| 3 | Inflammatory bowel disease-related colorectal cancer:Past,present and future perspectives显示文摘Inflammatory bowel disease-related colorectal cancer(IBD-CRC)is one of the most serious complications of IBD contributing to significant mortality in this cohort of patients.IBD is often associated with diet and lifestyle-related gut microbial dysbiosis,the interaction of genetic and environmental factors,leading to chronic gut inflammation.According to the“common ground hypothesis”,microbial dysbiosis and intestinal barrier impairment are at the core of the chronic inflammatory process associated with IBD-CRC.Among the many underlying factors known to increase the risk of IBD-CRC,perhaps the most important factor is chronic persistent inflammation.The persistent inflammation in the colon results in increased proliferation of cells necessary for repair but this also increases the risk of dysplastic changes due to chromosomal and microsatellite instability.Multiple pathways have been identified,regulated by many positive and negative factors involved in the development of cancer,which in this case follows the‘inflammation-dysplasia-carcinoma’sequence.Strategies to lower this risk are extremely important to reduce morbidity and mortality due to IBD-CRC,among which colonoscopic surveillance is the most widely accepted and implemented modality,forming part of many national and international guidelines.However,the effectiveness of surveillance in IBD has been a topic of much debate in recent years for multiple reasons—cost-benefit to health systems,resource requirements,and also because of studies showing conflicting long-term data.Our review provides a comprehensive overview of past,present,and future perspectives of IBD-CRC.We explore and analyse evidence from studies over decades and current best practices followed globally.In the future directions section,we cover emerging novel endoscopic techniques and artificial intelligence that could play an important role in managing the risk of IBD-CRC. | Snehali Majumder Uday Nagesh Shivaji Rangarajan Kasturi Alben Sigamani Subrata Ghosh Marietta Iacucci | 2022 | World Journal of Gastrointestinal Oncology2022，14，3： | 5 |
| 4 | Shortness of breath in clinical practice: A case for left atrial function and exercise stress testing for a comprehensive diastolic heart failure workup显示文摘The symptom cluster of shortness of breath(SOB) contributes significantly to the outpatient workload of cardiology services. The workup of these patients includes blood chemistry and biomarkers, imaging and functional testing of the heart and lungs. A diagnosis of diastolic heart failure is inferred through the exclusion of systolic abnormalities, a normal pulmonary function test and normal hemoglobin, coupled with diastolic abnormalities on echocardiography. Differentiating confounders such as obesity or deconditioning in a patient with diastolic abnormalities is difficult. While the most recent guidelines provide more avenues for diagnosis, such as incorporating the left atrial size, little emphasis is given to understanding left atrial function, which contributes to at least 25% of diastolic left ventricular filling; additionally, exercise stress testing to elicit symptoms and test the dynamics of diastolic parameters, especially when access to the 'gold standard' invasive tests is lacking, presents clinical translational gaps. | 7 | Anxiolytic property of hydro-alcohol extract of Lactuca sativa and its effect on behavioral activities of mice显示文摘Lactuca sativa,belonging to the Asteraceae family,is a leafy vegetable known for its medicinal properties.This study aimed to understand the mechanism of Lactuca sativa extract with respect to pharmacological action.We investigated the anxiolytic effects of hydro-alcoholic extract of leaves of Lactuca sativa on mice.The behavioral tests performed on mice models to assess anti-anxiety properties were:open field test (OFT),elevated plus maze test (EPM),elevated T maze test,and marble burying test.Increased locomotor activity and time spent in the 'open-arm' were observed in extract fed group.Malondialdehyde (MDA) and nitrite levels were decreased,catalase and glutathione levels were increased in Lactuca sativa treated mice.The data obtained in the present study suggests that the extract of Lactuca sativa can afford significant protection against anxiolytic activity. | Singapura Nagesh Harsha Kandangath Raghavan Anilakumar | 2013 | The Journal of Biomedical Research2013，27，1： | 4 |
