology2014,20,41:2

11Diabetes mellitus, revascularization and outcomes in elderly patients with myocardial infarction-related cardiogenic shock显示文摘Background The prognostic role of diabetes mellitus(DM)in elderly patients with myocardial infarction-related cardiogenic shock(MI-CS)remains controversial.Little information exists about the impact of intensive cardiac care unit(ICCU)and revascularization on outcomes of elderly patients with MI-CS.We aimed to assess the prognostic impact of DM according to age in patients with MI-CS,and to analyze the impact ICCU management and revascularization on in-hospital mortality in MI-CS patients at older ages.Methods Discharge episodes with diagnosis of CS associated with MI were selected from the Spanish National Health System’s Basic Data Set.Centers were classified according to their availability of ICCU.Main outcome measured was in-hospital mortality.Results A total of 23,590 episodes of MI-CS were identified,of whom 12,447(52.8%)were in patients aged≥75 years.The impact of DM on in-hospital mortality was different among age subgroups.While in younger patients,DM was associated to a higher mortality risk(0.52 vs.0.47,OR=1.12,95%CI:1.06–1.18,χ^2<0.001),this association became non-significant in older patients(0.76 vs.0.81,χ^2=0.09).Adjusted mortality rate of MI-CS aged≥75 years was lower in patients admitted to hospitals with ICCU(adjusted mortality rate:74.2%vs.77.7%,P<0.001)and in patients undergoing revascularization(74.9%vs.77.3%,P<0.001).Conclusions Prognostic impact of DM in patients with MI-CS was different according to age,with a significantly lower impact at older ages.The availability of ICCU and revascularization were associated with better outcomes in these complex patients.Miquel Gual Albert Ariza-Solé María García Márquez Cristina Fernández JoséL Bernal Francesc Formiga María-Isabel Barrionuevo JoséC Sánchez-Salado Victòria Lorente Júlia Pascual Isaac Llaó Oriol Alegre Angel Cequier Javier Elola 2020Journal of Geriatric Cardiology2020,17,10:2

20Ophiolite hosted chromitite formed by supra-subduction zone peridotite–plume interaction显示文摘Chromitite bodies hosted in peridotites typical of suboceanic mantle(s.l.ophiolitic)are found in the northern and central part of the Loma Caribe peridotite,in the Cordillera Central of the Dominican Republic.These chromitites are massive pods of small size(less than a few meters across)and veins that intrude both dunite and harzburgite.Compositionally,they are high-Cr chromitites[Cr#=Cr/(Cr+Al)atomic ratio=0.71-0.83]singularly enriched in TiO2(up to 1.25 wt.%),Fe2 O3(2.77-9.16 wt.%)as well as some trace elements(Ga,V,Co,Mn,and Zn)and PGE(up to 4548 ppb in whole-rock).This geochemical signature is unknown for chromitites hosted in oceanic upper mantle but akin to those chromites crystallized from mantle plume derived melts.Noteworthy,the melt estimated to be in equilibrium with such chromite from the Loma Caribe chromitites is similar to basalts derived from different source regions of a heterogeneous Caribbean mantle plume.This mantle plume is responsible for the formation of the Caribbean Large Igneous Province(CLIP).Dolerite dykes with back-arc basin basalt(BABB)and enriched mid-ocean ridge basalt(E-MORB)affinities commonly intrude the Loma Caribe peridotite,and are interpreted as evidence of the impact that the Caribbean plume had in the off-axis magmatism of the back-arc basin,developed after the Caribbean island-arc extension in the Late Cretaceous.We propose a model in which chromitites were formed in the shallow portion of the back-arc mantle as a result of the metasomatic reaction between the supra-subduction zone(SSZ)peridotites and upwelling plume-related melts.Júlia Farré-de-Pablo Joaquín A.Proenza JoséMaría González-Jiménez Thomas Aiglsperger Antonio Garcia-Casco Javier Escuder-Viruete Vanessa Colás Francisco Longo 2020Geoscience Frontiers2020,11,6:1
