1针刺风池穴即刻后效应与视觉皮层功能相关的静息态fMRI研究显示文摘目的运用静息态功能磁共振(rsfMRI)技术,观察针刺风池穴即刻后脑效应,探索其调制视觉中枢的脑功能机制。方法招募11名健康志愿者,均先后接受左侧风池穴手法针刺及表皮触觉对照刺激,以随机数字表法决定受试者接受2种剌激的先后顺序。每次刺激时间为30 s,频率60次/min。刺激前、即刻后均行6 min脑rsfMRI扫描。采用DPABI4.3软件对rsfMRI数据进行统计学分析,包括比率低频振幅(fALFF)、局部一致性(ReHo)、度中心(DC)分析局部脑活动变化;种子点双侧后扣带皮质(PCC)、腹内侧前额叶(VMPFC)分析全脑功能连接,观察针刺风池穴后的即刻脑功能及全脑功能连接的变化。结果与表皮触觉刺激比较,针刺左侧风池穴视觉相关脑区右侧楔叶、楔前叶局部活动增强(体素水平P<0.01,簇水平P<0.05),双侧种子点PCC与左侧顶下小叶脑功能连接增强,种子点VMPFC与右侧楔前叶脑功能连接降低(体素水平P<0.01,簇水平P<0.05)。结论风池穴可以调节视觉中枢及邻近脑区功能活动,调制默认网络脑功能连接,可能是其治疗眼部疾病等临床适应证的重要脑功能基础。李小娇 Mostafizi Narjes 许可 王燕平 韩铭 陈媛媛 方继良 2019北京中医药2019,0,5:21
2Do inhaled corticosteroids increase the risk of Pneumocystis pneumonia in people with lung cancer?显示文摘Pneumocystis pneumonia(PCP) is a life-threatening infection in immunocompromised patients. It is relatively uncommon in patients with lung cancer. We report a case of PCP in a 59-year-old man with a past medical history of chronic obstructive pulmonary disease treated with formoterol and a moderate daily dose of inhaled budesonide. He had also advanced stage non-small lung cancer treated with concurrent chemo-radiation with a cisplatin-etoposide containing regimen. The diagnosis of PCP was suspected based on the context of rapidly increasing dyspnea, lymphopenia and the imaging findings. Polymerase chain reaction testing on an induced sputum specimen was positive for Pneumocystis jirovecii. The patient was treated with oral trimethoprim-sulfamethoxazole and systemic corticotherapy and had showed clinical and radiological improvement. Six months after the PCP diagnosis, he developed a malignant pleural effusion and expired on hospice care. Through this case, we remind the importance of screening for PCP in lung cancer patients under chemotherapeutic regimens and with increasing dyspnea. In addition, we alert to the fact that long-term inhaled corticosteroids may be a risk factor for PCP in patients with lung cancer. 3Combining population genomics and ecological niche modeling to assess taxon limits between Carex jemtlandica and C. lepidocarpa显示文摘Carex section Ceratocystis(Cyperaceae)is a group of recently evolved plant species,in which hybridization is frequent,introgression is documented,taxonomy is complex,and morphological boundaries are vague.Within this section,a unified taxonomic treatment of the Carex jemtlandica-Carex lepidocarpa species complex does not exist,and Norway may currently be the sole country accepting species rank for both.Carex jemtlandica is mainly confined to Fennoscandia and is thus a Fennoscandian conservation responsibility.This motivated us to test the principal hypothesis that both C jemtlandica and C lepidocarpa represent evolutionary significant units,and that both deserve their current recognition at species level.We investigated their evolutionary distinctiveness in Norway,using restriction site-associated DNA sequencing and ecological niche modeling.Our genomic results reveal two genetic clusters,largely corresponding to C jemtlandica and C lepidocarpa that also remain distinct in sympatry,despite clear indications of ongoing hybridization and introgression.The ecological niche modeling suggests that they occupy different environmental niches.Jointly,our results clearly show that C.jemtlandica and C lepidocarpa represent separately evolving entities that should qualify recognition as evolutionary significant units.Given the high level of introgression compared to other hybridizing species pairs in Carex we recommend treating C jemtlandica as a subspecies of C lepidocarpa.Malene Nygaard Petri Kemppainen James D.M.Speed Reidar Elven Kjell Ivar Flatberg Leif P.Galten Narjes Yousefi Heidi Solstad Mika Bendiksby 2021Journal of Systematics and Evolution2021,59,4:3
4Bone marrow microenvironment: The guardian of leukemia stem cells显示文摘Bone marrow microenvironment (BMM) is the main sanctuary of leukemic stem cells (LSCs) and protects these cells against conventional therapies. However, it may open up an opportunity to target LSCs by breaking the close connection between LSCs and the BMM. The elimination of LSCs is of high importance, since they follow cancer stem cell theory as a part of this population. Based on cancer stem cell theory, a cell with stem cell-like features stands at the apex of the hierarchy and produces a heterogeneous population and governs the disease. Secretion of cytokines, chemokines, and extracellular vesicles, whether through autocrine or paracrine mechanisms by activation of downstream signaling pathways in LSCs, favors their persistence and makes the BMM less hospitable for normal stem cells. While all details about the interactions of the BMM and LSCs remain to be elucidated, some clinical trials have been designed to limit these reciprocal interactions to cure leukemia more effectively. 11Risk-based probabilistic thermal-stress analysis of concrete arch dams显示文摘The probabilistic risk of arch dam failure under thermal loading is studied. The incorporated uncertainties, which are defined as random variables, are associated with the most affecting structural (material) properties o f concrete and thermal loading conditions. Karaj arch dam is selected as case study. The dam is numerically modeled along with its foundation in three-dimensional space;the temperature and thermal stress distribution is investigated during the operating phase. The deterministic thermal finite element analysis o f the dam is combined with the structural reliability methods in order to obtain thermal response predictions, and estimate the probability o f failure in the risk analysis context. The tensile overstressing failure mode is considered for the reliability analysis. The thermal loading includes ambient air and reservoir temperature variations. The effect o f solar radiation is considered by an increase in the ambient temperatures. Three reliability methods are employed: the first-order second-moment method, the first-order reliability method, and the Monte-Carlo simulation with Latin Hypercube sampling. The estimated failure probabilities are discussed and the sensitivity o f random variables is investigated.
