| 1 | Improvement of prognosis for unresectable biliary tract cancer显示文摘AIM:To evaluate the chemotherapeutic outcomes and confirm the recent improvement of prognosis for unresectable biliary tract cancer.METHODS:A total of 186 consecutive patients with unresectable biliary tract cancer,who had been treated with chemotherapy between 2000 and 2009 at five institutions in Japan,were retrospectively analyzed.These patients were divided into three groups based on the year beginning chemotherapy:Group A(2000-2003),Group B(2004-2006),and Group C(2007-2009).The data were fixed at the end of December 2011.Overall survival and time-to-progression were analyzed and compared chronologically.RESULTS:No patient characteristics were significantly different among the three groups.The gallbladder was involved in about half of the patients in each group,and metastatic biliary tract cancer was present in three quarters of the enrollees.In Group A,5-fluorouracilbased chemotherapies were primarily selected as firstline chemotherapy,and only 24% were treated with second-line chemotherapy.In Group B,gemcitabine or S-1 monotherapy was mainly introduced as firstline chemotherapy,and 51% of the patients who were refractory to first-line chemotherapy were treated with second-line chemotherapy mainly with monotherapy.In Group C,the combination therapy with gemcitabine and S-1 was mainly chosen as first-line chemotherapy,and 53% of the patients refractory to first-line chemotherapy were treated with second-line chemotherapy mainly with combination therapy.The median timeto-progressions were 4.4 mo,3.5 mo and 5.9 mo in Groups A,B and C,respectively(4.4 mo vs 3.5 mo vs 5.9 mo,P < 0.01).The median overall survivals were 7.1,7.3,and 11.7 mo in Groups A,B and C(7.1 mo vs 7.3 mo vs 11.7 mo,P = 0.03).Induction rates of all three drugs(gemcitabine,platinum analogs,and fluoropyrimidine) in Groups A,B and C were 4%,2% and 27%(4% vs 2% vs 27%,P < 0.01).CONCLUSION:The prognosis of unresectable biliary tract cancer has improved recently.Using three effective drugs(gemcitabine,platinum analogs,and fluoropyrimidine) may improve the prognosis of this cancer. | Takashi Sasaki Hiroyuki Isayama Yousuke Nakai Naminatsu Takahara Naoki Sasahira Hirofumi Kogure Suguru Mizuno Hiroshi Yagioka Yukiko Ito Natsuyo Yamamoto Kenji Hirano Nobuo Toda Minoru Tada Masao Omata Kazuhiko Koike | 2013 | World Journal of Gastroenterology2013，19，1： | 8 |

| 9 | Comprehensive evaluation of dioxins and dioxin-like compounds in surface soils and river sediments from e-waste-processing sites in a village in northern Vietnam:Heading towards the environmentally sound management of e-waste显示文摘The management of electronic waste(e-waste),which can be a source of both useful materials and toxic substances,depending on the processing method,is important for promoting material cycling.In this study,we used the dioxin-responsive chemical-activated luciferase gene expression(DR-CALUX)assay combined with gas chromatographyehigh-resolution mass spectrometry to evaluate the levels of dioxin-like compounds in surface soils and river sediments collected in and around an e-waste-processing village in northern Vietnam.The WHO-TEQs(Toxic equivalents)of polychlorinated dibenzo-p-dioxins and dibenzofurans(PCDD/Fs),coplanar polychlorinated biphenyls(Co-PCBs),and polybrominated dibenzo-pdioxins and dibenzofurans(PBDD/Fs)in soils collected in January 2012 ranged from 0.29 to 310 pg/g(median 2.9 pg/g,n=32),and the WHO-TEQs in sediments ranged from 0.96 to 58 pg/g(median 4.4 pg/g,n=8).Dioxin-like activities(CALUX-TEQs[2,3,7,8-TCDD equivalent])in soils collected in January 2012,2013,and 2014 ranged from<30 to 4300 pg/g(median<30 pg/g,n=96),and the activities in sediments ranged from<30 to 4000 pg/g(median 33 pg/g,n=24).Dioxin-like compounds accumulated in samples collected around e-waste-processing areas such as open-burning sites and e-waste-processing workshops,and the compounds may be transported from their sources to surrounding areas over the course of several years.Some of the CALUX-TEQs,but not WHO-TEQs,values were higher than the maximum acceptable WHO-TEQs promulgated by various authorities,indicating that all dioxin-like compounds should be evaluated in samples collected from e-waste-processing areas.Our findings suggest that open burning and open storage of e-waste should be prohibited and that wastewater treatment should be implemented at each workshop to reduce contamination by dioxin-like compounds from e-waste. | Go Suzuki Masayuki Someya Hidenori Matsukami Nguyen Minh Tue Natsuyo Uchida Le Huu Tuyen Pham Hung Viet Shin Takahashi Shinsuke Tanabe Abraham Brouwer Hidetaka Takigami | 2016 | Emerging Contaminants2016，2，2： | 1 |

Since then, serum HCV-RNA and liver function tests had been negative. He had continued to drink more than 100 g/d of alcohol as before. In June 2003, a 5-cm tumor was found in the posterior segment of the liver. The tumor was curatively resected and the surgical specimen showed a well-differentiated hepatocellular carcinoma (HCC). Non-cancerous lesions of the liver revealed fibrosis at stage F3 with minimal to mild inflammation of grade A1.
