4儿童社区获得性脓毒性休克后身体和心理社会健康相关生活质量改变的轨迹和危险因素显示文摘目的评估社区获得性脓毒性休克后1年内儿童健康相关生活质量(HRQL)中身体和心理社会领域的情况,并探索不良身体和心理社会HRQL预后的相关因素。设计儿童脓毒症后生活评估二次分析(secondary analysis of the life after pediatric sepsis evaluation,LAPSE)。场所美国12家学术型PICUs。对象家庭监护者认为不存在严重基础发育残疾且自社区获得性脓毒性休克住院中存活的≥1月且<18岁患儿。干预措施家庭监护者完成患儿基线(反映入院前状态),入院后7 d及1,3,6和12个月时的儿童生活质量问卷(Pediatric Quality of Life Inventory,PedsQLTM,2~18岁儿童)或PedsQLTM婴儿量表(<2岁)。较高的PedsQLTM身体和心理社会健康总结得分表明HRQL较好。测量方法与主要结果 204名儿童中58名(28.2%)有复杂慢性合并症。复杂慢性合并症患儿基线身体HRQL(62.7±22.6 vs.84.1±19.7,P<0.001)和心理社会HRQL(68.4±14.1 vs.81.2±15.3,P<0.001)明显低于参考常量,而未合并慢性病的患儿基线得分接近参考常量。合并复杂慢性病的儿童恢复到基线HRQL,而未合并患儿则未恢复(身体HRQL 75.3±23.7 vs 83.2±20.1,P=0.008;心理社会HRQL 74.5±18.7 vs 80.5±17.9,P=0.006)年龄<2岁与较高的12月时身体HRQL独立相关,PICU住院期间被医护人员怀疑神经损伤和存在神经检查异常与较低的12月时身体HRQL独立相关。针对颅内压增高的治疗以及1月时应用医疗装置和较低的12月时心理社会HRQL独立相关。结论尽管许多患儿可恢复到基线水平,社区获得性脓毒性休克患儿随后1年内的身体和心理社会HRQL仍较参考常量降低。HRQL较差的危险因素包括住院期间发生神经系统并发症以及入院后一月时仍依赖医疗装置。方伯梁(译) 钱素云(校) Meert KL Reeder R Maddux A B 2020中国小儿急救医学2020,27,9:1
