| 1 | 主动脉夹层破裂出血沿肺动脉延伸的CT及临床表现显示文摘本研究的目的是评价主动脉夹层(AD)破裂出血沿肺动脉(PA)延伸的CT表现及其预后因素。在232例Stanford A型主动脉夹层病人中,诊断出21例(9.1%;女性11例;平均70-3岁)。12例病人具有双腔(经典型),9例病人有主动脉壁内血肿(IMH;闭合的假腔)。基于CT表现,出血可以分为3类:第1类,肺动脉IMH或环绕肺动脉的出血;第2类,出血延伸至小叶间隔;第3类,出血延伸至肺泡内。影响预后的因素包括CT表现和病人的临床特征。21例中有14例(66.7%)进行了急诊手术, | E. | 2 | E-cadherin and beta-catenin expression in Epstein-Barr virus-associated gastric carcinoma and their prognostic significance显示文摘AIM: To examine the role of E-cadherin and beta- catenin in carcinogenesis and to assess their prognostic implication in Epstein-Barr virus-associated gastric carcinomas (EBV-GCs). METHODS: We compared the frequency of E-cadherin and beta-catenin expression in 59 EBV-GCs and 120 non-EBV-GCs, and examined the association between patients' prognosis and the expressions of these proteins. RESULTS: Neither the cellular-membranous nor the cytoplasmic E-cadherin expression showed any difference between EBV-GCs and non-EBV-GCs. On the other hand, loss of membranous expression of beta- catenin occurred more frequently in non-EBV-GCs than EBV-GCs [odds ratio = 0.41; 95% confidence interval (CI), 0.19-0.90]. Furthermore, the nuclear and/or cytoplosmic expression of beta-catenin was seen more frequentlyin EBV-GCs than non-EBV-GCs (odds ratio = 2.23; 95% CI, 0.97-5.09), and was observed in a larger proportion of carcinoma cells of EBV-GCs than non-EBV-GCs (P = 0.024). Survival analysis for non-EBV-GC revealed that lymph node metastasis was significantly associated with poor prognosis (P < 0.001). Among EBV- GCs, the depth of invasion (P = 0.005), lymph node metastasis (P = 0.004) and an intestinal type by Lauren classification (hazard ratio = 9.47; 95% CI, 2.67-33.6) were significantly associated with poor prognosis. On the other hand, nuclear and/or cytoplasmic expression of beta-catenin was associated with a better prognosis in patients with EBV-GC (hazard ratio = 0.32; 95% CI, 0.11-0.93). CONCLUSION: We observed more frequent preservation of beta-catenin in cell membrane and accumulation in nuclei and/or cytoplasm in EBV-GCs than in non-EBV- GCs. | 3 | Numerical and experimental investigation of three-dimensionality in the dam-break flow against a vertical wall显示文摘The three-dimensionality extent of the dam break flow over a vertical wall is investigated numerically and experimentally in this paper. The numerical method is based on Reynolds averaged Navier-Stokes(RANS) equation that describes the three-dimensional incompressible turbulent flow. The free surface is captured by using the unstructured multi-dimensional interface capturing(UMTHINC) scheme. The equations are discretized on 2-D and 3-D unstructured grids using finite volume method. The numerical simulations are compared with newly conducted experiment with emphasis on the effect of three-dimensionality on both free surface and impact pressure. The comparison between the numerical and experimental results shows good agreement. Furthermore, the results also show that 3-D motion of the flow originates at the moment of impact at the lower corners of the impact wall and propagates to the inner region as time advances. | 5 | Fractionation of gamma-glutamyltransferase in patients with nonalcoholic fatty liver disease and alcoholic liver disease显示文摘AIM To assess how serum gamma-glutamyltransferase(GGT) fractions vary in patients with alcoholic liver disease(ALD) and non-alcoholic fatty liver disease(NAFLD). METHODS Serum samples were obtained from 14 patients with biopsy-proven alcoholic liver diseases and 9 patients with biopsy proven non-alcoholic fatty liver disease. In addition to these biopsy-proven cases, 16 obese(body mass index > 25) patients without any history of alcohol consumption but with a fatty liver on ultrasound examination and with elevated GGT were included for an additional analysis. Serum GGT fractionation was conducted by high-performance gel filtration liquid chromatography and was separated into the four fractions, big-GGT, medium-GGT, small-GGT(s-GGT), and free-GGT(f-GGT).RESULTS The results were expressed as a ratio of each fraction including the total GGT(t-GGT). The s-GGT/t-GGT ratioswere lowest for the control group and highest for the ALD group. The differences between the control and NAFLD groups and also between the NAFLD and ALD groups were statistically significant. In contrast, the f-GGT/t-GGT ratios were highest in the control group and lowest in the ALD group, with the differences being statistically significant. As a result, the s-GGT/f-GGT ratios were markedly increased in the NAFLD group as compared with the control group.
