| 2 | 世界家庭医生组织(WONCA)研究论文摘要汇编——基层医疗机构应用直肠指检筛查前列腺癌的系统综述及Meta分析显示文摘目的尽管临床上经常应用直肠指检筛查前列腺癌,但是关于其在基层医疗机构中的应用的文献报道还为数尚少。本文旨在对在基层医疗机构中应用直肠指检筛查前列腺癌准确性的研究进行综述及Meta分析。方法检索Medline、EMBase、DARE(效果评价研究综述摘要数据库)、Cochrane临床对照试验数据库、Cochrane系统综述数据库、CINAHL(护理和联合卫生文献累积索引)等数据库中有关基层医疗机构中应用直肠指检筛查前列腺癌的研究,检索时间范围为自建库起至2016年6月。6例研究人员分为3组分别进行文献筛选和数据提取。对纳入文献中的直肠指检筛查前列腺癌的敏感性、特异性、阳性预测值、阴性预测值进行逆方差meta分析。采用诊断准确性研究质量评价工具QUADAS-2和GRADE指南对纳入文献的偏倚风险和质量进行评估。结果初步检索出相关文献8 217篇,最终纳入7篇,共计9 241例患者。所有患者均做了直肠指检和活检。直肠指检的敏感性为0.51[95%CI(0.36,0.67);特异性为0.59[95%CI(0.41,0.76);阳性预测值为0.41[95%CI(0.31,0.52);],阴性预测值为0.64[95%CI(0.58,0.70);]。GRADE评价表明,文献质量很低。结论鉴于缺乏证据支持其准确性,本研究不建议基层医疗机构常规应用直肠指检筛查前列腺癌。 | NAJI L RANDHAWA H SOHANI Z | 2018 | 中国全科医学2018,21,16: | 8 |
