| 8 | Liver regeneration as treatment target for severe alcoholic hepatitis显示文摘Severe alcoholic hepatitis(AH)is a distinct entity in the spectrum of alcoholrelated liver disease,with limited treatment options and high mortality.Supportive medical care with corticosteroids in selected patients is the only currently available treatment option,often with poor outcomes.Based on the insights into the pathogenetic mechanisms of AH,which are mostly obtained from animal studies,several new treatment options are being explored.Studies have implicated impaired and deranged liver regeneration processes as one of the culprit mechanisms and a potential therapeutic target.Acknowledging evidence for the beneficial effects of granulocyte colony-stimulating factor(G-CSF)on liver regeneration and immunomodulation in animal models,several human studies investigated its role in the treatment of advanced alcohol-related liver disease and AH.Contrary to the previously published studies suggesting benefits of G-CSF in the outcomes of patients with severe AH,these effects were not confirmed by a recently published multicenter randomized trial,suggesting that other options should rather be pursued.Stem cell transplantation represents another option for improving liver regeneration,but evidence for its efficacy in patients with severe AH and advanced alcohol-related liver disease is still very scarce and unconvincing,with established lack of efficacy in patients with compensated cirrhosis.In this review,we summarize the current knowledge on the pathogenesis and experimental therapies targeting liver regeneration.The lack of high-quality studies and evidence is a major obstacle in further treatment development.New insights into the pathogenesis of not only liver injury,but also liver regeneration processes are mandatory for the development of new treatment options.A reliable experimental model of the pathogenesis of AH and processes involved in liver recovery is still missing,and data obtained from animal studies are essential for future research. | Lucija Virovic-Jukic Dominik Ljubas Sanja Stojsavljevic-Shapeski Neven Ljubičić Tajana Filipec Kanizaj Ivana Mikolasevic Ivica Grgurevic | 2022 | World Journal of Gastroenterology2022,28,32: | 0 |
| 9 | Epigastric hernias in children and the use of ultrasound in its diagnosis显示文摘Introduction There is a paucity of clinical data on pediatric epigastric hernias despite them accounting for up to 6%of all hernia repairs in children.We aimed to provide additional data to supplement those 117 cases of a recent systematic review and to further clarify the role of ultrasound in diagnosing pediatric epigastric hernia.Methods We retrospectively included all 60 patients treated for epigastric hernias in children in two tertiary pediatric surgical departments within 12 years.Associations were tested via point-biserial correlation analyses.Results Epigastric hernias primarily affected preschool children with a median age of 39 months.The vast majority of patients(88%)presented with swelling that was occasionally(30%)accompanied by pain.Fascial defects could be found during clinical examination in 45%of patients with a median size of 5 mm(95%CI 3 to 10).Smaller defects were less likely to be palpable(r=-0.44,95%CI-0.08 to-0.7,p=0.021).Likewise,ultrasound was used more frequently with smaller fascial defect sizes(r=-0.51,95%Cl-0.16 to-0.74,p=0.007).Laparoscopic repair was used in 11 patients(19%)and more often(4/11)in combination with another simultaneous procedure than open repair(11/48).Conclusions Epigastric hernias are primarily a condition of the preschool child.Ultrasound can be beneficial if the diagnosis cannot be made clinically;otherwise,it is abdicable if it does not change the management of the patient's epigastric hernia.Laparoscopic repairs might be beneficial for children with multiple defects or simultaneous procedures. | Tatjana T König Laura S Oerters Ljuba Spiller Martin Schwind Mark Born Christina Oetzmann von Sochaczewski Andreas C Heydweiller | 2023 | World Journal of Pediatric Surgery2023,6,2: | 0 |
| 10 | A network for long-term monitoring of vegetation in the area of Fildes Peninsula, King George Island显示文摘Climate warming has become evident in the maritime Antarctic over the past decades, and has already influenced the growing season and the population size of two native vascular plants in Antarctica, Deschampsia antarctica and Colobanthus quitensis. Both vascular plant species are therefore regarded as good bioindicators of regional warming in west Antarctica. To carry out long-term monitoring of vegetation(mainly using D. antarctica) and build a comprehensive research platform for multi-disciplinary study(including botany, microbiology, ecology, and environmental science) for Chinese scientists, 13 permanent plots were established in January and February of 2013–2015 in the area of Fildes Peninsula(King George Island). Here we present the benchmark data of the first observations from these plots, including site characteristics, and the population and associates of D. antarctica in each plot.
