| 2 | Clinical significance of hepatitis B surface antigen mutants显示文摘Hepatitis B virus(HBV) infection is a major public health problem in many countries, with nearly 300 million people worldwide carrying HBV chronic infection and over 1 million deaths per year due to cirrhosis and liver cancer. Several hepatitis B surface antigen(HBs Ag) mutations have been described, most frequently due to a single amino acid substitution and seldom to a nucleotide deletion. The majority of mutations are located in the S region, but they have also been found in the pre-S1 and pre-S2 regions. Single amino acid substitutions in the major hydrophilic region of HBs Ag, called the 'a' determinant, have been associated with immune escape and the consequent failure of HBV vaccination and HBs Ag detection, whereas deletions in the pre-S1 or pre-S2 regions have been associated with the development of hepatocellular carcinoma. | 3 | Role of occult hepatitis B virus infection in chronic hepatitis C显示文摘The development of sensitive assays to detect small amounts of hepatitis B virus(HBV) DNA has favored the identification of occult hepatitis B infection(OBI), a virological condition characterized by a low level of HBV replication with detectable levels of HBV DNA in liver tissue but an absence of detectable surface antigen of HBV(HBs Ag) in serum. The gold standard to diagnose OBI is the detection of HBV DNA in the hepatocytes by highly sensitive and specific techniques, a diagnostic procedure requiring liver tissue to be tested and the use of non-standardized non-commercially available techniques. Consequently, in everyday clinical practice, the detection of anti-hepatitis B core antibody(antiHBc) in serum of HBs Ag-negative subjects is used as a surrogate marker to identify patients with OBI. In patients with chronic hepatitis C(CHC), OBI has been identified in nearly one-third of these cases. Considerable data suggest that OBI favors the increase of liver damage and the development of hepatocellular carcinoma(HCC) in patients with CHC. | 4 | Micro-RNAs in hepatitis B virus-related chronic liver diseases and hepatocellular carcinoma显示文摘MicroRNAs(miRNAs) are small non-coding RNAs that modulate gene expression at the post-transcriptional level by affecting both the stability and translation of complementary mRNAs. Several studies have shown that mi RNAs are important regulators in the conflicting efforts between the virus(to manipulate the host for its successful propagation) and the host(to inhibit the virus),culminating in either the elimination of the virus or its persistence. An increasing number of studies report a role of miRNAs in hepatitis B virus(HBV) replication and pathogenesis. In fact,HBV is able to modulate different host miRNAs,particularly through the transcriptional transactivator HBx protein and,conversely,different cellular miRNAs can regulate HBV gene expression and replication by a direct binding to HBV transcripts or indirectly targeting host factors. | 5 | Hepatitis B virus and hepatitis C virus infection inhealthcare workers显示文摘Approximately 3 million healthcare workers per year receive an injury with an occupational instrument, with around 2000000 exposures to hepatitis B virus(HBV) and 1000000 to hepatitis C virus(HCV). Although an effective HBV vaccine has been available since the early eighties, and despite the worldwide application of universal vaccination programs started in the early nineties, HBV still remains a prominent agent of morbidity and mortality. There is no vaccine to limit the diffusion of HCV infection, which progresses to chronicity in the majority of cases and is a major cause of morbidity and mortality worldwide due to a chronic liver disease. Healthcare workers are frequently exposed by a mucosal-cutaneous or percutaneous route to accidental contact with human blood and other potentially infectious biological materials while carrying out their occupational duties. Mucosal-cutaneous exposure occurs when the biological material of a potentially infected patient accidentally comes in contact with the mucous membranes of the eyes or mouth or with the skin of a healthcare worker. Percutaneous exposure occurs when an operator accidentally injures himself with a sharp contaminated object, like a needle, blade or other sharp medical instrument. About 75% of the total occupational exposure is percutaneous and 25% mucosal-cutaneous, the risk of infecting a healthcare worker being higher in percutaneous than in mucosal-cutaneous exposure. | 6 | Epidemiology and management of hepatitis C virus infections in immigrant populations显示文摘Background:At present,there is a continuous flow of immigrants from the south of the world to north-western countries.Often immigrants originate from areas of high-prevalence of viral hepatitis and pose a challenge to the healthcare systems of the host nations.Aims of this study is to evaluate the prevalence and virological and clinical characteristics of hepatitis C virus(HCV)infection in immigrants and the strategies to identify and take care of the immigrants infected with HCV.Main body:We conducted an electronic literature search in several biomedical databases,including PubMed,Google Scholar,Scopus,Web of Science,using different combinations of key words:'HCV infection;chronic hepatitis C,immigrants;low-income countries'.We included studies written in English indicating the epidemiological data of HCV infection in the immigrant population,studies that assessed the clinical presentation,clinical management and treatment with directly acting antiviral agent in immigrants,HCV infection is unevenly distributed in different countries,with worldwide prevalence in the general population ranging from 0.5 to 6.5%.In Western countries and Australia this rate ranges from 0.5 to 1.5%,and reaches 2.3%in countries of south-east Asia and eastern Mediterranean regions,3.2%in China,0.9%in India,2.2%in Indonesia and 6.5%in Pakistan;in sub-Saharan Africa the prevalence of HCV infection varies from 4 to 9%.Immigrants and refugees from intermediate/high HCV endemic countries to less-or non-endemic areas are more likely to have an increased risk of HCV infection due to HCV exposure in their countries of origin.Because of the high HCV endemicity in immigrant populations and of the high efficacy of directly acting antiviral agent therapy,a campaign could be undertaken to eradicate the infection in this setting.Conclusions:The healthcare authorities should support screening programs for immigrants,performed with the help of cultural mediators and including educational aspects to break down the barriers limiting access to treatments,which obtain the HCV clearance in 95%of cases and frequently prevent the development of liver cirrhosis and hepatocellular carcinoma. | Nicola Coppola Loredana Alessio Lorenzo Onorato Caterina Sagnelli Margherita Macera Evangelista Sagnelli Mariantonietta Pisaturo | 2019 | Infectious Diseases of Poverty2019,8,2: | 3 |
| 7 | Hepatitis B virus infection in undocumented immigrants and refugees in Southern Italy:demographic,virological,and clinical features显示文摘Background:The data on hepatitis b virus(HBV)infection in immigrants population are scanty.The porpoise of this study was to define the demographic,virological,and clinical characteristics of subjects infected with HBV chronic infection in a cohort of immigrants living in Naples,Italy.Methods:A screening for HBV infection was offered to 1,331 immigrants,of whom 1,212(91%)(831 undocumented immigrants and 381 refugees)accepted and were screened for hepatitis B surface antigen(HBsAg)and anti-hepatitis B core antibody(HBc).Those found to be HBsAg positive were further investigated at third-level infectious disease units.Results:Of the 1,212 immigrants screened,116(9.6%)were HBsAg positive,490(40.4%)were HBsAg negative/anti-HBc positive,and 606(50%)were seronegative for both.Moreover,21(1.7%)were anti-human immunodeficiency virus positive and 45(3.7%)were anti-hepatitis C virus positive.The logistic regression analysis showed that male sex(OR:1.79;95%CI:1.28-2.51),Sub-Saharan African origin(OR:6.18;95%CI:3.37-11.36),low level of schooling(OR:0.96;95%CI:0.94-0.99),and minor parenteral risks for acquiring HBV infection(acupuncture,tattoo,piercing,or tribal practices,OR:1.54;95%CI:1.1-2.16)were independently associated with ongoing or past HBV infection.Of the 116 HBsAg-positive immigrants,90(77.6%)completed their diagnostic itinerary at a third-level infectious disease unit:29(32.2%)were asymptomatic non-viremic HBsAg carriers,43(47.8%)were asymptomatic viremic carriers,14(15.6%)had chronic hepatitis,and four(4.4%)had liver cirrhosis,with superimposed hepatocellular carcinoma in two.Conclusions:The data illustrate the demographic,clinical and virological characteristics of HBV infection in immigrants in Italy and indicate the need for Italian healthcare authorities to enhance their support for providing screening,HBV vaccination,treatment,and educational programs for this populations. | Nicola Coppola Loredana Alessio Luciano Gualdieri Mariantonietta Pisaturo Caterina Sagnelli Carmine Minichini Giovanni Di Caprio Mario Starace Lorenzo Onorato Giuseppe Signoriello Margherita Macera Italo Francesco Angelillo Giuseppe Pasquale Evangelista Sagnelli | 2017 | Infectious Diseases of Poverty2017,6,1: | 3 |
| 8 | Role of genetic polymorphisms in hepatitis C virus chronic infection显示文摘AIM: To analyze the host genetics factors influencing the clinical course and the response to antiviral treatment in patients with chronic hepatitis C(CHC).METHODS: We conducted an electronic search on the Pub Med and MEDLINE(2000-2014) databases and Cochrane library(2000-2014). A total of 73 articles were retrieved and their data were extensively evaluated and discussed by the authors and then analyzed in this review article.RESULTS: Several studies associated polymorphisms in the interleukin 28 B gene on chromosome 19(19q13.13) with a spontaneous viral clearance in acute hepatitis C and with the response to pegylated interferon(PegIFN)-based treatment in chronic hepatitis C patients. Other investigations demonstrated that inosine triphosphate pyrophosphatase genetic variants protect hepatitis C virus-genotype-1 CHC patients from ribavirin-induced anemia, and other studies that a polymorphism in the patatin-like phospholipase domain-containing protein 3 was associated with hepatic steatosis in CHC patients. Although not conclusive, some investigations suggested that the vitamin D-associated polymorphisms play an important role in the achievement of sustained virologic response in CHC patients treated with Peg-IFN-based antiviral therapy. Several other polymorphisms have been investigated to ascertain their possible impact on the natural history and on the response to treatment in patients with CHC, but the data are preliminary and warrant confirmation.
