| 1 | The importance of avoiding unnecessary right ventricular pacing in clinical practice显示文摘Symptomatic bradycardia is effectively treated with the implantation of a cardiac pacemaker. Although a highly successful therapy, during recent years there has been a focus on the negative effects associated with longterm pacing of the apex of the right ventricle(RV). It has been shown in both experimental and clinical studies that RV pacing leads to ventricular dyssynchrony, similar to that of left bundle branch block, with subsequent detrimental effects on cardiac structure and function, and in some cases adverse clinical outcomes such as atrial fibrillation, heart failure and death. There is substantial evidence that patients with reduced left ventricular function(LVEF) are at particular high risk of suffering the detrimental clinical effects of long-term RV pacing. The evidence is, however, incomplete, coming largely from subanalyses of pacemaker and implantable cardiac defibrillator studies. In this group of patients with reduced LVEF and an expected high amount of RV pacing, biventricular pacing(cardiac resynchronization therapy) devices can prevent the negative effects of RV pacing and reduce ventricular dyssynchrony. Therefore, cardiac resynchronization therapy has emerged as an attractive option with promising results and more clinical studies are underway. | 8 | Inverted Meckel’s diverticulum diagnosed using capsule endoscopy: A case report显示文摘BACKGROUND Meckel’s diverticulum is a common asymptomatic congenital gastrointestinal anomaly.However,its presentation as an inverted Meckel's diverticulum is a rare complication,of which few cases have been reported in the literature.CASE SUMMARY Here,we report the case of a 33-year-old man with iron deficiency anemia without manifestation of gastrointestinal bleeding.An upper gastrointestinal endoscopy and total colonoscopy were performed,but no abnormalities were found within the observed area.Finally,a capsule endoscopy was performed and offered us a clue to subsequently confirm the diagnosis of inverted Meckel's diverticulum via computed tomography scan.Laparoscopic intestinal resection surgery was performed.The final pathology report described a Meckel’s diverticulum.CONCLUSION Since inverted Meckel's diverticulum is an uncommon disease and its clinical presentation is not specific,it may go undetected by capsule endoscopy.Successful diagnosis and treatment of this disease requires a high index of clinical suspicion. | Ismael El Hajra Martínez Marta Calvo JoséLuis Martínez-Porras Lucia Gomez-Pimpollo Garcia Jose L Rodriguez Carmen Leon JoséLuis Calleja Panero | 2021 | World Journal of Gastroenterology2021,27,36: | 0 |
| 9 | Mid-infrared Ⅲ-Ⅴ semiconductor lasers epitaxially grown on Si substrates显示文摘There is currently much activity toward the integration of mid-infrared semiconductor lasers on Si substrates for developing a variety of smart,compact,sensors based on Si-photonics integrated circuits.We review this rapidly-evolving research field,focusing on the epitaxial integration of antimonide lasers,the only technology covering the whole mid-to-far-infrared spectral range.We explain how a dedicated molecular-beam epitaxy strategy allows for achieving high-performance GaSb-based diode lasers,InAs/AlSb quantum cascade lasers,and InAs/GaInSb interband cascade lasers by direct growth on on-axis(001)Si substrates,whereas GaAs-on-Si or GaSb-on-Si layers grown by metal-organic vapor phase epitaxy in large capability epitaxy tools are suitable templates for antimonide laser overgrowth.We also show that etching the facets of antimonide lasers grown on Si is a viable approach in view of photonic integrated circuits.Remarkably,this review shows that while diode lasers are sensitive to residual crystal defects,the quantum cascade and interband cascade lasers grown on Si exhibit performances comparable to those of similar devices grown on their native substrates,due to their particular band structures and radiative recombination channels.Long device lifetimes have been extrapolated for interband cascade lasers.Finally,routes to be further explored are also presented. | Eric Tournié Laura Monge Bartolome Marta Rio Calvo Zeineb Loghmari Daniel A.Díaz-Thomas Roland Teissier Alexei N.Baranov Laurent Cerutti Jean-Baptiste Rodriguez | 2022 | Light(Science & Applications)2022,11,7: | 0 |
