| 1 | Bio-inspired carbon electro-catalysts for the oxygen reduction reaction显示文摘We report the synthesis, characterisation and catalytic performance of two nature-inspired biomassderived electro-catalysts for the oxygen reduction reaction in fuel cells. The catalysts were prepared via pyrolysis of a real food waste(lobster shells) or by mimicking the composition of lobster shells using chitin and CaCO_3 particles followed by acid washing. The simplified model of artificial lobster was prepared for better reproducibility. The calcium carbonate in both samples acts as a pore agent, creating increased surface area and pore volume, though considerably higher in artificial lobster samples due to the better homogeneity of the components. Various characterisation techniques revealed the presence of a considerable amount of hydroxyapatite left in the real lobster samples after acid washing and a low content of carbon(23%), nitrogen and sulphur(<1%), limiting the surface area to 23 m^2/g, and consequently resulting in rather poor catalytic activity. | 2 | Giant retroperitoneal abscess following necrotizing pancreatitis treated with internal drainage显示文摘BACKGROUND:Retroperitoneal abscess is a dangerous complication of the inflammatory process in organs. The pancreas reacts by enzymatic autodigestion and inflammation to external and internal factors:alcohol abuse,trauma,biliary stones,and viral infections. Chronic pancreatitis and formation of pseudocysts are late complications.The diffuse spread of pancreatic inflammation may form a retroperitoneal phlegmon.A better outcome is the limitation of the process by capsule formation-retroperitoneal abscess. METHODS:A 62-year-old man,with a history of alcohol abuse,was admitted for intermittent abdominal pain,fever, and significant weight loss.Previous medical consultations (laboratory tests,US,CT)confirmed chronic pancreatitis with pseudocyst formation.A new CT depicted a giant retroperitoneal abscess. RESULTS:After preoperative preparation with antibiotics, laparotomy and internal drainage-longitudinal cystoje- junostomy with Roux-en-Y loop were performed.At the 8th postoperative day the patient was in good condition. CONCLUSIONS:Giant retroperitoneal abscess is a dangerous pathology with significant mortality and morbidity.Diagnosis strongly necessitates operative intervention in order to evacuate and drain the space.Theseconditions include one internal drainage(in the GI tract) with the stomach,duodenum,or jejunum,which does not cause early and late GI discomfort and will gradually liquidate the cavity.We propose internal drainage with Roux-en-Y jejunal loop as the only method for accomplishing these conditions in cases of giant retroperitoneal abscesses. | Ludmil Marinov Veltchev Manol Anastasov Kalniev | 2009 | Hepatobiliary & Pancreatic Diseases International2009,8,5: | 2 |

| 4 | Novel MLH1 frameshift mutation in an extended hereditary nonpolyposis colorectal cancer family显示文摘AIM: To present novel frameshift mutation c.31delC [p.L11X] in the MLH1 gene identified in an extended Bulgarian hereditary non-polyposis colorectal cancer (HNPCC) family and to analyze the molecular and clinical findings within the pedigree concerning the proposal of adequate individual prophylactic strategy for all mutation carriers. METHODS: The pedigree of the family consists of 42 members in four generations. Search for mutations in the MLH1 and hMSH2 genes was performed in the pro-band. After PCR amplification of all exons including flanking intronic regions, amplicons were directly sequenced. RESULTS: The mutation was found in nine from the thirteen pedigree members who signed informed consent to participate in the study. In three adenocarcinomas, microsatellite instability and lack of the MLH1 protein expression were detected. The only one tubulovillous adenoma analyzed was microsatellite stable and the MLH1 protein showed an intact staining. CONCLUSION: The newly described mutation c.31delC is HNPCC causative. Besides the typical clinical features of the syndrome, we found a specific pathologic manifestation such as moderate to high differentiated adenocarcinomas of the colon. One of the mutation carriers developed a benign giant cell soft tissue tumor. The primary tumor localizations were frequently extracolonic and detailed yearly gastrointestinal and gynecological examinations have been proposed to the mutation carriers.
