| 1 | Liver transplantation in patients with incidental hepatocellular carcinoma/cholangiocarcinoma and intrahepatic cholangiocarcinoma: a single-center experience显示文摘BACKGROUND: Reports of liver transplantation(LT) in patients with mixed hepatocellular carcinoma/cholangiocarcinoma(HCC/CC) and intrahepatic cholangiocarcinoma(ICC) are modest and have been mostly retrospective after pathological categorization in the setting of presumed HCC. Some studies suggest that patients undergoing LT with small and unifocal ICC or mixed HCC/CC can achieve about 40%-60% 5-year post-transplant survival. The study aimed to report our experience in patients undergoing LT with explant pathology revealing HCC/CC and ICC.METHODS: From a prospectively maintained database, we performed cohort analysis. We identified 13 patients who underwent LT with explant pathology revealing HCC/CC or ICC. RESULTS: The observed recurrence rate post-LT was 31%(4/13) and overall survival was 85%, 51%, and 51% at 1, 3 and 5 years, respectively. Disease-free survival was 68%, 51%, and 41% at 1, 3 and 5 years, respectively. In our cohort, four patients would have qualified for exception points based on updated HCC Organ Procurement and Transplantation Network imaging guidelines. CONCLUSIONS: Lesions which lack complete imaging characteristics of HCC may warrant pre-LT biopsy to fully elucidate their pathology. Identified patients with early HCC/CC or ICC may benefit from LT if unresectable. | 2 | Inflammatory bowel disease patient profiles are related to specific information needs: A nationwide survey显示文摘BACKGROUND Inflammatory bowel diseases(IBD) is a heterogenous, lifelong disease, with an unpredictable and potentially progressive course, that may impose negative psychosocial impact on patients.While informed patients with chronic illness have improved adherence and outcomes, previous research showed that the majority of IBD patients receive insufficient information regarding their disease.The large heterogeneity of IBD and the wide range of information topics makes a one-size fits all knowledge resource overwhelming and cumbersome.We hypothesized that different patient profiles may have different and specific information needs, the identification of which will allow building personalized computer-based information resources in the future.AIM To evaluate the scope of disease-related knowledge among IBD patients and determine whether different patient profiles drive unique information needs.METHODS We conducted a nationwide survey addressing hospital-based IBD clinics.A Total of 571 patients completed a 28-item questionnaire, rating the amount of information received at time of diagnosis and the importance of information, as perceived by participants, for a newly diagnosed patient, and for the participants themselves, at current time.We performed an exploratory factor analysis of the crude responses aiming to create a number of representative knowledge domains(factors), and analyzed the responses of a set of 15 real-life patient profiles generated by the study team.RESULTS Participants gave low ratings for the amount of information received at disease onset(averaging 0.9/5) and high ratings for importance, both for the newly diagnosed patients(mean 4.2/5) and for the participants themselves at current time(mean 3.5/5).Factor analysis grouped responses into six informationdomains.The responses of selected profiles, compared with the rest of the participants, yielded significant associations(defined as a difference in rating of >0.5 points with a P < 0.05).Patients with active disease showed a higher interest in work-disability, stress-coping, and therapy-complications.Patients newly diagnosed at age > 50, and patients with long-standing disease(> 10 years)showed less interest in work-disability.Patients in remission with mesalamine or no therapy showed less interest in all domains except for nutrition and long-term complications.CONCLUSION We demonstrate unmet patient information needs.Analysis of various patient profiles revealed associations with specific information topics, paving the way for building patient-tailored information resources. | Saleh Daher Tawfik Khoury Ariel Benson John R Walker Oded Hammerman Ron Kedem Timna Naftali Rami Eliakim Ofer Ben-Bassat Charles N Bernstein Eran Israeli | 2019 | World Journal of Gastroenterology2019,25,30: | 3 |

| 5 | Removing Moisture Effect on Soil Reflectance Properties: A Case Study of Clay Content Prediction显示文摘Visible, near-infrared and shortwave-infrared(VNIR-SWIR) spectroscopy is an efficient approach for predicting soil properties because it reduces the time and cost of analyses. However, its advantages are hampered by the presence of soil moisture, which masks the major spectral absorptions of the soil and distorts the overall spectral shape. Hence, developing a procedure that skips the drying process for soil properties assessment directly from wet soil samples could save invaluable time. The goal of this study was twofold:proposing two approaches, partial least squares(PLS) and nearest neighbor spectral correction(NNSC), for dry spectral prediction and utilizing those spectra to demonstrate the ability to predict soil clay content. For these purposes, we measured 830 samples taken from eight common soil types in Israel that were sampled at 66 different locations. The dry spectrum accuracy was measured using the spectral angle mapper(SAM) and the average sum of deviations squared(ASDS), which resulted in low prediction errors of less than 8% and 14%, respectively. Later, our hypothesis was tested using the predicted dry soil spectra to predict the clay content, which resulted in R^2 of 0.69 and 0.58 in the PLS and NNSC methods, respectively. Finally, our results were compared to those obtained by external parameter orthogonalization(EPO) and direct standardization(DS).
