| 1 | 反置式肩关节成形术:择期手术的当前趋势显示文摘反置式肩关节成形术(reverse shoulder arthroplasty, RSA)在近些年才被广泛认可为一种成功术式,但是其概念很早就已出现。20世纪70年代初,为了应对治疗肩袖撕裂性肩关节病(cufftear arthropathy,CTA)所面临的挑战,出现了一些反置式肩关节成形装置,但均以失败告终。Grammont医生团队自1985年至1995年,为治疗CTA设计并提出了现代成功的反置式人工肩关节假体。内移和远置肩关节旋转中心是Grammont反置式肩关节假体最为突出的设计原则。RSA手术数量在过去十年中呈指数级增长,目前适应证除了CTA以外,还包括巨大不可修复性肩袖撕裂、肱骨近端骨折或骨折后遗症、炎症性肩关节病、肩胛盂形态异常引起的骨关节炎、解剖型全肩或半肩失败后翻修以及肩部肿瘤。RSA的绝对禁忌症包括感染、完全性腋神经瘫痪、神经性肩关节病以及严重肩胛盂骨侵蚀或缺损。随着RSA应用的迅速增长,术后并发症和再手术量明显增加。RSA手术并发症可以分为非特异性并发症、与RSA相关的特异性并发症以及与假体品牌相关的特殊并发症。RSA手术中对肩胛盂骨缺损的处理极具挑战性,目前处理方法有三种:肩胛盂不对称打磨和使用外移的肩胛盂假体,肩胛盂植骨,以及使用加强型肩胛盂基座。近年来,随着3D打印技术的广泛应用,对RSA患者进行术前个体化定制假体植入辅助工具以进行精准外科治疗,并已经开始了商品化应用。骨科医生应当选择好RSA的手术适应证,把握其手术要领和技术细节,努力学习并掌握新技术,降低RSA手术并发症并提高其临床疗效。 | 陆博 Marius M. | 2 | 中国骨科SCI论文3年现状分析显示文摘目的:了解和分析近3年中国骨科国际论文的发展现状,分布情况和主要特点。方法:使用Web of Science网站检索近3年SCI收录的中国骨科论文,根据论文数,按照科研机构和来源期刊进行排序;通过引用频次排序,获取引用数高的论文,并进行分析。结果:近3年分别有1238篇,1264篇和1225篇来自中国的骨科论文被SCI数据库收录,上海交通大学(n=75)和Eur Spine J(n=114)为发表中国论文数量最多的机构和杂志。纳入的143篇高引论文中,发表最多的研究类型为临床论著(n=70)和系统综述/meta分析(n=50)。高引临床论著涉及最多的主题为'膝',高引系统综述/meta分析最多主题为'脊柱'。结论:2015—2017年,上海交通大学和Eur Spine J发表最多中国骨科SCI论文;高引论文中,涉及最多的主题为'脊柱'和'膝',论文类型分布不均衡,主要集中在临床论著和系统综述。 | 孙静 Marius M Scarlat 李银平 胡永成 | 2018 | 中国中西医结合外科杂志2018，24，3： | 3 |
