1Single center experience of capsule endoscopy in patients with obscure gastrointestinal bleeding显示文摘AIM:To identify optimum timing to maximize diagnostic yield by capsule endoscopy (CE) in patients with obscure gastrointestinal bleeding (OGIB).METHODS:We identified patients who underwent CE at our institution from August 2003 to December 2009.Patient medical records were reviewed to determine type of OGIB (occult,overt),CE results and complications,and timing of CE with respect to onset of bleeding.RESULTS:Out of 385 patients investigated for OGIB,284 (74%) had some lesion detected by CE.In 222 patients (58%),definite lesions were detected that could unequivocally explain OGIB.Small bowel ulcer/erosions secondary to Crohn's disease,tuberculosis or non-steroidal anti-inflammatory agent use were the commonest lesions detected.Patients with overt GI bleeding for < 48 h before CE had the highest diagnostic yield (87%).This was significantly greater (P < 0.05) compared to that in patients with overt bleeding prior to 48 h (68%),as well as those with occult OGIB (59%).CONCLUSION:We established the importance of early CE in management of OGIB.CE within 48 h of overt bleeding has the greatest potential for lesion detection.Mahesh Kumar Goenka Shounak Majumder Sanjeev Kumar Pradeepta Kumar Sethy Usha Goenka 2011World Journal of Gastroenterology2011,17,6:39
2A deep learning-enabled portable imaging flow cytometer for cost-effective, highthroughput, and label-free analysis of natural water samples显示文摘We report a deep learning-enabled field-portable and cost-effective imaging flow cytometer that automatically captures phase-contrast color images of the contents of a continuously flowing water sample at a throughput of 100 mL/h.The device is based on partially coherent lens-free holographic microscopy and acquires the diffraction patterns of flowing micro-objects inside a microfluidic channel.These holographic diffraction patterns are reconstructed in real time using a deep learning-based phase-recovery and image-reconstruction method to produce a color image of each micro-object without the use of external labeling.Motion blur is eliminated by simultaneously illuminating the sample with red,green,and blue light-emitting diodes that are pulsed.Operated by a laptop computer,this portable device measures 15.5 cm×15 cm×12.5 cm,weighs 1 kg,and compared to standard imaging flow cytometers,it provides extreme reductions of cost,size and weight while also providing a high volumetric throughput over a large object size range.We demonstrated the capabilities of this device by measuring ocean samples at the Los Angeles coastline and obtaining images of its micro-and nanoplankton composition.Furthermore,we measured the concentration of a potentially toxic alga(Pseudo-nitzschia)in six public beaches in Los Angeles and achieved good agreement with measurements conducted by the California Department of Public Health.The cost-effectiveness,compactness,and simplicity of this computational platform might lead to the creation of a network of imaging flow cytometers for largescale and continuous monitoring of the ocean microbiome,including its plankton composition.Zoltán Gӧrӧcs Miu Tamamitsu Vittorio Bianco Patrick Wolf Shounak Roy Koyoshi Shindo Kyrollos Yanny Yichen Wu Hatice Ceylan Koydemir Yair Rivenson Aydogan Ozcan 2018Light(Science & Applications)2018,7,1:14
3Capsule endoscopy:Present status and future expectation显示文摘Video capsule endoscopy(CE)since its introduction 13years back,has revolutionized our approach to small intestinal diseases.Obscure gastrointestinal bleed(OGIB)continues to be the most important indication for CE with a high sensitivity,specificity as well as positive and negative predictive values.It is best performed during ongoing bleed or immediately thereafter.Overt OGIB has a higher diagnostic yield than occult OGIB.However,even in iron deficiency anemia,CE is emerging as important investigation after initial negative work up.In suspected Crohn’s disease(CD),CE has been shown superior to traditional imaging and endoscopic technique and should be considered after a negative ileocolonoscopy.Although CE has also been used for evaluating established CD,a high capsule retention rate precludes its use ahead of cross-sectional imaging.Celiac disease,particularly where gastro-duodenoscopy cannot be performed or is normal,can also be investigated by CE.Small bowel tumor,hereditary polyposis syndrome,and non-steroidal anti-inflammatory drugs induced intestinal damage are other indications for CE.Capsule retention is the only significant adverse outcome of CE and occurs mostly in presence of intestinal obstruction.This can be prevented by use of Patency capsule prior to CE examination.Presence of cardiac pacemaker and intracardiac devices continue to be relative contraindications for CE,though data do not suggest interference of CE with these devices.Major limitations of CE today include failure to control its movement from outside,inability of CE to acquire tissue for diagnosis,and lack of therapeutic help.With ongoing interesting and exciting developments taking place in these areas,these issues would be solved in all probability in near future.CE has the potential to become one of the most important tools in diagnostic and possibly in the therapeutic field of gastrointestinal disorder.Mahesh K Goenka Shounak Majumder Usha Goenka 2014World Journal of Gastroenterology2014,20,29:9

5Molecular detection of pancreatic neoplasia: Current status and future promise显示文摘Pancreatic cancer is usually diagnosed at an advanced stage and curative resection is feasible in only a small minority of patients at the time of diagnosis. Diagnosis at an early stage is unequivocally associated with better long-term survival. Several candidate molecular markers for early detection are currently under investigation in different phases of discovery and validation. Recent advances in the technology for whole genome,methylome,ribonucleome,and proteome interrogation has enabled rapid advancements in the field of biomarker discovery. 12Adaptive-atom whale optimization algorithm using autonomous mobile ad hoc network for energy-efficient multipath routing显示文摘Mobile ad hoc network(MANET)consists of a group of self-operated mobile nodes that form a network without considering predefined infrastructure.In the network,each node poses less energy due to battery-operated sensor nodes.Numerous methods are devised for routing,but each method suffered from the energy constraint.This paper proposes an energy-efficient multipath routing protocol for exploiting different routes between source and destination to mitigate energy constraints.The key idea is to determine the optimal path from a set of paths available between the source and the target node.To improve the security in routing protocol,the factors,like energy,trust,and mobility are considered as major components.Here,the trust is modeled by considering trust factors,like average encounter rate(AER),successful cooperation frequency(SCF),direct trust,integrity factor,and forwarding rate factor.Moreover,secure routing is performed between the nodes using the proposed Adaptive-Atom Whale Optimization algorithm(AdaptiveAWOA).The proposed (Adaptive-AWOA)is designed by incorporating the Adaptive concept in the Atom Whale Optimization algorithm(AWOA).The fitness function is newly modeled considering mobility,energy,and trust factors.The proposed AdaptiveAWOA outperformed the other methods with a minimal end-to-end delay of 0.009,maximal throughput of 85.802%and maximal packet delivery ratio(PDR)of 97.391%,respectively.Suresh R.Halhalli B.N.Jagdale Shounak Rushikesh Sugave 2020International Journal of Modeling, Simulation, and Scientific Computing2020,11,6:0
