| 2 | Hepatocellular carcinoma: Review of disease and tumor biomarkers显示文摘Hepatocellular carcinoma(HCC) is a common malignancy and now the second commonest global cause of cancer death. HCC tumorigenesis is relatively silent and patients experience late symptomatic presentation. As the option for curative treatments is limited to early stage cancers, diagnosis in non-symptomatic individuals is crucial. International guidelines advise regular surveillance of high-risk populations but the current tools lack sufficient sensitivity for early stage tumors on the background of a cirrhotic nodular liver. A number of novel biomarkers have now been suggested in the literature, which may reinforce the current surveillance methods. In addition, recent metabonomic and proteomic discoveries have established specific metabolite expressions in HCC, according to Warburg's phenomenon of altered energy metabolism. | 3 | Effects of exogenous enzymes, Lactobacillus acidophilus or their combination on feed performance response and carcass characteristics of rabbits fed sugarcane bagasse显示文摘The aim of this study was to evaluate the effect of Lactobacillus acidophilus(LAC), exogenous enzymes of ZAD(exogenous enzyme preparation) or their combination on feed conversion, and dressing of rabbits fed different treatments of sugarcane bagasse(SCB). Five rations were allotted randomly to five groups of New-Zealand White(NZW) rabbits(n=10) with initial live body weight of(838±42.4) g and 5 weeks of age. Rabbits were fed on diets with different sources of fiber as follows,(i) a control diet composed of 100% berseem hay and 0% SCB,(ii) 50% berseem hay and 50% untreated SCB(USCB),(iii) 50% berseem hay and 50% SCB treated with L. acidophilus(LAC),(iv) 50% berseem hay and 50% SCB treated with ZAD(ZAD), and(v) 50% berseem hay and 50% SCB treated with a combination of LAC+ZAD(LZ). Treatments of SCB with L. acidophilus, ZAD and LAC+ZAD had the highest feed conversion ratio than both USCB and control. The dressing percentage of rabbits that fed the LAC and LZ diets was higher(P<0.05) compared with that in the other groups. Performance index(PI) for LAC group was improved(P<0.05) compared to that for the other groups; however, PI for USCB group was the lowest(P<0.05). | 4 | Pediatric non-alcoholic steatohepatitis:The first report on the ultrastructure of hepatocyte mitochondria显示文摘AIM:To investigate the ultrastructure of abnormal hepatocyte mitochondria,including their cellular and hepatic zonal distribution,in bioptates in pediatric nonalcoholic steatohepatitis(NASH).METHODS:Ultrastructural investigations were conducted on biopsy liver specimens obtained from 10children(6 boys and 4 girls)aged 2-14 years with previously clinicopathologically diagnosed NASH.The disease was diagnosed if liver biopsy revealed steatosis,inflammation,ballooned hepatocytes,Mallory hyaline,or focal necrosis,varying degrees of fibrosis in the absence of clinical,serological,or histological findings of infectious liver diseases,autoimmune hepatitis,metabolic liver diseases,or celiac disease.For ultrastructural analysis,fresh small liver blocks(1 mm3 volume)were fixed in a solution containing 2%paraformaldehyde and 2.5%glutaraldehyde in 0.1 mol/L cacodylate buffer.The specimens were postfixed in osmium tetroxide,subsequently dehydrated through a graded series of ethanols and propylene oxide,and embedded in Epon812.The material was sectioned on a Reichert ultramicrotome to obtain semithin sections,which were stained with methylene blue in sodium borate.Ultrathin sections were contrasted with uranyl acetate and lead citrate,and examined using an Opton EM 900 transmission electron microscope.RESULTS:Ultrastructural analysis of bioptates obtained from children with non-alcoholic steatohepatitis revealed characteristic repetitive mitochondrial abnormalities within hepatocytes;mainly mitochondrial polymorphisms such as megamitochondria,loss of mitochondrial cristae,and the presence of linear crystalline inclusions within the mitochondrial matrix of an increased electron density.The crystalline inclusions were particularly evident within megamitochondria(MMC),which seemed to be distributed randomly both within the hepatic parenchymal cell and the zones of hepatic lobule,without special variations in abundance.The inclusions appeared as bundles viewed longitudinally,or as an evenly spaced matrix in cross section,and frequently caused mitochondrial deformation.The average diameter of these linear structures was 10 nm and the average space between them 20 nm.Sometimes enlarged intramitochondrial granules were seen in their vicinity.Foamy cytoplasm of hepatocytes was found,resulting from the proliferation of smooth endoplasmic reticulum and glycogen accumulation.The perivascular space of Disse was frequently dilated,and contained transitional hepatic stellate cells,as well as mature and/or newly forming collagen fiber bundles.CONCLUSION:Marked ultrastructural abnormalities observed in hepatocyte mitochondria,especially their polymorphism in the form of MMC and loss of mitochondrial cristae,accompanied by foamy cytoplasm,clearly indicate a major role of these organelles in the morphogenesis of pediatric NASH.Our findings seem to prove the high effectiveness of electron microscopy in the diagnosis of the disease. | Joanna M Lotowska Maria E Sobaniec-Lotowska Sylwia B Bockowska Dariusz M Lebensztejn | 2014 | World Journal of Gastroenterology2014，20，15： | 3 |
| 5 | Relationship between Crohn’s disease, infection with Mycobacterium avium subspecies paratuberculosis and SLC11A1 gene polymorphisms in Sardinian patients显示文摘AIM: To study the association between Crohn’s disease (CD), Mycobacterium avium subspecies paratubercu- losis (MAP), and genetic factors by examining the role of natural resistance-associated macrophage protein 1 (NRAMP1) gene polymorphisms (now SLC11A1) in Sar dinian patients with CD and controls. METHODS: Thirty-seven CD patients and 34 controls with no inflammatory bowel disease (IBD) were recruited at the University of Sassari after giving written con sent. Six SCL11A1 polymorphisms previously reported to be the most significantly associated with IBD were searched. M. paratuberculosis was identified by IS900 PCR and sequencing. Logistic regression was used to cal culate odds ratios (OR) for the associations among CD presence of MAP, and 6 loci described above. RESULTS: For the first time, a strong association was observed between polymorphisms at NRAMP1 locus 823C/T and CD. While CD was strongly associated withboth NRAMP1 and MAP, NRAMP1 polymorphisms and MAP themselves were not correlated. | 7 | Non-pharmacological management of pediatric functional abdominal pain disorders:Current evidence and future perspectives显示文摘Functional abdominal pain disorders(FAPDs) are an important and prevalent cause of functional gastrointestinal disorders among children, encompassing the diagnoses of functional dyspepsia, irritable bowel syndrome, abdominal migraine, and the one not previously present in Rome Ⅲ, functional abdominal pain not otherwise specified. In the absence of sufficiently effective and safe pharmacological treatments for this public problem, non-pharmacological therapies emerge as a viable means of treating these patients, avoiding not only possible side effects, but also unnecessary prescription, since many of the pharmacological treatments prescribed do not have good efficacy when compared to placebo. Thus, the present study provides a review of current and relevant evidence on non-pharmacological management of FAPDs, covering the most commonly indicated treatments, from cognitive behavioral therapy to meditation, acupuncture, yoga, massage, spinal manipulation, moxibustion, and physical activities. In addition, this article also analyzes the quality of publications in the area, assessing whether it is possible to state if non-pharmacological therapies are viable, safe, and sufficiently well-based for an appropriate and effective prescription of these treatments. Finally, it is possible to observe an increase not only in the number of publications on the non-pharmacological treatments for FAPDs in recent years, but also an increase in the quality of these publications.
