1Comparison of the efficacy and safety of sedation between dexmedetomidine-remifentanil and propofol-remifentanil during endoscopic submucosal dissection显示文摘AIM: To compare the efficacy and safety of sedation protocols for endoscopic submucosal dissection(ESD) between dexmedetomidine-remifentanil and propofolremifentanil.METHODS: Fifty-nine patients scheduled for ESD were randomly allocated into a dexmedetomidineremifentanil(DR) group or a propofol-remifentanil(PR) group. To control patient anxiety, dexmedetomidine or propofol was infused to maintain a score of 4-5 on the Modified Observer's Assessment of Alertness/Sedation scale. Remifentanil was infused continuously at a rate of 6 μg/kg per hour in both groups. The ease of advancing the scope into the throat, gastric motility grading, and satisfaction of the endoscopist and patient were assessed. Hemodynamic variables and hypoxemic events were compared to evaluate patient safety.RESULTS: Demographic data were comparable between the groups. The hemodynamic variables and pulse oximetry values were stable during the procedure in both groups despite a lower heart rate in the DR group. No oxygen desaturation events occurred in either group. Although advancing the scope into the throat was easier in the PR group('very easy' 24.1% vs 56.7%, P = 0.010), gastric motility was moresuppressed in the DR group('no + mild' 96.6% vs 73.3%, P = 0.013). The endoscopists felt that the procedure was more favorable in the DR group('very good + good' 100% vs 86.7%, P = 0.042), whereas patient satisfaction scores were comparable between the groups. En bloc resection was performed 100% of the time in both groups, and the complete resection rate was 94.4% in the DR group and 100% in the PR group(P = 0.477). CONCLUSION: The efficacy and safety of dexmedetomidine and remifentanil were comparable to propofol and remifentanil during ESD. 7The Contribution of Root and Tuber Crops to Food Security:A Review显示文摘Nigeria is a country endowed with enormous food and agricultural resources such as yam, cassava, cocoa, rice, sugar cane, maize and millet. In the early years of Nigeria’s independence, cash crops such as rubber, cashew, groundnut and oil palm were the major crops around which the economy revolved. Today, there is a shift to alternatives for biofuel, pharmaceutical starch and derivatives using root and tuber crops, which have continued to play a significant role in the agricultural revolution of the country. The current agricultural transformation agenda in Nigeria is intended to engineer the bulk of root and tuber crops in the country into value-added products to match the increasing new tastes and demands for products across the globe. It is also intended to provide job opportunities and to generate additional income for low-income earners, especially in the rural areas. There is, therefore, the need to intensify research efforts aimed at exploring the potentials of major and minor root and tuber crops to ensure food security. In this review, six (6) major root and tuber crops, namely cassava (Manihot esculenta Crantz), yam (Dioscorea spp.), sweet potato (Ipomoea batatas (L.) Lam), potato (Solanum tuberosum L.), cocoyam (Xanthosoma sagittifolium Schott) and ginger (Zingiber officinale L.) as well as three (3) minor ones, namely the Hausa potato (Solenostemon rotundifolius (Poir) J. K. Morton), Livingstone potato (Plectranthus esculentus N.E.Br.) and the Polynesian arrowroot (Tacca leontopetaloides (L.) Kuntze) were examined. In spite of the numerous benefits derivable from these crops in terms of nutrition and medicinal values, some have been neglected to the extent that they are classified as orphan crops. 9LipL21 mRNA expression in lungs of hamsters infected with pathogenic Leptospira显示文摘Objective:Pulmonary haemorrhage is an increasing cause of death in leptospirosis patients.However,molecular mechanism underlying pathologies in this organ is not clearly understood.It has been shown that sodium transport was disturbed following Leptospira infection.LipL21 is the second abundant outer membrane protein found only in pathogenic Leptospira.Its expression in vivo has been shown which suggests that this protein may be involved in survival in hosts or pathogenesis.However,the expression of this protein in host organs and its role in lung pathology has not been demonstrated.In this study we demonstrated the expression of LipL21 in lungs of hamsters infected with pathogenic Leptospira.Methods:Lung tissues were collected from Golden Syrian hamsters injected with Leptospira interrogans serovar Pyrogenes at days 3,5 and 7 post-infection.Four hamsters were used for each time point.Lungs from non-infected hamsters were collected as a control group.LipL21 mRNA expression in lung tissues was investigated by reverse transcription and nested PCR.Results:LipL21 mRNA expression was detected in all lung tissues from hamsters infected with pathogenic Leptospira.No PCR product was detected when tissues from non-infected hamsters were investigated.Conclusion:Our data demonstrated that LipL21 is expressed in lungs of hamsters infected with pathogenic Leptospira.Additional experiments such as quantitation and localization of LipL21 expression in lungs will provide further information whether this protein is involved in pathogenesis.Chintana Chirathaworn Namo Suksomyos Somchai Utivamek Somboon Keelawat Duangjai Suwancharoen Duangporn Phulsuksombati Yong Poovorawan 2009Asian Pacific Journal of Tropical Medicine2009,2,5:0
