1A prospective randomized antiplatelet trial of cilostazol versus clopidogrel in patients with bare metal stent显示文摘Background Cilostazol is a newly developed antiplatelet drug that has been widely applied for clinical use. Its antiplatelet action appears to be mainly related to inhibition of intracellular phosphodiesterase activity. Recently, cilostazol has been used for antiplatelet therapy after coronary bare metal stent implantation for thrombosis and restenosis prevention. This prospective randomized and double blind trial was designed to investigate the safety and efficacy of cilostazol for the prevention of late restenosis and acute or subacute stent thrombosis. Methods One hundred and twenty patients who underwent elective stent were randomly assigned to treatment group with cilostazol 200 mg/d (n = 60), clopidogrel 75 mg/d and aspirin 100 mg/d or to control group with clopidogrel treatment 75 mg/d (n = 60) and aspirin 100 mg/d. Follow-up coronary angiography was performed 6-9 months later. Results Nine months major adverse cardio-cerebral event (MACCE) were lower in treatment groups (P<0.05). The quantitative coronary angiography (QCA) at 6 months follow-up showed that minimum lumen diameter (MLD) was higher in treatment group than that of control group [(2.14±0.52)mm vs (1.82±0.36)mm, P<0.05]. Late lumen loss (LL) [(0.82±0.42)mm vs (1.31±0.58)mm; P<0.01], restenosis rate (RR) (14% vs 32%; P<0.05) and target lesion revascularizaion (TLR) rate (5% vs 17%; P<0.05) were lower in treatment group than in control group. 2Therapeutic approaches to non-alcoholic fatty liver disease: past achievements and future challenges显示文摘BACKGROUND: Non-alcoholic fatty liver disease (NAFLD) is a leading cause of chronic liver injury and mortality in Western countries and China. However, as to date, there is no direct and effective therapy for this disease. The aim of this review is to analyze the key progress and challenges of main current therapeutic approaches in NAFLD. DATA SOURCE: We carried out a PubMed search of English-language articles relevant to NAFLD therapy. RESULTS: There are two major therapeutic strategies for NAFLD treatment: (1) lifestyle interventions (including weight reduction, dietary modification and physical exercise) and (2) pharmaceutical therapies. Lifestyle interventions, particularly chronic and moderate intensity exercise, are the most effective and recognized clinical therapies for NAFLD. For pharmaceutical therapies, although their effects and mechanisms have been extensively investigated in laboratory studies, they still need further tests and investigations in clinical human trials. CONCLUSION: Future advancement of NAFLD therapy should focus on the mechanistic studies on cell based and animal models and human clinical trials of exercise, as well as the combination of lifestyle intervention and pharmaceutical therapy specifically targeting main signaling pathways related to lipid metabolism, oxidative stress and inflammation.Jia Xiao Rui Guo Man Lung Fung Emily C Liong George L Tipoe 2013Hepatobiliary & Pancreatic Diseases International2013,12,2:14
3Uniform surface polished method of complex holes in abrasive flow machining显示文摘Abrasive flow machining(AFM) is an effective method that can remove the recasting layer produced by wire electrical discharge machining(WEDM). However, the surface roughness will not be easily uniform when a complex hole is polished by this method. CFD numerical method is aided to design good passageways to find the smooth roughness on the complex hole in AFM. Through the present method, it reveals that the shear forces in the polishing process and the flow properties of the medium in AFM play the roles in controlling the roughness on the entire surface. A power law model was firstly set up by utilizing the effect of shear rates on the medium viscosities, and the coefficients of the power law would be found by solving the algebraic equation from the relations between the shear rates and viscosities. Then the velocities, strain rates and shear forces of the medium acting on the surface would be obtained in the constant pressure by CFD software. Finally, the optimal mold core put into the complex hole could be designed after these simulations. The results show that the shear forces and strain rates change sharply on the entire surface if no mold core is inserted into the complex hole, whereas they hardly make any difference when the core shape is similar to the complex hole. Three experimental types of mold core were used.
