1A comparative analysis of distal locked and unlocked long proximal femoral nail antirotation (PFNA-II) in the fixation of stable intertrochanteric fractures显示文摘Purpose:Long proximal femoral nail anti-rotation (PFNA-II) is a preferred implant in recent years for fixation of pertrochanteric fractures,especially in osteoporotic patients.The purpose of this study is to prospectively investigate the effect of distal locking in long PFNA-II fixation of stable intertrochanteric fractures.Methods:A total of 58 patients with isolated stable intertrochanteric fractures and treated in our hospital during the study period of 2017–2019 by distal locked or unlocked long PFNA-II fixation were included in this study.Patients who had multiple injuries or open fractures were excluded.There were 40 female and 18 male patients,with 33 affecting the left side and 25 the right side.Of them,31 belonged to the distal locked group (group A) and 27 to the unlocked group (group B).Surgical procedures and implants used in both groups were similar except for the distal locking of the nails.General data (age,gender,fracture side,etc.) showed no significant difference between two groups (allp > 0.05).The intraoperative parameters like operative time,radiation exposure and follow-up parameters like functional and radiological outcomes were recorded and compared.Statistical tests like the independent samplest-test Fischer’’s exact and Chi-square test were used to analyze association.Results:The distribution of the fractures according to AO/OTA classification and 31A1.2 type of intertrochanteric fractures were most common in our study.All the included fractures united and the average functional outcome in both groups were good and comparable at the end of one year.The operative time (mL,107.1 ± 12.6vs.77.0 ± 12.0,p < 0.001) and radiation exposure (s,78.6 ± 11.0vs.40.3 ± 9.3,p < 0.001) were significantly less among the patients in group B.Fracture consolidation,three months after the operative procedures,was seen in a significantly greater proportion of patients in group B (92.6%vs.67.7%,p = 0.025).Hardware irritation because of distal locking bolt was exclusively seen in group A,however this was not statistically significant (p = 0.241).Conclusion:We conclude that,in fixation of stable intertrochanteric fractures by long PFNA-II nail,distal locking not only increases the operative time and radiation exposure but also delays the fracture consolidation and increases the chances of hardware irritation,and hence is not required.Atmananda Hegde Vikrant Khanna Prajwal Mane Chethan Shetty Nitin Joseph 2023Chinese Journal of Traumatology2023,26,2:1

3Paraneoplastic Leukocytosis and Thrombocytosis as Prognostic Biomarkers in Non-small Cell Lung Cancer显示文摘Background and Objectives: Search for inexpensive laboratory markers have identified associations between blood counts and lung cancer outcomes. In this study, we evaluated the prognostic value of paraneoplastic leukocytosis(p-Leukocytosis) and paraneoplastic thrombocytosis(p-Thrombocytosis) in patients with non-small cell lung cancer(NSCLC). We also studied their relation to the expression of commonly detected molecular markers. Methods: We conducted a retrospective chart review on 571 consecutive NSCLC patients over a 10 year period. Blood counts were recorded at the time of cancer diagnosis. Kaplan-Meier survival curves were used to compare overall survival(OS) between patients with and without p-Leukocytosis(or) p-Thrombocytosis(p-Leuko/Thrombocytosis). Cox regression was used to determine if leukocytosis/thrombocytosis was a predictor of OS in NSCLC.Results: Patients with p-Leukocytosis and p-Thrombocytosis had a significantly poorer survival compared patients with normal blood counts(P<0.001). In a multivariate survival analysis, both continued to correlate even when adjusted for histology, gender, stage and chemotherapy(P<0.01, 0.03 respectively). Stage I and II NSCLC with p-Leuko/Thrombocytosis did not perform poorly compared to stage I/II NSCLC patients without paraneoplasia. Patients with the combined leukothrombocytosis syndrome did not have worse outcomes compared to those with either paraneoplastic syndrome alone. Conclusions: p-Leuko/Thrombocytosis is an accessible laboratory parameter of prognostic value in NSCLC. Evidence of p-Leuko/Thrombocytosis portends poor survival. The role of various cytokines in tumor pathobiology provides a rationale for identifying cytokine factors responsible for the paraneoplasia and administering anti-cytokine therapies alongside traditional chemotherapy in an attempt to improve survival outcomes in these subset of patients.Prajwal BODDU Dana VILLLINES Mebea AKLILU 2016中国肺癌杂志2016,19,11:1

6Preoperative estimation of humerus intramedullary nail length using clinical landmarks显示文摘Purpose:Intramedullary interlocking nailing is one of the accepted methods of treating humerus diaphyseal fractures.Appropriate nail length and diameter are of paramount importance to achieve a stable fracture fixation.Estimating the nail length can be as challenging in certain cases as it is important.This study aims to provide an easy-to-use formula utilizing clinical measurements from contra lateral arm to accurately estimate humeral nail length.Methods:This descriptive cross-sectional study was conducted at 3 tertiary care hospitals in Mangalore,India.Patients above the age of 18 years coming to the outpatient department with elbow,shoulder or arm complaints requiring radiological investigation from July 2021 to July 2022 were included.Patients with fractures or dislocations of upper limbs,malunited or non-united fractures of upper limbs,congenital or developmental deformities and patients with open growth plates were excluded.Patients'variables(like age and gender),radiological humerus length and contralateral arm clinical measurements were recorded.An independent samples t-test was used for univariate analysis,and linear regression analysis was done to estimate the desired nail length using the clinical measurement of the humerus(cm)in both genders separately.The significance level was set at p<0.05.Results:Our study included 204 participants of which 108 were male and 96 were female.The formula for predicting humeral nail length in males is(-2.029)+(0.883×clinical measurement).The formula for females is 1.862+(0.741×clinical measurement).A simplified formula to determine humeral nail length is 0.9 clinical length-2 cm(in males)and 0.7×clinical length+2 cm(in females).Conclusion:To improve the stability of fixation with intramedullary nails it is imperative to select the appropriate nail length.There have been studies that devised reliable methods of determining nail lengths in the tibia and femur using preoperative clinical measurements.A similar clinical method of determining humeral nail length is lacking in the literature.Our study was able to correlate radiological lengths of the humerus medullary canal with clinical measurements performed using anatomical landmarks to arrive at a formula.This allows for a reliable and easy nail length determination preoperatively.Atmananda S.Hegde Chethan B.Shetty Nitin Joseph Prajwal Mane Samarth Thakkar 2023Chinese Journal of Traumatology2023,26,5:0
7Ets-2 deletion in myeloid cells attenuates IL-1α-mediated inflammatory disease caused by a Ptpn6 point mutation显示文摘The SHP-1 protein encoded by the Ptpn6 gene has been extensively studied in hematopoietic cells in the context of inflammation.A point mutation in this gene(Ptpn6spin)causes spontaneous inflammation in mice,which has a striking similarity to neutrophilic dermatoses in humans.Recent findings highlighted the role of signaling adapters and kinases in promoting inflammation in Ptpn6spin mice;however,the underlying transcriptional regulation is poorly understood.Here,we report that SYK is important for driving neutrophil infiltration and initiating wound healing responses in Ptpn6spin mice.Moreover,we found that deletion of the transcription factor Ets2 in myeloid cells ameliorates cutaneous inflammatory disease in Ptpn6spin mice through transcriptional regulation of its target inflammatory genes.Furthermore,Ets-2 drives IL-1α-mediated inflammatory signaling in neutrophils of Ptpn6spin mice.Overall,in addition to its well-known role in driving inflammation in cancer,Ets-2 plays a major role in regulating IL-1α-driven Ptpn6spin-mediated neutrophilic dermatoses.Sarang Tartey Prajwal Gurung Rajendra Karki Amanda Burton Paul Hertzog Thirumala-Devi Kanneganti 2021Cellular & Molecular Immunology2021,18,7:0
8Standardization and Quality control parameters of Kayam Churna显示文摘Kayam Churna is proprietary ayurvedic formulation which is blend of seven ingredients which are Senna leaves(Cassia angustifolia),Black Salt,Mulethi(Glycyrrhiza glabra),Nishoth(Operculina turpethum),Ajwain(Trachyspermum ammi),Haritaki(Terminalia chebula)and Svarjikshara(Sodium bicarbonate)with Senna as a major ingredient.Standardization and stability studies play a vital role for each herbal formulation,so the research was carried out to prepare the formulation in the laboratory and to standardize the Churna,by means of its physicochemical properties,phytochemical analysis and microbiology study.The organoleptic and powdered characteristics were studied and compared with monographs.Various physicochemical analysis was carried out like bulk density,tapped density,percent compressibility,loss on drying,ash value and heavy metal determination.Phytochemical analysis was carried out to find the presence of tannins,alkaloids,glycosides,flavonoids and steroids.Microbiological analysis was evaluated using the Total Viable Count method.HPTLC Chromatographic technique was used for fingerprinting.Microscopic analysis of Formulation showed the presence of Vessels,Fibre,Calcium oxalate crystals,etc.Tannins,Alkaloids,flavonoids were found to be present in the formulation.Microbial count and all physicochemical parameters were found to be within the limits as per the WHO guidelines.Further studies can be carried out for finding out its efficacy and potency.Prajwal Gupta Ashwini Kendle Diksha Bhagat Mahera Khan Prabuddha Wakode Shruti Shah Sonali Patil 2022TMR Modern Herbal Medicine2022,5,3:0
