| 1 | Role of bacterial and genetic factors in gastric cancer in Costa Rica显示文摘AIM: To evaluate several risk factors for gastric cancer (GC) in Costa Rican regions with contrasting GC incidence rate (GCIR). METHODS: According to GCIR, 191 Helicobacter pylori (H pylori)-positive patients were classified into groups A (high GCIR, n = 101) and B (low GCIR, n = 90). Human DNA obtained from biopsy specimens was used in the determination of polymorphisms of the genes coding for interleukin (IL)-1β and IL-10 by PCR-RFLP, and IL-1RN by PCR. H pylori DNA extractions obtained from clinical isolates of 83 patients were used for PCR-based genotyping of H pylori cagA, vacA and babA2. Human DNA from gastric biopsies of 52 GC patients was utilized for comparative purposes. RESULTS: Cytokine polymorphisms showed no association with GCIR variability. However, gastric atrophy, intestinal metaplasia and strains with different vacA genotypes in the same stomach (mixed strain infection) were more frequently found in group A than in group B, and cagA and vacA s1b were signif icantly associated with high GCIR (P = 0.026 and 0.041, respectively). IL- 1β+3954_T/C (OR 2.1, 1.0-4.3), IL-1RN*2/L (OR 3.5, 1.7-7.3) and IL-10-592_C/A (OR 3.2, 1.5-6.8) were individually associated with GC, and a combination of these cytokine polymorphisms with H pylori vacA s1b and m1 further increased the risk (OR 7.2, 1.4-36.4). | 2 | Intraoperative laparoscopic complications for urological cancer procedures显示文摘AIM: To structure the rate of intraoperative complications that requires an intraoperative or perioperative resolution. METHODS: We perform a literature review of Medline database. The research was focused on intraoperative laparoscopic procedures inside the field of urological oncology. General rate of perioperative complications in laparoscopic urologic surgery is described to be around 12.4%. Most of the manuscripts published do not make differences between pure intraoperative, intraoperative with postoperative consequences and postoperative complications. RESULTS: We expose a narrative statement of complications, possible solutions and possible preventions for most frequent retroperitoneal and pelvic laparoscopic surgery. We expose the results with the following order: retroperitoneal laparoscopic surgery(radical nephrectomy, partial nephrectomy, nephroureterectomy and adrenalectomy) and pelvic laparoscopic surgery(radical prostatectomy and radical cystectomy).CONCLUSION: Intraoperative complications vary from different series. | 3 | Pyonephrosis as a sign of sarcomatoid carcinoma of the renal pelvis显示文摘We report the case of an urgent nephrectomy because of a pyonephrosis and sepsis due to an unsuspected sarcomatoid transitional cell carcinoma, an infrequent subtype with a bad oncological prognosis. We present a 58-year-old man assessed by internal medicine for a general syndrome and weakness many months previously. A pyonephrotic kidney was observed at abdominal computed tomography in the context of septic shock, without suspecting the underlying cause. The pathology report described a sarcomatoid transitional cell carcinoma. Sarcomatoid transitional cell carcinoma is an invasive and infrequent subtype of urothelial tumors. | 7 | Fagus sylvatica and Quercus pyrenaica:Two neighbors with few things in common显示文摘Background:The Iberian Peninsula comprises one of the largest boundaries between Mediterranean and Eurosiberian vegetation,known as sub-Mediterranean zone.This ecotone hosts many unique plant species and communities and constitutes the low-latitude(warm)margin of numerous central European species which co-occur with Mediterranean vegetation.Two of the main species found in this region are the Eurosiberian European beech(Fagus sylvatica L.)and the Mediterranean Pyrenean oak(Quercus pyrenaica Willd.).It remains unclear how the different physiological and adaptive strategies of these two species reflect their niche partitioning within a subMediterranean community and to what extent phenotypic variation(intraspecific variability)is driving niche partitioning across Eurosiberian and Mediterranean species.Methods:We quantified functional niche partitioning,based on the n-dimensional hypervolume to nine traits related to resource acquisition strategies(leaf,stem and root)plus relative growth rate as an additional wholeplant trait,and the environmental niche similarity between Pyrenean oak and European beech.Further,we analyzed the degree of phenotypic variation of both target species and its relationship with relative growth rates(RGR)and environmental conditions.Plant recruitment was measured for both target species as a proxy for the average fitness.Results:Species’functional space was highly segregated(13.09%overlap),mainly due to differences in niche breadth(59.7%)rather than niche replacement(25.6%),and beech showed higher trait variability,i.e.,had larger functional space.However,both species shared the environmental space,i.e.,environmental niches were overlapped.Most plant traits were not related to abiotic variables or RGR,neither did RGR to plant traits.Conclusions:Both target species share similar environmental space,however,show notably different functional resource-use strategies,promoting a high complementarity that contributes to maintaining a high functionality in sub-Mediterranean ecosystems.Therefore,we propose that conservation efforts be oriented to preserve both species in these habitats to maximize ecosystem functionality and resilience. | Sergio de Tomás Marín Jesús Rodríguez-Calcerrada Salvador Arenas-Castro Iván Prieto Guillermo González Luis Gil Enrique G.de la Riva | 2023 | Forest Ecosystems2023,10,2: | 0 |
| 8 | Hydatid cyst of the gallbaldder: A systematic review of the literature显示文摘AIM To evaluate all the references about primary gallbladder hidatidosis looking for best treatment evidence.METHODS Search:1966-2015 in MEDLINE,Cochrane Library,SciELO,and Tripdatabase.Key words: 'gallabladder hydatid disease' and 'gallbladder hydatid cyst'.We found 124 papers in our searches but only 14 papers including 16 cases were about hydatid cyst of the gallbladder(GBHC).RESULTS Eight cases of GBHC were women and seven men.One not mentioned.Median age was 48.3 years.The most frequent clinical symptom was abdominal pain(94%) usually in the right upper quadrant.Ultrasound was performed in ten patients(62.5%) but in most cases a combination of several techniques was performed.The location of the cysts was intravesicular in five patients.Five patients presented GBHC and liver hydatid cysts.Two patients presented cholelithiasis and one choledocholithiasis.The most frequent surgical technique was cholecystectomy by laparotomy(81.25%).Simultaneous surgery of liver cysts was carried out in five cases.Eleven patients did not present postoperative complications,but one died.The mean hospital stay was seven days.No recurrence of GBHC was recorded.CONCLUSION In GBHC,the most frequent symptom is right hypocondrium pain(evidence level V).Best diagnostic methods are ultrasound and computed tomography(level V,grade D).Suggested treatment is open cholecystectomy and postoperative albendazole(level V,grade D) obtaining good clinical results and none relapses. | Roberto Gómez Yousef Allaoua Rafael Colmenares Sergio Gil Pilar Roquero José M Ramia | 2016 | World Journal of Hepatology2016,8,25: | 0 |
