| 3 | Parameters of a severe disease course in ulcerative colitis显示文摘AIM:To detect high risk patients with a progressive disease course of ulcerative colitis(UC) requiring immunosuppressive therapy(IT).METHODS:A retrospective,multicenter analysis of 262 UC patients from eight German tertiary inflammatory bowel disease centres was performed.Patients were divided into two groups depending on the patients need to initiate immunosuppressive therapy in the disease course.A comparison between the two groups was made with regard to demographics,clinical and laboratory parameters obtained within three months after UC diagnosis and the response to first medical therapy.Using this data,a prognostic model was established to predict the individual patients probability of requiring an immunosuppressive therapy.RESULTS:In 104(39.7%) out of 262 patients,UC therapy required an immunosuppressive treatment.Patients in this group were significantly younger at time of diagnosis(HR = 0.981 ± 0.014 per year,P = 0.009),and required significantly more often a hospitalisation(HR = 2.5 ± 1.0,P < 0.001) and a systemic corticosteroid therapy at disease onset(HR = 2.4 ± 0.8,P < 0.001),respectively.Response to steroid treatment was significantly different between the two groups of patients(HR = 5.2 ± 3.9 to 50.8 ± 35.6 compared to no steroids,P = 0.016 to P < 0.001).Furthermore,in the IT group an extended disease(HR = 3.5 ± 2.4 to 6.1 ± 4.0 compared to proctitis,P = 0.007 to P = 0.001),anemia(HR = 2.2 ± 0.8,P < 0.001),thrombocytosis(HR = 1.9 ± 1.8,P = 0.009),elevated C-reactive protein(CRP)(HR = 2.1 ± 0.9,P < 0.001),and extraintestinal manifestations in the course of disease(HR = 2.6 ± 1.1,P = 0.004) were observed.Six simple clinical items were used to establish a prognostic model to predict the individual risk requiring an IT.This probability ranges from less than 2% up to 100% after 5 years.Using this,the necessity of an immunosuppressive therapy can be predicted in 60% of patients.Our model can determine the need for an immunosuppressive drug therapy or if a 'watch and wait' approach is reasonable already early in the treatment course of UC.CONCLUSION:Using six simple clinical parameters,we can estimate the patients individual risk of developing a progressive disease course. | Andreas Stallmach Luisa Nickel Thomas Lehmann Bernd Bokemeyer Martin Bürger Dietrich Hüppe Wolfgang Kruis Susanna Nikolaus Jan C Preiss Andreas Sturm Niels Teich Carsten Schmidt | 2014 | World Journal of Gastroenterology2014，20，35： | 2 |
| 4 | Increased duodenal expression of mi R-146a and-155 in pediatric Crohn's disease显示文摘AIM: To evaluate the role of micro RNA(mi R)-146 a,-155 and-122 in the duodenal mucosa of pediatric patients with Crohn's disease(CD) and the effect of transforming growth factor-β(TGF-β) on these mi Rs in duodenal epithelial and fibroblast cells.METHODS: Formalin-fixed, paraffin-embedded biopsies derived from the macroscopically inflamed(CD inflamed: n = 10) and intact(CD intact: n = 10) duodenal mucosa of pediatric CD patients and control children(C: n = 10) were examined. Expression of mi R-146 a,-155 and-122 was determined by realtime polymerase-chain reaction(PCR). The expression of the above mi Rs was investigated in recombinant human TGF-β(1 nmol/L, 24 h) or vehicle treated small intestinal epithelial cells(CCL-241) and primary duodenal fibroblast cells derived from healthy children as well.RESULTS: Expression of mi R-146 a was significantly higher in the inflamed duodenal mucosa compared to the intact duodenal mucosa of children with CD(CD inflamed: 3.21 ± 0.50 vs CD intact: 0.62 ± 0.26, p ≤ 0.01) and to the control group(CD inflamed: 3.21 ± 0.50 vs C: 1.00 ± 0.33, p ≤ 0.05). The expression of mi R-155 was significantly increased in the inflamed region of the duodenum compared to the control group(CD inflamed: 4.87 ± 1.02 vs Control: 1.00 ± 0.40, p ≤ 0.001). The expression of mi R-122 was unchanged in the inflamed or intact mucosa of CD patients compared to controls. TGF-β treatment significantly decreased the expression of mi R-155 in small intestinal epithelial cells(TGF-β: 0.7 ± 0.083 vs Control: 1 ± 0.09, p ≤ 0.05) and also the expression of mi R-146a(TGF-β: 0.67 ± 0.04 vs Control: 1 ± 0.15, p ≤ 0.01) and mi R-155(TGF-β: 0.72 ± 0.09 vs Control: 1 ± 0.06, p ≤ 0.05) in primary duodenal fibroblasts compared to corresponding vehicle treated controls. TGF-β treatment did not influence the expression of mi R-122.CONCLUSION: The elevated expression of mi R-146 a and-155 in the inflamed duodenal mucosa of CD patients suggests the role of these mi Rs in the pathomechanism of inflammatory bowel disease. | 5 | Enterprising Academics:Heterarchical Policy Networks for Artificial Intelligence in British Higher Education显示文摘Purpose:There is limited scholarship on artificial intelligence(AI)in higher education governance,despite the growing prevalence of AI-powered technologies in many fields,including education.However,as the technology is still nascent and has yet to reach its full potential,ideas and arguments abound,championing or cautioning against the use of these technologies.Design/Approach/Methods:To fill this gap in research on policy networks and AI in British higher education,this article employs network ethnography and discourse analysis to study how ideas about AI-powered technologies in higher education circulate in policy networks in the United Kingdom.Findings:The findings evidence a policy network showing signs of a heterarchy permeated by neoliberal rationales and populated by policy actors actively promoting artificial intelligence technologies to be used in education.Originality/Value:This paper builds on existing research by looking at the university and notfor-profit sectors,in addition to the governmental and educational technology sectors.Using network ethnography,this article expands our understanding of the policy actors involved and critically analyzes ideas regarding the use of AI in education. | Dániel Béla Gellai | 2023 | ECNU Review of Education2023，6，4： | 2 |
