1决定新生儿脓毒性休克转归和预后的因素显示文摘背景有关新生儿脓毒性休克转归和预后的资料相对缺乏。目的描述新生儿脓毒性休克的转归,研究与死亡或负面转归相关的因素。设计回顾性队列研究。场所一所大学医院的三级NICU。对象6年期间所有符合下述标准的NICU患儿:证实或高度怀疑感染;伴低血压和(或)需液体复苏或血管活性药物治疗。干预无。测量指标和主要结果主要转归指标为28d病死率,负面转归定义为纠正年龄18个月时确定死亡或存在严重后遗症(脑瘫、严重发育迟缓、听力损害、致盲或短肠综合征)。共有48例患儿纳入研究,其中46例在纠正年龄18个月时获得随访数据。结果发现,28d病死率为40%(19例死亡);24例在纠正年龄18个月时观察到负面转归(病死率为52%;死亡19例,严重后遗症5例)。总计28%的患儿存活且18个月时检查正常。有负面转归的患儿孕周、出生体重、Apgar评分、发生脓毒症时的体质量、pH值均偏低。差异具有显著性。更常见的负面因素包括革兰阴性菌感染、胎儿发育迟缓、低血糖和血小板减少。经多因素分析,对28d病死率和纠正年龄18个月时负面转归较有意义的预测因素是脓毒症发生时体重(OR值分别为0.14、0.21;95%a分别为0.03~0.55、0.06~0.74),以及革兰阴性菌感染(OR值分别为10.1、45.5;95%CI分别为1.5~65.7.3~637)。结论新生儿脓毒性休克的转归很差。研究结果强调极低出生体重儿对脓毒性休克,尤其革兰阴性菌感染所致者抵抗力极差。Kermorvant-Duchemin E Laborie S Rabilloud M 2008中国小儿急救医学2008,15,2:2
