	11	Effectiveness of repetitive transcranial magnetic stimulation over sham therapy in major depression: a systematic review and Meta analysis of randomized controlled trials显示文摘Background: Repetitive transcranial magnetic stimulation (rTMS) is an emerging treatment option in psychiatry for major depressive disorder and many randomized control trials has been carried out for the same but the results from individual studies are not quite convincing to arrive at a conclusion. Aim: To evaluate the effectiveness of r TMS in Major depressive Disorder. Method: We conducted a systematic review of randomized control trials that evaluated the effectiveness of left dorsolateralprefrontal cortex (DLPFC) high frequency r T.M.S over r T.M.S with sham in patients with depression. The quality of the included studies was assessed and a metaanalysis was carried out. Results: Nine potential studies meeting the inclusion criteria were selected. The primary outcome of depression measured by HAM-D score was subjected to pooled analysis (standard mean difference).the pooled effect shown a significant reduction in the HAM-D score -(0.67 [-1.04,-0.30]) for patients with active r T.M.S over r T.M.S with sham. Conclusion: The trials included were heterogeneous as the I2 and chi-square statistics were high so the results should be interpreted with the limitations and heterogeneous nature of the sample even though a random effect model has been applied for metaanalysis. The nurse practitioner should frame roles for independent administration of rTMS and the effectiveness of the therapy should be studied and documented.
