| 1 | Helicobacter pylori infection and respiratory diseases:a review显示文摘In the past few years, a variety of extradigestive disorders,including cardiovascular, skin, rheumatic and liver diseases,have been associated with Helicobacter pylori(H. pylori)infection. The activation of inflammatory mediators by H. pyloriseems to be the pathogenetic mechanism underlying theobserved associations. The present review summarizes thecurrent literature, including our own studies, concerning theassociation between H. pvloriinfection and respiratory diseases.A small number of epidemiological and serologic, case-control studies suggest that H. pyloriinfection may beassociated with the development of chronic bronchitis. Afrequent coexistence of pulmonary tuberculosis and H. pyloriinfection has also been found. Moreover, recent studies haveshown an increasedH. pyloriseroprevalence in patients withbronchiectasis and in those with lung cancer. On the otherhand, bronchial asthma seems not to be related withH. pyloriinfection.All associations between H. pyloriinfection and respiratorydiseases are primarily based on case-control studies,concerning relatively small numbers of patients. | 2 | Transcription factor PDX-1 in human colorectal adenocarcinoma:A potential tumor marker?显示文摘AIM: To examine the expression of pancreatic duodenal homeobox-1 (PDX-1) transcription factor in human colorectal cancer. METHODS: RT-PCR, Western blotting, and immuno-histochemistry were performed to determine the expression pattern of transcription factor PDX-1 in primary colorectal tumor, hepatic metastasis, and benign colon tissue from a single patient. RESULTS: The highest PDX-1 transcription levels were detected in the metastasis material. Lower levels of PDX-1 were found to be present in the primary tumor, while normal colon tissue failed to express detectable levels of PDX-1. Western blot data revealed a PDX-1 expression pattern identical to that of mRNA expression. Immunohistochemistry confirmed high metastasis PDX-1 expression, lower levels in the primary tumor, and the presence of only traces of PDX-1 in normal colon tissue. | 5 | Current evidence of drug-elution therapy for infrapopliteal arterial disease显示文摘New and sophisticated endovascular devices, such as drug-eluting stents(DES)and drug-coated balloons(DCB), provide targeted drug delivery to affected vessels. The invention of these devices has made it possible to address the reparative cascade of arterial wall injury following balloon angioplasty that results in restenosis. DESs were first used for the treatment of infrapopliteal lesions almost 20 years ago. More recently, however, DCB technology is being investigated to improve outcomes of endovascular below-the-knee arterial procedures, avoiding the need for a metallic scaffold. Today, level IA evidence supports the use of infrapopliteal DES for short to medium length lesions,although robust evidence that justifies the use of DCBs in this anatomical area is missing. | 10 | Successful en bloc resection of primary hepatocellular carcinoma directly invading the stomach and pancreas显示文摘Multivisceral surgical resection for cure was successfully performed in a 70-year-old man suffering from a primary hepatocellular carcinoma(HCC)associated with direct invasion to the stomach and pancreas.The patient presented with gastric outlet obstruction,upper abdominal pain and a history of chronic liver disease due to hepatitis B virus(HBV)infection.Upper gastro-intestinal(GI)endoscopy revealed an infiltrating tumor protruding through the gastric wall and obliterating the lumen.Computer tomograghy(CT)and magnetic resonance imaging(MRI)scan demonstrated a 15-cm tumor in the left lateral segment of the liver with invasion to the stomach and pancreas.Alpha-foetoprotein (AFP)levels and liver function tests were normal.The patient underwent an en bloc left hepatectomy,total gastrectomy,distal pancreatectomy with splenectomy and radical lymphadenectomy.Pathology revealed a poorly differentiated,giant cell HCC involving the stom-ach and pancreas.Disease-free margins of resection were achieved.The patient's postoperative course was uneventful.Sixteen months after surgery,he has norecurrence or distal metastasis.Direct invasion of HCC into the GI tract is rarely encountered.Complete surgical resection should be considered in selected patients with an appropriate hepatic functional reserve. | Dimitris P Korkolis Chrysanthi Aggeli George D Plataniotis Emmanuel Gontikakis Helen Zerbinis Nikitas Papantoniou Dimitris Xinopoulos Nikiforos Apostolikas Perikles P Vassilopoulos | 2009 | World Journal of Gastroenterology2009，15，9： | 1 |
