| 4 | Murine lung eosinophil activation and chemokine production in allergic airway inflammation显示文摘Eosinophils play important roles in asthma and lung infections.Murine models are widely used for assessing the functional significance and mechanistic basis for eosinophil involvements in these diseases.However,little is known about tissue eosinophils in homeostasis.In addition,little data on eosinophil chemokine production during allergic airway inflammation are available.In this study,the properties and functions of homeostatic and activated eosinophils were compared.Eosinophils from normal tissues expressed costimulation and adhesion molecules B7-1,B7-2 and ICAM-1 for Ag presentation but little major histocompatibility complex(MHC)class II,and were found to be poor stimulators of T-cell proliferation.However,these eosinophils expressed high levels of chemokine mRNA including C10,macrophage inflammatory protein(MIP)-1a,MIP-1c,MIP-2,eotaxin and monocyte chemoattractant protein-5(MCP-5),and produced chemokine proteins.Eosinophil intracellular chemokines decreased rapidly with concomitant surface marker downregulation upon in vitro culturing consistent with piecemeal degranulation.Lung eosinophils from mice with induced allergic airway inflammation exhibited increased chemokines mRNA expression and chemokines protein production and upregulated MHC class II and CD11c expression.They were also found to be the predominant producers of the CCR1 ligands CCL6/C10 and CCL9/MIP-1c in inflamed lungs.Eosinophil production of C10 and MIP-1c correlated with the marked influx of CD11bhigh lung dendritic cells during allergic airway inflammation and the high expression of CCR1 on these dendritic cells(DCs).The study provided baseline information on tissue eosinophils,documented the upregulation of activation markers and chemokine production in activated eosinophils,and indicated that eosinophils were a key chemokine-producing cell type in allergic lung inflammation. | C Edward Rose Jr Joanne A Lannigan Paul Kim James J Lee Shu Man Fu Sun-sang J Sung | 2010 | Cellular & Molecular Immunology2010,7,5: | 6 |

| 8 | Transversus abdominis plane infiltration for laparoscopic gastric banding: A pilot study显示文摘AIM: To estimate an effect size for the transversus abdominis plane(TAP) infiltration on quality of recovery in patients undergoing laparoscopic gastric band surgery. METHODS: The pilot study was a randomized, double blinded, placebo controlled trial. Patients undergoing laparoscopic gastric band surgery were randomized to receive a bilateral TAP infiltration with 20 mL of 0.5% ropivacaine or saline. The evaluated outcomes included quality of recovery-40(QoR-40) at 24 h, postoperative opioid consumption and pain. Data was examined using the Mann-Whitney U test. RESULTS: Nineteen subjects were recruited. There was a positive trend favoring the TAP infiltration group in global QoR-40 scores at 24 h after surgery, median [interquartile range(IQR)] of 175.5(170-189) com-pared to 170(160-175) in the control group(P = 0.06). There also a positive trend toward a lower cumulative opioid consumption in the TAP infiltration group, median(IQR) of 7.5(2.5-11.5) mg iv morphine equivalents compared to 13(7-21.5) in the control group(P = 0.07). Correlation analysis(Spearman's Rho) demonstrated an inverse relationship between 24 h cumulative opioid consumption and global QoR-40 scores,-0.49(P = 0.03). CONCLUSION: The use of multimodal analgesic techniques to reduce opioid related side effects is particularly desirable in morbidly obese patients undergoing gastric reduction surgery. The TAP infiltration seems to have a clinically important effect in reducing postoperative opioid consumption and improve quality of recovery after laparoscopic gastric band surgery in morbid obese patients. | 11 | Observing the stages of bystander intervention in virtual reality simulation显示文摘BACKGROUND: Understanding bystander reactions to an emergency is an important component of effective training. Four stages of bystander intervention (BI) have been previously described: noticing the situation as a problem, interpreting when it is appropriate to intervene, recognizing personal responsibility to intervene, and knowing how to intervene. Using virtual reality (VR) to simulate emergencies such as sudden cardiac arrest (SCA) can be used to study these stages. METHODS: In a secondary analysis of an observational cohort study, we analyzed bystander self-effi cacy for stages of BI before and after simulated SCA. Each subject participated in a singleplayer, immersive, VR SCA scenario. Subjects interacted with simulated bystanders through voice commands (“call 911”,“get an AED”). Actions taken in scenario, like performing CPR, were documented. Scenario BI actions were compared based on dichotomized comfort/discomfort. RESULTS: From June 2016 to June 2017, 119 subjects participated. Average age was 37±14 years, 44% were female and 46% reported CPR training within 2 years. During the scenario, 98%“noticed the event” and “interpreted it as a problem”, 78%“took responsibility”, and 54%“possessed the necessary skills”. Self-effi cacy increased from pre- to post-scenario: noticing the event increased from 80% to 96%;interpreting as a problem increased from 86% to 97%;taking responsibility increased from 56% to 93%;possessing necessary skills increased from 47% to 63%(P<0.001). CONCLUSION: Self-efficacy to respond to an SCA event increased pre- to post-scenario. Bystanders who reported feeling comfortable “taking responsibility to intervene” during an emergency were more likely to take action during a simulated emergency. | David G.
